Critical violations

Biggby Coffee, 4031 McCord, Sylvania, inspected Dec. 14. Employee drink without lid. Provide lids for drinks to prevent contamination. Cracked/broken containers. Replace containers as needed to prevent contamination. Observed leak under 3-bay sink. Repair leak. Inspector: Brandon Tester.

Cousino's Steak House, 1842 Woodville, Oregon, inspected Dec. 14. Salad area hand sink with no hot water. Hand sinks must be fully stocked with soap, paper towels, and hot water of 100 degrees or higher. This is for proper hand washing. Provide hot water to the hand sink. Uncovered fully cooled food products (carrots) uncovered in the walk-in cooler and in the dry storage area (bread). All open fully cooled product must be covered to protect from cross-contamination. Cover open food items. Sanitizer buckets and bar's 3-compartment sink at 0 ppm in sanitizer buckets. Buckets and sink must remain at 200 ppm to properly sanitize counters or equipment. Refill the buckets and sink more frequently. Ground beef above the whole cut meat in the walk-in cooler. Follow the proper storage order to prevent cross contamination. See CCP for proper storage order. The bar hand sink blocked with bottles. The hand sink must remain unblocked for proper hand washing. Inspector: Julie Nye.

Subway, 3324 Secor, inspected Dec. 14. The hand sink in the back of the facility was blocked at inspection. Hand sinks must be kept clear to facilitate proper hand washing. Corrected. Inspector: Gillian Wilke.

Hikari Japanese Restaurant, 3150 Navarre, Oregon, inspected Dec. 15. There was no supply of sanitizer set up. During all hours of operation, a sanitizer bucket must be made to the proper concentration. Quat tablets are used, maintained at 200 ppm. Label all spray bottles with the common name for proper identification. Inspector: Nye.

My Brother's Place, 316 Adams, inspected Dec. 15. Deli meats being thawed in the prep sink in water with a little ice holding at 70 degrees. Proper thawing methods must be performed to prevent the growth of bacteria. If you choose to thaw in your prep sink, cold running water must be hitting the thawing product. Inspector: Kelly Cipiti.

Oregon Nursing & Rehabilitation Center, 904 Isaac, Oregon, inspected Dec. 15. There was no sanitizer bucket set up in the kitchen at inspection. A sanitizer bucket either with 200 ppm quat or 100 ppm chlorine must be set up to be able to sanitize counters and equipment. Inspector: Nye.

Meadows Choice Community School, 1853 South, inspected Dec. 16. No sanitizer bucket set up. Sanitizer bucket must be available with 100 ppm chlorine. This is to be able to wipe counters, equipment, and tables. Inspector: Nye.

Tropical Grill, 5001 Monroe, inspected Dec. 16. The pina colada mix is holding at 51 degrees. Food must be held at 41 degrees or less to prevent bacteria growth. The quat sanitizer in the wipe cloth bucket was too strong at over 400 ppm. Maintain concentration at 200 ppm. Corrected. Inspector: Wilke.

Oakleaf Toledo, 4220 Holland Sylvania, inspected Dec. 18. Chipped paint on wall next to shelf above 3-bay sink. Repair wall to avoid chipping paint and physical contamination. Employee jacket on top of food equipment. Store personal belongings in designated areas separate from food prep to prevent contamination. Inspector: Tester.

Arbors At Toledo, 2920 Cherry, inspected Dec. 21. Dish machine is not running its cycles and is not dispensing chlorine in final rinse. Service has been called. Until machine is repaired all dishes must be washed, rinsed, and sanitized in 3-bay sink. Cream of mushroom soup at 63 degrees leftover from lunch, covered with plastic wrap. Cooled foods must go from 135 degrees to 70 degrees in two hours or less, then from 70 degrees to 41 degrees in four hours or less. Do not cover foods until they reach 41 degrees or less. Corrected. Buffalo Wild Wings, 3555 Navarre, Oregon, inspected Dec. 21. Hand sink blocked with a chemical spray bottle. Hand sinks must be free from blockage for proper hand washing accessibility. Inspector: Nye.

Dave's Diner, 1945 Woodville, Oregon, inspected Dec. 21. Mashed potatoes at unsafe temperatures of 88-110 degrees. Food must be held at 135 degrees or higher to prevent bacteria that can cause food-borne disease. One container of mashed potatoes was discarded. The 3-compartment sink at 0 ppm and the sanitizer buckets over 200 ppm chlorine. The sink and buckets must remain at 50 and 100 ppm chlorine to properly sanitize without leaving a chemical residue. Inspector: Nye.

Frisch's Big Boy, 3537 Secor, inspected Dec. 21. The dessert case is holding food above 41 degrees. Food must be held at 41 degrees or less to prevent bacteria growth. Adjust the temperature of the cooler and contact a repair service if necessary. Inspector: Wilke.

Club 300, 3304 Collingwood, inspected Dec. 22. One pan of taco meat in 4 inch plastic covered pan at 45 degrees in walk-in cooler. Hold potentially hazardous food at 41 degrees and less to inhibit the growth of bacteria. Put in freezer, rechecked now at 30 degrees. Inspector: Dacquisto.

Judy's Again, 11407 Corduroy, Curtice, inspected Dec. 22. Food at unsafe temperatures: mayonnaise at 42 degrees, cheese at 42 degrees, and dressing at 43 degrees. Food must be at 41 degrees or below to prevent bacteria growth. Lower the temperature of the cooler or have the cooler repaired. No hot water in the 3-compartment sink and hand sink. Hot water of 100 degrees must be available for proper ware and hand washing. Provide hot water to facility. The facility stated that they just turned the hot water tank on. If the facility is shutting off hot water tank, it must be turned on before the facility is opened so it has hot water available. Food handler starting to cook without properly washing hands. The food handler must wash hands before food handling, in between tasks, and before glove use, etc. The hand sink blocked. Hand sink must remain free from blockage for proper hand washing accessibility. No sanitizer set up in the bar or kitchen. Sanitizer bucket must be set up with 200 ppm quat to properly sanitize counter tops, tables, and equipment. Facility with clutter and miscellaneous items in food area (example: fast orange hand cleaner, darts, and winter gloves on a food rack and dirty rag, pen, and a cellular phone on the prep top cooler cutting board). The facility must clean and organize the kitchen so it is good clean repair and prevent contamination. Inspector: Nye.

The Hungry I, 6060 Renaissance Place, inspected Dec. 22. Prep top cooler holding food at unsafe temperatures above 41 degrees. Food must be held at 41 degrees or less to limit bacteria growth. Keep less product in cooler during rush times and keep doors closed as much as possible. Raw burger stored over ribs in walk-in cooler. Inspector: Tester.

BP/Barney's Convenience Mart, 4128 Monroe, inspected Dec. 14. The hand sink was blocked. Do not store items on the sink so that it is always accessible for food handlers to wash hands. Found improper labeling on cheesecakes and chocolate cake slices in the reach in cooler. All packaged food for self-service must have a label indicating the common name of the food, a complete ingredients list, net weight, and a responsibility statement indicating who produced the food. Pull items off the shelves. Corrected. The self-serve donut case does not have a booklet of information available for the public to view. Provide ingredient statements for each individual donut available for purchase so that the public is aware of what is in the donuts. Found moldy hot dogs and taquitos stored in the walk-in cooler. The taquitos were date 10/31/09. Prepared/opened food may only be held up to seven days to prevent bacteria growth. These items should also be dated to ensure proper rotation. Discard the spoiled food. Inspector: Wilke.

Stop N Go, 732 Conant, Maumee, inspected Dec. 18. Walk-in cooler holding foods at 46-47 degrees. Foods held cold must maintain an internal temperature of 41 degrees or below. Monitor cooler and contact repair if necessary. Inspector: Paul Bauman.

BP/Barney's Convenience Mart, 3532 Stickney, inspected Dec. 21. Bagged ice stored on the floor in the walk-in freezer. Food items must be protected from contamination, store in a clean and dry area at least six inches off the ground. Observed a plate stored in the hand sink in the back ware washing area. Do not block the hand sink, keep accessible for proper hand washing. Do not use the hand sink for storage. Inspector: Kerry Stanley.

Central Market, 330 West Central, inspected Dec. 21. Observed build up inside of the ice machine. Clean and sanitize unit, maintain clean. Inspector: Dacquisto.

Sunoco, 1855 Woodville, Oregon, inspected Dec. 21. Hot dogs and tomatoes without a date mark. Food items must have a date mark to determine a discard date. This is to limit bacteria growth. A chemical stored on the sanitizer bay of the 3-compartment sink. Chemicals must be stored below and away from clean food-contact items. Inspector: Nye.
