VAN WERT, Ohio — A woman who drove into the path of an oncoming tractor-trailer in a rural part of Van Wert County on Monday was pronounced dead at the accident scene, the Ohio Highway Patrol post in Van Wert said.

Carole B. Dunaway, 76, of Centerville, Ohio, was heading north on U.S. 127 near Walnut Grove Church Road in Van Wert County’s York Township about 12:20 p.m., when she crossed the center line in her car and collided with the semi head-on, officials said.

The crash site is about six miles south of Van Wert.

The tractor-trailer was driven by Ben A. Bernard, 69, of Zanesville, Ohio. He was treated for minor injuries at Van Wert County Hospital.
