A reinspection will be conducted to ensure the cooler has been moved and the hand sink is accessible. Do not use cooler for potentially hazardous food and beverages until gaskets have been replaced and cooler air temperature is 34 to 38 degrees. Sylvania Tam-O-Shanter Inc., 7060 W. Sylvania, Sylvania, inspected Dec. 12. No sanitizer in the sanitizer spray bottle. Set up sanitizer wiping cloth bucket with quat sanitizer before beginning to work. Store in-use wiping cloths in sanitizer bucket. Quat should test at 200 ppm. Change as needed. Sylvania Diner, 5623 W. Alexis, inspected Dec. 14. Dead roach by the under-counter cooler next to the cupboard with the silverware. Staff cannot find the latest pest control report. According to health department records, the last pest control treatment by Stanchin & Son’s Pest Control was Nov. 28. The facility was receiving weekly pest control treatments for roaches. Facility is seeing less and less roaches. The facility must keep up with pest control until the roach problem is eliminated. Once the roach problem is eliminated, maintain program and continue to clean all equipment, floors, and walls to prevent pests. No seven-day discard date labeling observed on open and prepared time/temperature control for safety foods. Unlabeled chemical spray bottle observed hanging by the dish machine. Label chemical spray bottles to allow employees to know contents. Store chemicals away from food and food-contact items to prevent contamination. Inspector: Gottschalk.
New Millennium, 4307 W. Sylvania, inspected Dec. 12. Hot dogs on the roller grill holding hot at 105 to 115 degrees. Such food must be held hot at 135 degrees or higher to inhibit bacteria growth. Adjust heat setting on the roller grill. Repeat violation. If this violation is repeated again, a public health order will be issued forbidding this facility to serve hot foods. Hot dogs must be discarded. Sausage biscuit sandwiches holding at 119 degrees in hot hold case. Sandwiches must be heated to at least 135 degrees in the microwave before they are placed into hot holding units. This piece of equipment is designed only to maintain foods at hot holding temperature, not to heat foods. Sandwiches must be discarded. No thermometer available for checking food. Provide thermometer to check food temperatures. Repeat violation. There is no ingredient statement available for the turnovers for sale as required. Repeat violation. Boxes of to-go bags and boxes of food-contact items stored on the floor; store such items at least six inches off floor to prevent contamination. The facility was licensed with an acid bath unit to wash and sanitize the racks in the smokers, but acid-bath unit has been discarded because it didn’t work. The facility at time of inspection lacks proper means to wash, rinse, and sanitize racks from the smokers. Cafe Marie, 6819 W. Central, inspected Jan. 11. Small pan of chicken cooling with lid on container in the under-counter cooler. Follow proper cooling requirements. Cut sausage double-stacked in the top section of the prep-top cooler. The sliced sausage had a temperature of 54 degrees. Do not double-stack food containers in the top section of the prep-top cooler. Sausage was discarded. Raw steak stored over the bag of chocolate chips in the prep-top cooler. Raw meat must be stored on the bottom below all ready-to-eat food and produce to protect from cross contamination. Inspector: Gottschalk.

Gravy being reheated in the steam table. Foods must be reheated to at least 165 degrees on a direct heat source before being placed into the steam table for hot holding. Cleaning products stored above bulk food bins. Store chemicals away from or below food and food-prep areas. Raw chicken stored above broccoli in the walk-in cooler. Rally’s, 3033 Cherry, inspected Jan. 5. Hot holding unit on line not turned on; must be turned on before placing food inside it. The unit was holding time and temperature controlled for safety foods at 95 degrees. Also, hot dogs on line in drawer at 110 degrees. Hot hold at 135 degrees or higher to prevent bacteria growth. Hot dogs were grilled back to 165 degrees. Food at 45 degrees in walk-in cooler. The ambient air temperature of cooler was at 46 degrees. Cold hold at 41 degrees or lower to prevent bacteria growth. Licensed repairman was contacted to repair cooler. Lukewarm water on both the three-compartment sink and hand sink. Hot water must be available for proper hand washing and ware washing. Contact licensed plumber to fix problem. Observed water leak in the hand sink trap. Plumbing needs to be free of leaks and in good working condition. Contact licensed plumber to make repair.
