Recently released inspection reports of Lucas County food-service operations

Arbors of Sylvania, 7120 Port Sylvania, Sylvania, inspected Aug. 21. Critical violations: vegetable prep sink needs 2-inch air gap on waste plumbing line. Make buckets with correct sanitizer concentration for wiping cloths. Inspector: Herzberg.

New Empire Restaurant, 6546 Centers, Springfield Township, inspected Aug. 21. Critical violations: do not use 3-vat sink for food prep. Thaw shrimp and other foods under cold running water, in food prep sink. Soak and clean pop nozzles and holsters daily. Maintain shelf stock labels and records for at least 90 days. Inspector: H. Sanders.

Positive Beginnings, 401 Fassett, inspected Aug. 22. Critical violation: replace all homestyle equipment with commercially approved equipment when it breaks down. Inspector: Gottschalk

Avva's Kitchen, 7229 West Central, Sylvania Township, inspected Aug. 22. Critical violations: store racks of utensils and glasses at least 6 inches off floor. Adjust thermostat of cooler or get it repaired to keep proper cool temperature of 41 degrees. Store bread in plastic container, not in cardboard box. keep back door tightly closed to keep out insects. Chemicals must not be stored with utensils, corrected. Inspector: Herzberg.

Lucky Dollar, 1713 Jefferson, inspected Aug. 18. Critical violation: date mark all pre-made sandwiches and sandwiches being sold, with the date they are taken from the freezer. Inspector: Debbie Dacquisto.

Giant Eagle, 6930 West Central, Sylvania Township, inspected Aug. 20. Critical violations: glass doors for pizza hot holding case are nicked; new doors on order. Do not store insulated catering bags in meat cooler with raw meat. Inspector: Herzberg.

Edgewater Bait and Tackle, 3258 131st, inspected Aug. 21. Critical violations: food products must not be stored below raw cut fish. Also, remove cardboard. Inspector: Gottschalk.

Chili's Grill and Bar, 4801 Talmadge, inspected Aug. 24. Critical violations: prep-top line end coolers not keeping proper temperatures. Use only food safe chemicals in the establishment; contact pest control company, corrected. Inspector: Gottschalk.

Doc Watsons, 1515 South Byrne, inspected Aug. 24. Critical violations: date mark all opened or prepared food items with an expiration date. Thoroughly clean and sanitize interior of ice machine. Inspector: Jeff Neidstadt.

Bob Evans Restaurant, 2433 South Reynolds, inspected Aug. 24. Critical violations: single-service packaged butters stored in pans of other foods; cross contamination possible. Keep food items covered in the walk-in cooler and reach-in refrigerators. Keep all wipe rags in bucket of sanitizer in between use. Inspector: Neistadt.

China Buffet, 2325 South Reynolds, inspected Aug. 24. Critical violations: keep food items and food pans covered in walk-in cooler and reach-in refrigerators. Store all food items 6 inches or more off floor. Keep hot food at above 140 degrees at all times on buffet table. Label all chemical spray bottles. Inspector: Neistadt.

Charlie's, 12401 Airport, Swanton Township, inspected Aug. 21. Critical violation: don't use three-vat sink for food prep; use prep sink. Inspector: Sara Becker.

Cousino's Steak House, 1842 Woodville, inspected Aug. 25. Critical violations: provide soap dispenser for hand sink in kitchen and handsink behind bar, and soap and paper towels for the dishwashing person in the dish area. All potentially hazardous food items and dressings need to be marked with date of preparation or opening and discarded after 4 days at 45 degrees. Food out of temperature in walk-in and dressing prep top; call maintenance to evaluate unit. Inspector: Jacklyn Kistler.

Magic Wok, 2829 Airport, inspected Aug. 26. Critical violation: a person in charge who has basic knowledge of food safety must be present at all times. Inspector: Becker.

Magic Wok, 2040 West Laskey, inspected Aug. 26. Critical violations: fix soap dispenser by handsink and provide handsoap. If a hose is to be connected to mop sink, obtain back flow prevention device. Label all bulk containers in storage area with name of bulk product. Keep all food items covered in walk-in cooler. Clean and sanitize metal shelving unit used to transport shallow pans. All prepared or opened food items in walk-in cooler must be date-marked if not used within 24 hours. Obtain and use commercial grade food scoop with a handle; keep handles stored outside of food product. Inspector: Neistadt.

Chi-Chi's Mexican Restaurant, 4038 Talmadge, inspected Aug. 26. Critical violations: label all containers with bulk foods and spices. Obtain backflow prevention device for mop sink if hose is to be connected to it. Thoroughly clean and sanitize interior of ice machine by the bars. Inspector: Neistadt.

Grape Leaf Diner, 909 South McCord, Springfield Township, inspected Aug. 27. Critical violation: keep sanitizer rags in sanitizer solutions at all times between use. Inspector: Sanders.

KFC, 3506 Dorr, inspected Aug. 28. Critical violation: thoroughly clean mixer bowl of food build up. Inspector: Sanders.

Gladieux Enterprises, 11013 Airport, Swanton Township, inspected Aug. 28. Critical violations: all potentially hazardous foods must be date-marked with 7-day expiration date and discarded after those 7 days. Thoroughly clean and sanitize fruit and bagel area of fruit fly issue; contact pest control for options. Inspector: Becker.

Big Boy, 5820 West Central, inspected Aug. 28. Critical violations: do not store ice scoop in direct contact with top of ice maker; store in washable container. Pressurized CO2 tanks must be chained to prevent falling. Managers must wear hat or hairnet while preparing food, corrected. Sanitizer in wiping cloth bucket was too strong, corrected. Inspector: Herzberg.

Beaner's Coffee, 4031 North McCord, Sylvania, inspected Aug. 28. Critical violations: clean dust from portable ventilation fan and from cooling unit above walk in cooler. One employee preparing food and coffee was not wearing hair restraint, corrected. Inspector: Herzberg.

Washington Jr. High School, 5700 Whitmer, inspected Aug. 28. Critical violation: install handsink for proper hand washing; handsink scheduled to be installed. Inspector: Neistadt.

Collins Golf Course, 624 Reineck, inspec|ted Aug. 28. Critical violation: provide discard date on open food; no date marking observed. Inspector: Gottschalk.

Harborside Health Care, 5757 Whiteford, inspected Sept. 1. Critical violations: all personal items must be stored in designated area away from food storage or clean and sanitized items. Only pasteurized eggs or egg substitutes allowed in recipes to this population. Inspector: Karim Baroudi.

Max and Erma's, 7060 West Central, Sylvania Township, inspected Aug. 28. Critical violation: tortilla soup and Italian wedding soup in cooler not cool enough, discarded. Inspector: Herzberg.

McDonald's, 7159 West Central, Sylvania Township, inspected Aug. 26. Critical violation: clean dust from wire grills over fans in cooling units in walk-in cooler. Inspector: Herzberg.

Avva's Kitchen, 7229 West Central, Sylvania Township, inspected Aug. 29. Critical violations: ice scoop should be stored in ice bucket or on clean tray. Do not use plastic gallon milk jugs to store drink mix; use multi-use washable container, corrected. Raw foods must be stored in proper order below vegetables and cooked foods, corrected. All wiping cloths must be kept in sanitizer bucket. No sanitizer on premises. Inspector: Herzberg.
