Kentucky Fried Chicken, 5649 W. Alexis, inspected Dec. 7. Chicken in the top section of the warmer hot holding at 118 to 132 degrees. The chicken is held for 1½ hours after cooking, then discarded. Also found shredded barbecue chicken at 124 degrees in the warmer by the fryers. Time/temperature control for safety foods must be held hot at 135 degrees or higher to prevent bacteria growth. Cinco De Mayo Amigos, 6975 W. Central, inspected Dec. 7. Prep-top cooler is blocking hand sink. Repeat violation. Move prep cooler. No paper towels at the bar hand sink. Hand sinks must be accessible and stocked at all times to allow employees to properly wash their hands which is very important in preventing contamination. A reinspection will be conducted to ensure the cooler has been moved and the hand sink is accessible. Raw shell eggs stored above vegetables, raw hamburger stored above vegetables, and raw shrimp stored above cooked beef in the walk-in cooler. Follow proper food storage rules to prevent cross contamination. Repeat violation. Inconsistent date labeling observed in walk-in cooler. Caribou Coffee, 6975 W. Central, inspected Dec. 7. Under-counter cooler has an air temperature of 53 degrees. Open smoothie drink mix discarded at time of inspection. Cooler is not maintaining food and beverages at proper temperature. New gaskets have been ordered. Do not use cooler for potentially hazardous food and beverages until gaskets have been replaced and cooler air temperature is 34 to 38 degrees. No paper towels available at the back hand sink by the three-bay sink. Stock hand sinks with soap, paper towels, and hot/cold running water. Inspector: Gottschalk.

Ralphie’s Franklin Park, 5702 Monroe, Sylvania, inspected Dec. 13. Found time/temperature control for safety foods above 41 degrees in the prep-top cooler. This is a new cooler. Temperature was adjusted to proper cooling level.Salad dressings in an ice bath at 43 degrees and 53 degrees. Make sure level of ice and food match. Dressing was discarded. Inspector: Gottschalk.

Sylvania Tam-O-Shanter Inc., 7060 W. Sylvania, Sylvania, inspected Dec. 12. No sanitizer in the sanitizer spray bottle. Set up sanitizer wiping cloth bucket with quat sanitizer before beginning to work. Store in-use wiping cloths in sanitizer bucket. Quat should test at 200 ppm. Change as needed. Cookie dough and milk stored in the Coke cooler. The Coke cooler is a beverage cooler and is not approved to cold hold foods. Inspector: Gottschalk.

Sylvania Diner, 5623 W. Alexis, inspected Dec. 14. Dead roach by the under-counter cooler next to the cupboard with the silverware. Staff cannot find the latest pest control report. According to health department records, the last pest control treatment by Stanchin & Son’s Pest Control was Nov. 28. The facility was receiving weekly pest control treatments for roaches. Facility is seeing less and less roaches. The facility must keep up with pest control until the roach problem is eliminated. Once the roach problem is eliminated, maintain program and continue to clean all equipment, floors, and walls to prevent pests. No seven-day discard date labeling observed on open and prepared time/temperature control for safety foods. Unlabeled chemical spray bottle observed hanging by the dish machine. Label chemical spray bottles to allow employees to know contents. Store chemicals away from food and food-contact items to prevent contamination. Inspector: Gottschalk.

New Millennium, 4307 W. Sylvania, inspected Dec. 12. Hot dogs on the roller grill holding hot at 105 to 115 degrees. Such food must be held hot at 135 degrees or higher to inhibit bacteria growth. Adjust heat setting on the roller grill. Repeat violation. If this violation is repeated again, a public health order will be issued forbidding this facility to serve hot foods. Hot dogs must be discarded. Sausage biscuit sandwiches holding at 119 degrees in hot hold case. Sandwiches must be heated to at least 135 degrees in the microwave before they are placed into hot holding units. This piece of equipment is designed only to maintain foods at hot holding temperature, not to heat foods. Sandwiches must be discarded. No thermometer available for checking food. Provide thermometer to check food temperatures. Repeat violation. There is no ingredient statement available for the turnovers for sale as required. Repeat violation. Raw eggs stored above ready-to-eat pickles and bottled water in the walk-in cooler. To prevent cross contamination, store raw eggs or meats below ready-to-eat foods and beverages. Repeat violation. Hand-washing sink blocked by ice bucket. This sink must be clear and accessible so that food handlers can properly wash their hands. Squeeze bottle of mayonnaise stored at room temperature with condiments. Package is labeled “refrigerate after opening” and must be held cold at 41 degrees or lower. Discard this bottle. Boxes of to-go bags and boxes of food-contact items stored on the floor; store such items at least six inches off floor to prevent contamination. The facility was licensed with an acid bath unit to wash and sanitize the racks in the smokers, but acid-bath unit has been discarded because it didn’t work. The facility at time of inspection lacks proper means to wash, rinse, and sanitize racks from the smokers. Facility must install an approved method to properly wash, rinse, and sanitize these racks to remove food debris. Inspector: Gottschalk.

Cafe Marie, 6819 W. Central, inspected Jan. 11. Small pan of chicken cooling with lid on container in the under-counter cooler. Follow proper cooling requirements. Cut sausage double-stacked in the top section of the prep-top cooler. The sliced sausage had a temperature of 54 degrees. Do not double-stack food containers in the top section of the prep-top cooler. Sausage was discarded. Raw steak stored over the bag of chocolate chips in the prep-top cooler. Raw meat must be stored on the bottom below all ready-to-eat food and produce to protect from cross contamination. Inspector: Gottschalk.

Panda Chinese Restaurant, 1531 Sylvania, inspected Jan. 10. No sanitizer detected in the wipe cloth bucket. Change sanitizer as needed to maintain chlorine bleach at 50 to 100 ppm. Gravy being reheated in the steam table. Foods must be reheated to at least 165 degrees on a direct heat source before being placed into the steam table for hot holding. Cleaning products stored above bulk food bins. Store chemicals away from or below food and food-prep areas. Raw chicken stored above broccoli in the walk-in cooler. Store raw meats, poultry, and eggs below or away from ready-to-eat foods to prevent cross contamination. Inspector: Sattler.

Rally’s, 3033 Cherry, inspected Jan. 5. Hot holding unit on line not turned on; must be turned on before placing food inside it. The unit was holding time and temperature controlled for safety foods at 95 degrees. Also, hot dogs on line in drawer at 110 degrees. Hot hold at 135 degrees or higher to prevent bacteria growth. Hot dogs were grilled back to 165 degrees. Food at 45 degrees in walk-in cooler. The ambient air temperature of cooler was at 46 degrees. Cold hold at 41 degrees or lower to prevent bacteria growth. Licensed repairman was contacted to repair cooler. Lukewarm water on both the three-compartment sink and hand sink. Hot water must be available for proper hand washing and ware washing. Contact licensed plumber to fix problem. Observed water leak in the hand sink trap. Plumbing needs to be free of leaks and in good working condition. Contact licensed plumber to make repair.
