Oregon refinery fined over safety issues

Toledo Refining Company LLC has been fined $21,000 by the Occupational Safety and Health Administration for violations at its facility on Woodville Road in Oregon.

OSHA said the company did not document that equipment complied with recognized and generally accepted engineering practices, did not correct deficiencies that were outside acceptable limits, and did not implement written operating procedures that provided clear instructions for certain procedures.

The inspection was conducted Jan. 31. The company has 15 days to pay the fine or contest it.
