Recently released inspection reports of Lucas County food-service operations.

Violations:

Applebee’s Neighborhood Grill & Bar, 266 E. Alexis, inspected Nov. 16. Uncovered employee beverages stored on the cook line and above food prep areas. Employee beverages must have a lid and be stored to protect food and clean items/surfaces from contamination. Corrected. Chili base cooling covered in the walk-in cooler. Items must be left uncovered or vented until 41 degrees or less. Product vented. The faucet of the dipper well at the bar unable to stay in place. Repair dipper well. Repeat violation. Eddie Toledo 24 LLC, 5424 Telegraph, inspected Nov. 20. The hand sink near the three compartment sink blocked by equipment. Keep sink accessible to ensure proper hand washing. Insect spray stored above food contact items on the sales shelf. Store chemicals below or away from food and food contact items. Inspector: Stanley.

The Mirage, 950 Buckingham, inspected Nov. 19. No hot water in hand sink in woman’s restroom. Restore hot water in woman’s restroom. No paper towels or hand soap at hand sink in kitchen. Facility needs to use hand soap not hand sanitizer in kitchen. Hand soap and paper towels need to be available for proper hand washing. Inspector: Kurt Susdorf.

San Marcos Restaurant, 3439 Hill, inspected Nov. 19. Inconsistent date marking in walk in cooler. Once open or prepared, time and temperature controlled for safety foods must have a seven-day discard date. Hand sink near prep sink with no soap or paper towels. Properly stock hand sink with soap and paper towels. Inspector: Susdorf.

Merhi Enterprises Inc. dba Rocket’s Cafe, 2023 N. Reynolds, inspected Nov. 20. Chemical dish machine at 0 ppm final rinse. Chlorine sanitizer bottle was empty. Manager replaced bottle. Monitor bottles more closely and replace when needed. Chlorine sanitizer above 200 ppm in sanitizer sink at bar. Use test strips to maintain proper concentration. Inspector: Susdorf.

El Sol Mexican & American Restaurant, 1007 S. McCord, Holland, inspected Nov. 13. Items reheated improperly in the steam table rice. Follow proper reheating process. Sausage at 122 degrees in the steam table. Steam table was not turned on. Sausage was reheated and then placed in the steam table once turned on. Improper storage order of food; follow proper storage order. Inspector: Julie Nye.

The Wings Station, 1801 N. McCord, inspected Nov. 14. Dessert cooler holding at improper temperatures. Lower temperature or repair. Ready-to-eat food lacks proper seven-day discard date. Product was labeled. Several spray bottles without labels; properly label chemicals. Inspector: Nye.

Longhorn Steakhouse, 4155 Talmadge, inspected Nov. 13. Raw beef stored above ready-to-eat foods such as nacho cheese in the walk in cooler. To prevent cross contamination, store raw meats, poultry and eggs separate from or below all ready-to-eat foods. Inspector: Kelly Sattler.

Toledo Retirement Residence LLC d/b/a: Alexis, 4560 W. Alexis, inspected Nov. 16. Quat sanitizer bucket tested below 200 ppm. Bucket was changed and tested at 200 ppm. Use test kit to check the concentration; change solution as needed. Inspector: Jennifer Gottschalk.

Los Hermanos Enterprises LLC d/b/a Magdalenas, 1955 W. Alexis, inspected Nov. 19. Portion cups inside seasonings in dry stock area and sauces in walk-in cooler. Use scoop with handle to prevent contamination. Bulk food items improperly stored. Once opened, bulk items must be placed in a bulk food container to prevent pest infiltration. Hot dogs and ribs lack required seven-day discard date. Provide wipe buckets for cook line and prep line. Wet wipe rag sitting on prep table; store rags in wipe bucket with proper level of sanitizing solution. Chemical spray bottles with no labels; properly label spray bottles. Towels under matting with glasses in bar. Towels harbor bacteria and may not be used under glasses. Inspector: Stacy Seger.

Jesse James West, 1515 W. Laskey, inspected Nov. 19. Improper storage of chemical bottles; store below or away from food and food contact items. Corrected. Hot dogs, cheese, burgers, and other foods lack required seven-day discard date. Both hand sinks lack towels and soap. Properly stock sinks. Quat sanitizer above 400 ppm rather than at 200 ppm to prevent chemical residue/chemical poisoning. Corrected. Inspector: Seger.

Express Carryout of Toledo LLC, 1920 Mulberry, inspected Nov. 14. Uncovered employee beverage stored on a rack above the prep top cooler. Store employee beverages, with lid, in separate area. Corrected. A bottle of Gatorade stored in edible ice. Do not use edible ice as an exterior coolant. Corrected. Inspector: Stanley.

Paris Marina District LLC d/b/a: Paris Nite Club, 1202 Front, inspected Nov. 20. During all hours of operation, set up a supply of sanitizer solution to ensure proper sanitation is taking place. Wipe down food contact surfaces with sanitizer. Repeat violation. A white substance being stored inside a spaghetti jar without a label. Mark this container with the proper name for identification. Do not store over a food contact area. Inspector: Kelly Cipiti.

Sbarro, 5001 Monroe, inspected Nov. 19. Found food above 41 degrees in prep top cooler. Cold hold at 41 degrees or colder. Food discarded. Quat sanitizer bucket tested at 0 ppm; bucket was changed and retested. Corrected. Improper cooling procedures observed. Follow proper cooling procedure. Inspector: Gottschalk.

Sylvania Franciscan Academy, 5335 Silica, Sylvania, inspected Nov. 20. Required seven-day discard date lacking on open turkey lunch meat. Inspector: Gottschalk.

Vito’s Pizza, 5412 Monroe, inspected Nov. 19. Required seven-day discard date lacking on open bags of wings. Inspector: Gottschalk.

Jet’s Pizza, 5392 Monroe, inspected Nov. 20. Chemicals stored on the top shelf above portion cups. Store chemicals in separate area. Corrected. Found pizza by the slice at 107 and 118 degrees in the right warmer. Hot holding food items must be held hot at 135 degrees or higher. Four slices of pizza were discarded by the manager. Del Taco, 3348 Secor, inspected Nov. 20. Whipped cream sitting out at room temperature; manager said can was empty and was discarded. Wet wiping towels stored out on prep surface rather than in sanitizer. No sanitizer bucket set up in this area as required. Inspector: Sattler.

Claddagh Irish Pub, 5001 Monroe, inspected Nov. 20. Glass washer at the bar is hooked up to sanitize using quat sanitize. Quat sanitizer is not approved for use in a glass washer or dish machine. Quat sanitizer was removed and iodine was hooked up. Meatloaf at 54 degrees, chicken at 50 degrees, and burgers at 48 degrees in the drawer cooler under the charbroiler. Food must be held cold at 41 degrees or lower. Sour cream, dressing cups, whipped cream, and bleu cheese bulk dressing were discarded. Repair cooler immediately. Discontinue use until repaired. Raw turkeys thawing above ready-to-eat food items in the walk-in cooler. Follow proper storage order. Hot water handle of a hand washing sink is broken; repair immediately. Wet wiping towels out on work surfaces; store such towels in sanitizer. Inspector: Sattler.

China House, 1122 N. Byrne, inspected Nov. 21. Found fried chicken and egg rolls that were sitting out or in prep for too long with temperatures ranging 58 to 63 degrees. Food must be held at 41 degrees or less to prevent bacteria. Repeat violation. Raw shell eggs stored on vegetables in the prep top cooler. Store all ready-to-eat food above raw meats/poultry to prevent cross contamination. Wet towels on the counter tops. Store wet towels in the sanitizer bucket. Repeat violation.Back hand sink blocked and was not stocked with paper towels. Do not block the hand sink and provide paper towels to properly wash hands. Corrected. Inspector: Gillian Wilke.

Nori Japan OH Inc., 5001 Monroe, inspected Nov. 19. The fried rice is holding at 123 degrees. Food shall be held at 135 degrees or greater to prevent the growth of bacteria. Raw chicken stored above vegetables in the walk in cooler. Store all ready-to-eat food above raw meats to prevent cross contamination. Inspector: Wilke.

Biggby Coffee, 3160 Markway, inspected Nov. 24. Open display cooler holding yogurt at 53 to 54 degrees rather than at 41 degrees or colder. Yogurt was voluntarily discarded. Discontinue use of cooler until repaired and reinspected. Repeat violation. Wet towel by the espresso machine is in a bowl with no sanitizer water and the sanitizer bucket that is set up is at 100 ppm rather than at 200 ppm to properly sanitize food contact surfaces. Bayview Park Amvets Post, 4133 N. Summit, inspected Nov. 13. A tray of buns stored on the mop sink. Store food in clean, dry area at least six inches off the floor. Corrected. Inspector: Stanley.

Caribou Coffee Company, 5001 Monroe, inspected Nov. 19. Wet towels stored on counter tops. Store wet towels in sanitizer bucket. Hand sink is draining very slowly; repair sink. Inspector: Wilke.

Zahle LLC, 2942 W. Sylvania, inspected Nov. 19. The hot water at the hand sinks is only reaching 92 degrees at the cook line sink and 78 degrees at the deli side sink. Hand sinks shall have 100 degrees for proper hand washing. Restore the hot water immediately. Found a packaged container of assorted pastries not labeled. Provide a label for packaged goods that includes the food’s common name, an ingredients statement, the net weight and responsibility statement. Inspector: Wilke.

St. Francis De Sales High School, 2323 W. Bancroft, inspected Nov. 19. No sanitizer bucket set up as required. Corrected. Found chicken holding at 109 degrees in the hot holding cabinet. Cold hold at 41 degrees or less or 135 degrees or greater. Practice keeping foods out of the temperature danger zone as much as possible. Corrected. Inspector: Wilke.

Elephant Bar Restaurant, 5001 Monroe, inspected Nov. 19. Shredded cheese and fresh mozzarella on the prep top cooler holding cold at 46 degrees and 51 degrees rather than at 41 degrees or lower; items discarded. No sanitizer bucket set up in the back prep area as required. Corrected. Inspector: Sattler.

McDonald’s, 3138 Secor, inspected Nov. 20. Wet wiping towels stored out on work surfaces; store in sanitizer. Corrected. observed employee drop a condiment packet on the floor and then put it into a bag with food to be served at the drive thru. Discard items dropped on floor and employee must wash hands after handling items that were on the floor. Review this procedure with all employees. Inspector: Sattler.

Tropical Grill, 5001 Monroe, inspected Nov. 15. No sanitizer detected in wipe cloth bucket. Change sanitizer as needed to maintain proper sanitizer level. Corrected. Inspector: Sattler.

Ottawa Hills Elementary, 3602 Indian, inspected Nov. 20. Chicken nuggets at 115 degrees in steam table rather than at 135 degrees or hotter. Chicken nuggets were placed back into oven and reheated to 165 degrees. Inspector: Susdorf.

Deja Vu, 135 S. Byrne, inspected Nov. 20. No soap and paper towels at hand sink by fryer. Obtain soap and paper towels for hand sink. Inspector: Susdorf.

Shop N Save, 653 Main, inspected Nov. 19. Cease the sale of the portioned ice in cups for retail sale. Retail packaged items must be correctly labeled with responsibility statement. Inspector: Cipiti.

Lazeez, 337 N. St. Clair, inspected Nov. 19. Food in prep top cooler holding above 41 degrees rather than at 41 dgrees or colder. Food was removed; cooler will be serviced and re-checked. Inspector: Cipiti.

Wally’s Market, 561 E. Broadway, inspected Nov. 14. Provide hand soap and paper towels at the hand sink; keep sink accessible for hand washing. Provide supply of sanitizer for proper ware washing. Do not keep adding slushie mix without properly washing unit first. Inspector: Cipiti.

Wendy’s, 108 Main, inspected Nov. 20. Front under counter cooler holding above 41 degrees; adjust temperature to hold at 41 degrees or colder. Corrected. Quat sanitizer was below 200 ppm; container was almost emptied. A new bag was placed on dispenser. Use the test kit to verify concentration. Inspector: Cipiti.

Texas Roadhouse, 6137 Trust, Holland, inspected Oct. 26. Some food items in the walk-in cooler holding at 42 to 43 degrees rather than at 41 degrees or colder. Adjust temperature. Beans improperly cooling in covered pans in the walk-in cooler. Follow proper cooling procedure. Dish washer handling dirty dishes and then clean dishes. After handling dirty dishes, you must remove gloves and wash hands before handling clean dishes. Dish room hand sink blocked with a rack of glasses. Keep sink accessible for hand washing. Corrected. Fortune Inn Restaurant, 6725 W. Central, inspected Oct. 29. Food at improper temperature in the prep top cooler; adjust temperature or make repairs. No date marking in the facility. Properly label ready-to-eat food items with seven-day discard date. Improper storage order raw food items; follow proper storage order. Severe build up throughout the facility. Clean and maintain all areas. Inspector: Nye.

Quiznos Sub, 6819 W. Central, inspected Nov. 13. Back hand sink lacks soap and working soap dispenser; properly stock hand sink with soap, paper towels, and hot water. Prep top cooler and walk-in cooler holding food items at improper temperatures rather than at 41 degrees or colder; repair coolers. Happy Hour, 1519 Eleanor, inspected Nov. 15. Opened can of ketchup in refrigerator. Once opened all products must be placed in food-grade containers. Inspector: Seger.

Campus Oasis, 3303 Dorr, inspected Nov. 16. Whisk stored in hand sink and a food container stored in another hand sink. Hand sinks must be kept clear for hand washing. Corrected. Inspector: Wilke.

University of Toledo - Starbucks, 2801 W. Bancroft, inspected Nov. 15. Sanitizer buckets by espresso machine were not full and the wet towels as a result were not submerged in the solution. Keep sanitizer buckets full to control the growth of bacteria in the towels and to properly sanitize food contact surfaces. Corrected. Cream cheese in display case was 43 degrees. Suggest storing the cream cheese on the bottom shelf of cooler. Inspector: Wilke.

Target, 5225 Monroe, inspected Nov. 14. Snack food stored below lighters in the dollar store section of the store. Store chemical products away from or below food and food contact items. Found package of baby formula with an expiration date of Sept. 7; no out of date baby food items may be offered for sale. Item was discarded. Inspector: Sattler.

Jing Chuan Restaurant, 4424 Secor, inspected Nov. 16. Food items in metal cans. Once opened, food must be place in approved food-grade containers. Re-use of single-use containers is prohibited. KFC Express, 5001 Monroe, inspected Nov. 13. Hot water handle of a hand washing sink is broken; repair or replace handle immediately. Repairs were made. No sanitizer detected in either wipe cloth bucket. Change sanitizer as needed to maintain proper sanitizer level. Corrected. Raw salmon stored above shredded cheese in a cooler. Winterfield Ventures Academy, 305 Wenz, inspected Nov. 16. Apples washed in three-compartment sink. Produce needs to be washed in an air-gapped prep sink. Facility does not have an air gapped prep sink and needs to only serve ready-to-eat produce. Inspector: Susdorf.

Hong Kong Buffet, 3017 Glendale, inspected Nov. 19. No documentation of sushi rice ph or make/discard time. Shrimp and pork sausage thawing on counter tops. Follow proper thawing procedure. Raw fish stored below raw chicken in walk-in cooler. Chicken must be stored below fish. Wipe cloths without sanitizer solution in use. This is a must.
