Recently released inspection reports of Lucas County food-service operations:

Critical violations

Loyal Order of Moose, 226 South Byrne, inspected June 9. Observed mold/mildew build up in soda dispenser holder. Being corrected at time of inspection. Properly sanitize holder on a routine basis. No sanitizing test strips in kitchen area, bar area had test strips present. Inspector: Patrick Niles.

Rudy s Hot Dog, 6069 Summit, inspected June 9. Observed TCS food items (time/temperature control for safety) above 41 degrees in the pie case. All cold-holding TCS food items must be held below 41 degrees at all times to prevent the potential growth of bacteria. Items in the pie case have been in the unit for less than four hours and moved to the beer cooler to store until the pie case can be repaired. Inspector: Jennifer Gottschalk.

Tim Horton s, 4735 Monroe, inspected June 9. Cooked hash browns are being held at room temperature with no hot holding. This is a TCS food and must be held hot at 135 degrees or higher at all times to prevent the growth of bacteria. A wipe cloth bucket with the correct concentration of sanitizer must be set up at all times of operation. Work surfaces must be sanitized periodically during and before work begins. Also, all wet wiping towels must be stored in sanitizer when not in used. Inspector: Kelly Sattler.

Biggby Coffee, 4204 Sylvania, inspected June 11. Quat sanitizer concentration is below 100 ppm in the wipe cloth bucket. Always maintain quat sanitizer at 200 ppm to effectively sanitize food contact surfaces. Also, wet wiping towels are stored out on counter tops; store in sanitizer bucket. Inspector: Sattler.

Avenue Bistro, 6710 West Central, inspected June 15. Date marking in walk-in cooler, some items marked, others not marked. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in a food service operation or a retail food establishment for more than 24 hours, shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, based on the temperature and time combinations specified. The day of preparation shall be counted as Day One. Inspector: Nick Kusina.

Fricker s, 1418 Reynolds, Maumee, inspected June 15. Cook line prep-top cooler and walk-in cooler holding food between 43-44 degrees. Contact repair service and adjust coolers as needed. Food stored on the floor of walk-in freezer. All food must be stored at least six inches off of the floor at all times. Rearrange food storage in the walk-in freezer and remove all product from the floor. Inspector: Paul Bauman.

Golden City Restaurant, 5051 North Summit, inspected June 16. Observed wet cloth on the cutting board on the prep top cooler, must be kept in bucket when not in use. Observed cooked noodles at 58 degrees in a pan of water at room temperature. Once the noodles are cooked, they must remain under temperature control to prevent bacteria growth. Noodles placed in walk-in cooler. Raw chicken over raw beef in the bottom part of the prep top cooler. Follow proper food storage order in all coolers to prevent cross contamination. Observed chicken cooling with a cover on it. Cool food in smaller amounts, on trays or shallow pans. Leave food uncovered until fully cooled. Cool food from 135 degrees to 70 degrees in two hours and from 70 degrees to 41 degrees an additional four hours. Monitor both time and temperature of cooling food. Corrected. Bean sprouts and box of chicken on the floor in walk-in cooler and containers of cabbage on the floor. All food must be stored at least six inches off the floor at all times to prevent contamination. Monitor all food stored in the coolers and in dry stock area. Observed hand washing in the prep sink. Hands must be properly washed in the kitchen hand sink only. The food prep sink is only used to prepare food (thaw, cool), or wash produce. Prep sink may not be used to wash hands or wash dishes. Sanitize the prep sink prior to each use. Observed pest activity in the basement on one glue board by the right side wall next to freezer and white tub sink. Facility has monthly pest control by licensed company. Continue to work with pest control company. Facility must be free of pests. Inspector: Gottschalk.

Java Mill, 5053 North Summit, inspected June 16. Observed gallons of milk, juice, and products for the coffee area in the sales area beverage cooler. The cooler is only approved for prepackaged non-potentially hazardous beverages. Do not place food/beverages for the coffee area in this cooler. Inspector: Gottschalk.

Point Place Post VFW, 5416 North Summit, inspected June 16. No hand soap at the bar hand sink. All hand sinks must be stocked at all times with hand soap, paper towels, and hot/cold water (100 degrees) for proper hand washing to occur. Inspector: Gottschalk.

Rudy s Hot Dog, 3208 West Alexis, inspected June 16. Observed food stored on floor in walk-in cooler. All food must be stored a minimum of six inches above the floor. Inspector: Stacy Seger.

Navy Bistro/Tango s, 30 Main, inspected June 17. The hand sink next to the Tango entrance to the walk-in cooler was removed without approval. The kitchen was approved with this hand sink location. Due to the size of the kitchen and location of the other hand sink, this hand sink must be reinstalled. The hand sink location next to the Tango side of the walk-in cooler not only serves the prep area, it also serves the wait staff for the party room. Hand washing is the number one barrier against food borne illness. Stock the sink properly. This violation must be corrected in 30 days. Observed dirty dishes on the clean dish rack in the Tango dish room. Remove the dishes from the rack and rewash them. Do not place dirty dishes with clean dishes; as the clean ones will be contaminated. Dish washers must look over the racks when they come through the dish machine to ensure proper cleaning was achieved. Dirty water and debris on the clean side of the dish machine drain board in both dish machine rooms. The clean drain board must be cleaned and sanitized immediately. Observed dishes washed and becoming contaminated from the debris on the drain board. Instruct all dish washers that the clean side of the drain board must remain clean and sanitized on a regular basis. Raw beef stored above ready-to-eat pasta salad in the small walk-in cooler in the Bistro kitchen. To prevent cross contamination, follow proper food storage order. Continue to store raw meats and shell eggs according to required cooking temperatures. Onion soup in the Bistro kitchen and chicken, taco meat, refried beans, and rice from Tango s not properly cooled. Follow proper cooling procedures. Facility must be using shallow pans no more than two inches deep of food in each pan, ice bath up to the level of food in each container, freezer, or ice wands. Corn dogs and stuffed peppers double-panned in the salsa prep top cooler at 45-46 degrees. Maintain at proper cold holding temperature. Do not double pan items in the prep top cooler. Use cooler as intended. Pans must be dropped into the prep top cooler to maintain proper temperatures. Bistro dish machine dispensing 0 ppm chlorine. To properly sanitize with chlorine, the dish machine must be dispensing 50-100 ppm, use test kit to verify. Corrected. Chemicals stored next to fountain pop machine and near small plates in the server area. To prevent contamination, store all chemicals below or away from all food and food contact items. Inspector: Gottschalk.

Pool Room Sports Bar, 502 Main, inspected June 17. Small leak under four-compartment bar sink. Repair leak immediately. All plumbing must be in good condition. Inspector: Gottschalk.

The Bottom Line, 2053 Starr, inspected June 17. Observed pizza made at Main Street Bar location and transported to this location and placed in the freezer. No production date was observed on the pizzas. No ingredient labels observed on the pizza. All pre-made pizza must have the production date on it to allow employees to know when they were produced. Ingredients for the dough and every item on the pizza is also required. The person in charge must have the ability to read the ingredient labels if a customer asks if one of the major allergens is used in the pizza. Retail food label handout provided at inspection. Place ingredient labels on each individual pizza or make a book and update as needed.

Happy s Pizza, 1472 Sylvania, inspected June 18. Wipe buckets with no sanitizer solution. A sanitizing solution of 50-100 ppm bleach or 200 ppm quat must be available at all times. Observed back door opened at time of inspection heavy fly infestation as a result. All doors must be kept closed with a tight fitting seal to prevent pest infiltration. A tight-fitting screen door may be used. Breading mix stored on bottom shelf with chemicals. All chemicals must be stored below/away from food items to prevent chemical contamination. Several items with no date mark. All TCS foods must have a seven-day discard date. Bulk items not properly covered. All food products must be properly covered to prevent contamination. TCS foods sitting out at room temperature. Must be held cold at proper temperature to prevent bacteria growth. Store all sauces and food items that state must be refrigerated after opening inside cooler. Inspector: Stacy Seger.

Kingston Residence, 4125 King, Sylvania, inspected June 18. Walk-in cooler holding TCS foods at unsafe temperatures. Adjust or service cooler to hold proper temperature. Will reinspect cooler next week. Inspector: Brandon Tester.

Arby s, 1205 West Alexis, inspected June 19. Observed tomatoes holding at 53 degrees and dressing at 55 degrees. Inspector: Seger.

Brownstone Tavern, 847 West Alexis, inspected June 19. TCS foods in the prep cooler above 41 degrees. Food discarded. Improper storage gaskets on prep cooler and walk-in cooler. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer s specifications. Inspector: Seger.

Malibu s, 2045 West Alexis, inspected June 19. Observed cans of pop stored in ice behind bar. Beverage ice must be free from contamination. Only ice scoop with handle up can be stored in ice bin. Inspector: Seger.

VFW Post #2898, 3925 West Alexis, inspected June 19. Eggs stored on shelf above other ready-to-eat food items. Always store on bottom shelf to prevent cross contamination. Inspector: Seger.

Rite Aid, 3325 West Central, inspected June 10. Raw shell eggs stored above iced tea and other beverages. Always store raw meats or eggs separate from or below ready-to-eat foods and beverages. Inspector: Sattler.

Dollar Fifty Plus, 1312 Nebraska, inspected June 11. Facility using residential BBQ grill barrel to cook ribs and chicken. This facility has been told numerous times to change machinery. Facility must have new equipment by June 19, or this will be closed. Facility has been notified of upcoming food license, to either change food menu, or change classification. Meat being prepped in three-compartment sink without proper air gap. Provide proper air gap for sink, if air gap is not provided, facility must use pre-packaged and pre-cut foods. Repeat violation. Inspector: Niles.

Stop & Go, 3730 Monroe, inspected June 15. Replace two-compartment sink with a three-bay sink with two drain boards for wash, rinse, and sanitize of coffee pots and ice scoop. Provide labeling with store name and address on the ice bags. Keep raw shell eggs below milk in display cooler to prevent potential cross contamination. Inspector: Debbie Dacquisto.

Big Lots, 3946 West Alexis, inspected June 16. Observed baby food with expiration dates covered by price tag. Inspector: Seger.

East Side Mini Mart, 615 Oak, inspected June 16. Observed employee open food and beverage in the ice cream freezer. Do not place employee food in the ice cream freezer or in the reach in coolers/freezers. Place all employee food/beverages in the walk-in cooler in one labeled area away from food for sale to prevent contamination. Corrected. Bag of ice from the facility for sale. All bagged ice must be labeled. Inspector: Gottschalk.
