Recently released inspection reports of Lucas County food-service operations.
Violations:
Plummer Pool, 6848 Maplewood, Sylvania, inspected Aug. 8. Employee handling nacho chips with bare hands. No bare hand contact with ready-to-eat food is permitted. Always wash hands and put on gloves before handling ready-to-eat food to prevent cross contamination. Ready-to-eat food may be handled with gloves, utensils, or deli paper. Bottle labeled sanitizer did not test for bleach and it had a yellow color and did not smell like bleach. Bottle was emptied and discarded. Properly label chemical spray bottles to ensure employees know what it should contain. No bleach available at the stand for sanitizer. Provide bleach to use as sanitizer. Sanitizer must be available at all times to use when setting up the sink and to place in wiping cloth bucket. Inspector: Jennifer Gottschalk.
Centennial Quarry & Terrace, 5773 Centennial, Sylvania, inspected Aug. 8. No sanitizer wiping cloth available. Before starting to work, a sanitizer wiping cloth bucket must be set up. Store in use wiping cloths in the sanitizer bucket. For quat sanitizer it should test at 200 ppm. Check concentration with the test kit, change as needed. No seven-day discard date observed on open hotdogs in the cooler and hotdogs dated July 7. Open and prepared food must be dated. Discard hotdogs labeled July 11. Cleaning chemical stored with pop syrup boxes. Store chemicals away from food and food contact items to prevent cross contamination. Inspector: Gottschalk.
Tim Hortons, 5800 W. Alexis, Sylvania, inspected Aug. 8. No paper towels at the hand sink in front. All hand sinks must be in good working order, and stocked with hand soap, paper towels, hot/cold running water to allow for proper hand washing. Sanitizer bucket tested at 0 ppm for quat sanitizer. The quat sanitizer wiping cloth bucket must test at 200 ppm at all times. Corrected. Coffee pot iced down in the dump sink in front. The only sink that may be used to ice down coffee is the prep sink. Inspector: Gottschalk.
Jimmy John's, 6385 Monroe, Sylvania, inspected Aug. 10. Unlabeled chemical spray bottle observed. Chemical spray bottles must be properly labeled. Store chemicals away from food and food contact items. No sanitizer wiping cloth bucket set up as required. Store wiping cloths in sanitizer bucket and change as needed. Inspector: Gottschalk.
Taco Bell, 5860 W. Alexis, Sylvania, inspected Aug. 10. Chicken hot holding at 124 degrees and steak at 123 degrees on the line. All time/temperature control for safety foods must be hot held at 135 degrees or higher. One pan of each was discarded. Monitor temperatures of all hot holding items. Inspector: Gottschalk.
Chuck E Cheese's, 5077 Monroe, Sylvania, inspected Aug. 7. No hot water available at hand sink in kitchen. Hot water must be available for proper hand washing. A can of dust spray stored with pop boxes in the storage room. Store cleaning supplies and chemicals in a designated area away from or below food and food contact items. Inspector: Kelly Sattler.
Big Mama's, 1008 Nebraska, inspected Aug. 8. Coleslaw in serving table at 46 degrees. Cold hold such food at 41 degrees or less. Chlorine sanitizer bucket at 10 ppm. Chlorine sanitizer buckets need to be between 50 and 100 ppm to properly wash food-contact surfaces. Change chlorine sanitizer bucket more frequently to maintain concentration. Obsesrved improper storage order of food in cooler. Follow proper storage order. Inspector: Kurt Susdorf.
Spot Mart Inc., 525 Dorr, inspected Aug. 8. Nacho cheese and chili sauce at 128 degrees in steam table. Hot hold such food at 135 degrees or higher to prevent bacteria growth. Cardboard on floor in kitchen and rugs by freezer. Flooring needs to be smooth and easily cleanable. Cardboard and rugs attract water and dirt. Replace with a smooth and easily cleanable surface. Grape Leaf Express, 5236 Monroe, inspected Aug. 10. Observed improper cooling of soups. Follow proper cooling process. A new smoothie bar has been added without health department approval. Any changes or additions to this operation must be submitted in writing to the health department and approved prior to construction. Provide a drawing of the smoothie area and a list with make and model number of all added equipment. There is no hot water available at the hand washing sink at the new smoothie bar. Hot water must be available at all time for proper hand washing. Grape leaves and rice holding in the steam table below 135 degrees rather than at 135 degrees or higher. Grape leaves and beef holding at 45 degrees in the glass door merchandiser rather than at 41 degrees or less. Rice pudding and creme caramel at 48 degrees in the dessert cooler; cold hold such food at 41 degrees or less to prevent the growth of bacteria. Dirty dishes in the hand washing sink at the cook line and sink is inaccessible because there is a trash can in front of it. Keep hand washing sinks accessible. Wet wiping towels out on work surfaces rather than stored in sanitizer bucket. Many spray bottles with chemicals in them lack labels. Label spray bottles with content names. Some items are properly date marked, but not all foods. All refrigerated, ready-to-eat foods must be date marked when they are made or the manufacturer's packaging is opened. Date mark all prepared salads, hommos, rice pudding, creme caramel, grape leaves, etc. Foods may be held up to seven days at 41 degrees or lower. No sanitizer in the bucket set up. Maintain chlorine sanitizer at 50 to 100 ppm to safely sanitize food contact surfaces. Margarine stored directly on top of raw chicken. Store ready-to-eat food separate from raw meats. Due to the amount of critical violations found, it is evident there is a lack of food safety knowledge amongst the staff. More food safety education is required. Inspector: Sattler.
Bakery Unlimited, 4427 Secor, inspected Aug. 15. Hood system above cake donut maker needs to be cleaned. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Inspector: Stacy Seger.
San Marco's Super Markets, 719 Galena, inspected Aug. 15. Damp/soiled cloths stored on work surfaces. Store damp/soiled cloths in a sanitizer bucket. Corrected. Bag of rice stored on the floor in the back area. Store food and food-contact items at least six inches off the floor. Corrected. Cooked peppers in the steam table below 135 degrees rather than at 135 degrees or above or cold hold at 41 degrees or less. Auto dispensing unit dispensing at 100 ppm quat. To properly sanitize using this product, the concentration must be 150 ppm to 400 ppm. Provide the proper test kit. Corrected. The wipe cloth bucket at 0 ppm. Facility was only using water in the wipe cloth bucket. Use proper level of sanitizer. Corrected. Glass cleaner hanging on the movable cart with food contact items. Store chemicals below or away from all food to prevent chemical contamination Repeat violation. Corrected.
Toledo 76 Inc., 5432 N. Summit, inspected Aug. 15. Bag of towels stored in the hand sink. Keep hand sink accessible. Chemicals stored above beverages in several areas in the back room. Designate an area for all chemicals below or away from all food and food contact items. Inspector: Stanley.
Point Place Hot Dogs Inc., 4820 N. Summit, inspected Aug. 15. Front three-compartment sink and wipe cloth bucket setup at 0 ppm chlorine. To properly sanitize with chlorine, this sink must be setup at 50 to 100 ppm and the wipe cloth bucket must be setup at 100 ppm. Corrected. Inspector: Stanley.
Circle K, 6008 Secor, inspected Aug. 15. Inconsistent date marking in roller cooler. Food must have a seven-day discard date. Inspector: Seger.
GFS Market Place LLC, 609 W. Alexis, inspected Aug. 14. Chemicals intermixed with food and food contact items. Store poisonous or toxic materials where they can't contaminate food, equipment, etc. Inspector: Seger.
Toledo Sports Center, 1516 Starr, inspected Aug. 15. Missing date mark on container of taco meat. Properly label food with seven-day discard date. Missing labels on spray bottles. Label all spray bottles with common name of contents for proper identification. Inspector: Brandon Tester.
Le Pam Pam Pizzeria, 901 Madison, inspected July 30. Raw bacon stored above ready-to-eat food in walk in cooler. Food should be stored below ready to eat food to prevent possible contamination. Inspector: Nathan Fries.
Glass Pavilion Cafe/Toledo Museum of Art, 2444 Monroe, inspected July 25. Milk in espresso machine being held above 41 degrees. Food should be held below 41 degrees to prevent growth of harmful organisms. Inspectors: Fries.
Olney Market, 1950 N. Stadium, Oregon, inspected Aug. 14. Two-door cooler holding foods at unsafe temperatures. Adjust/service cooler to maintain proper temperatures of foods. Inspector: Tester.
Sonny Berry's Bayshore Supper Club, Inc., 5307 Bayshore, Oregon, inspected Aug. 15. Ground meats stored above whole meats and ready to eat foods in 3 door cooler. Practice proper food storage order to prevent possible cross contamination. Poultry/eggs on bottom them ground meats whole meats/fish then ready to eat foods on top. Inspector: Tester.
Health Foods by Claudia, 3904 Secor, inspected Aug. 10. The deli cooler is holding food cold at 45 degrees. Food must be at 41 degrees or lower. Adjust or repair cooler. No ingredient statements on the packaged food. An ingredient statement must be included or each package of food that has two or more ingredients. Also, there is no net weight listed on the packaged omega meats. There must be a net weight not including packaging weight on each package of meat. Inspector: Sattler.
Rally's, 5855 W. Central, inspected Aug. 11. No sanitizer detected in either the three-compartment sink or the wipe cloth bucket. Change sanitizer solution as needed to maintain chlorine at 50 to 100 ppm. Corrected. Inspector: Sattler.
Toddler Tech, 4030 Royer, inspect Aug. 7. Raw ground beef stored above cooked chicken in the reach-in cooler. Store raw meats and eggs below ready-to-eat foods to prevent cross contamination. Inspector: Sattler.
Tim Hortons, 4735 Monroe, inspected Aug. 7. Walk-in cooler is holding foods at 43 to 47 degrees. Food must be held cold at 41 degrees or lower at all time. Adjust or repair cooler. Inspector: Sattler.
Famous Dave's, 4757 Monroe, inspected Aug. 7. Pork is holding hot at 118 to 125 degrees because it is double panned in the steam well. Food must be held hot at 135 degrees or higher. Pork was discarded. No soap available at the bar hand washing sink for proper hand washing. Provide soap and paper towels at all hand washing sinks at all times. Inspector: Sattler.
Toledo Robin Inc., 4850 Monroe, inspected Aug. 6. Shake mix in the soft serve machine is holding cold at 45 degrees. Food must be held cold at 41 degrees or lower. Adjust or repair the soft serve machine as needed to maintain proper temperature at all times. Chemical sanitizing dish machine at the bar is out of sanitizer. Always check sanitizer levels before using the dish machine to make sure that chlorine sanitizer is dispensing properly at 50 to 100 ppm. Corrected. Inspector: Sattler.
Buzz Family Diner, LLC, 6670 Providence, Whitehouse, inspected Aug. 8. Chemical squirt bottles found hanging from the pot and pan rack. Chemicals must be stored where they can't contaminate food or clean equipment. Cross contamination present in upright freezer. Poultry, pork, and beef found stored intermingled on freezer shelves. Rearrange the freezer to segregate the meat types. Inspector: Sara Becker.
Ko's Garden, 14241 Airport, Swanton, inspected Aug. 8. Date mark all potentially hazardous foods with a seven day date once the product has been cooked. Chicken cooked in the facility in cold holding had no date label. Inspector: Becker.
