Star Diner II, 457 W. Dussel, Maumee, inspected Dec. 10. Inadequately sanitizing surfaces due to malfunction of chemical sanitizer dispensing unit. Repair dispenser. Inspector: Paul Bauman.

Kroger, 4633 Suder, inspected Dec. 11. Improper thawing of chicken in the prep sink in standing water. To limit bacteria growth, follow proper thawing procedure. Bakery hand sink blocked by a shopping cart of markdown items. Keep accessible to ensure proper hand washing. Corrected. Improper storage or raw meats in the walk-in cooler; follow proper food storage order to prevent cross contamination. Repeat violation. Corrected. Inspector: Kerry Stanley.

Meijer, 1500 E. Alexis, inspected Dec. 10. Chemical spray bottle on a prep table with food-contact containers. Designate an area for chemicals below or away from food and food-contact items. Repeat violation. Corrected. Ground beef stored above whole cuts of beef in the sales case; follow proper food storage order. Corrected. Inspector: Stanley.

B Gump’s 101 Restaurant & Lounge, 5147 S. Main, Sylvania, inspected Dec. 7. Found steak in the top section of prep-top cooler at 56 degrees. Meat pans are overstocked. Time/temperature control for safety food must be held at 41 degrees or lower. One steak was discarded. Unlabeled spray bottle contains olive oil. Spray bottles must be labeled to allow employees to know contents. Store spray bottles appropriately to prevent cross contamination. Found bottles of caramel sauce, chocolate sauce and jerky sauce open and out at room temperature. Items require refrigeration after opening. Sauces were discarded. Inspector: Jennifer Gottschalk.

Mayberry Diner, 8253 Mayberry, Sylvania, inspected Dec. 11. Food stored on cooler floor. Store food at least 6 inches off the floor to protect from contamination. Unlabeled spray bottles at the front counter. Chemical spray bottles must be properly labeled to allow all users to know contents. Store chemical spray bottles away from food and food-contact items to protect from contamination. Hands must be washed prior to placing gloves on, and hands must be washed again and gloves changed when changing tasks to prevent cross contamination. Inspector: Gottschalk.

Jet’s Pizza, 4900 N. McCord, Sylvania, inspected Dec. 11. Unlabeled chemical spray bottle. Chemical spray bottles must be properly labeled to allow employees to know contents. Store chemicals away from food and food-contact items to prevent cross contamination. Inspector: Gottschalk.

Jojo’s Famous Chili Dog, 5155 S. Main, Sylvania, inspected Dec. 10. No paper towels. Hand sinks must be stocked with hand soap, paper towels and hot/cold running water to allow proper hand washing. Corrected. No seven-day discard date label on open cole slaw. Corrected. Food on cooler floor; store food at least six inches off the ground to prevent contamination. Store potatoes off ground in the walk-in cooler. A leak in the walk-in cooler fan unit. The walk-in cooler must be in good condition to prevent food contamination. Repair leak immediately. Wiping cloth bucket made with bleach and soap. When making a sanitizer wiping cloth bucket, use regular unscented bleach with warm water. Use the chlorine test kit to check the concentration of sanitizer available. Change sanitizer bucket as needed. Inspector: Gottschalk.

Sylvania Country Club - Main Kitchen, 5201 Corey, Sylvania, inspected Dec. 12. Grease trap water leaking over the ground outside by the Dumpsters. Water is flowing out of one drain cover down the hill onto the golf course. Water is also flowing from another drain cover. Employee washing hands in the food-prep sink. Employees must wash hands in the hand sink. Found chili without required seven-day discard date. Chili discarded. Corrected. No paper towels available at a hand sink; properly stock hand sink with hand soap, paper towels, and hot/cold running water. Inspector: Gottschalk.

Secor Lanes, 5520 Secor, inspected Dec. 12. Prep-top cooler holding at 60 degrees. Cold hold food at 41 degrees or below to prevent bacteria growth. Food discarded. Wet wipe rags sitting out on counter; store rags in sanitizer bucket. Corrected. No wipe bucket set up as required. Corrected. Quat in three-bay bar sink above 400 ppm. When using quat as a sanitizer it should be at 200 ppm to prevent chemical poisoning/chemical residue. Corrected. Inspector: Stacy Seger.

Kroger, 833 W. Alexis, inspected Dec. 11. Chemical bottles hanging on hand sink in bakery. Do not store chemicals on hand sinks. No light shield in meat and seafood display cases. Light bulbs must be shielded, coated, or otherwise shatter resistant in areas where there is clean equipment, utensils, or linens; exposed food, or unwrapped single serve or single use articles. Provide light shields or rubber coated bulbs. Provide a working thermometer for cheese display case in center aisle. Provide test strips for all area sinks. Inspector: Seger.

The Andersons General Store, 4701 Talmadge, inspected Dec. 10. Found seven 7-ounce containers of Gerber squash for sale with an expiration date of October 2012. Check and rotate baby food items on a regular basis. Out-of-date baby food and formula may not be offered for sale. Bakery items with improper ingredient statements on their labels. Ingredients must be listed in descending order by weight for each variety of baked good. Corn meal stored below raw shell eggs in the refrigerated case. To prevent cross contamination, store raw meats, poultry, and shell eggs below or separate from ready-to-eat food. Inspector: Kelly Sattler.

Lido Lanes, 865 South, inspected Dec. 10. Nacho cheese in dispenser at 110 degrees. Food needs to be held at 135 degrees or higher to prevent bacteria growth. Repair or replace dispenser. Repeat violation. Obtain hand soap for hand sink by fryers. Hand soap needs to be available for proper hand washing. Hand sanitizer is not an approved method of hand washing. River Diner, 2040 Ottawa River, inspected Dec. 12. Uncovered cup of coffee stored on a shelving unit with food items. Employee beverages must have lids and be stored to protect food and clean items/surfaces from contamination. Corrected. Raw burger patties stored above ready-to-eat food items. To prevent cross contamination, store raw meats and shell eggs below other food items. Corrected. A soiled cloth stored on the cutting board of the prep-top cooler. When not in use, store damp/soiled cloths in the wipe cloth bucke. Corrected. Gravy and chili not properly reheated before placing into the steam table. Follow proper reheating procedure. Corrected. Egg mix at 50 degrees stored at room temperature. Do not store food at room temperature. Corrected. Repeat violation. Macaroni and cheese in the walk-in cooler was prepared on 11-26-12. To limit bacteria growth, ready-to-eat food items may only be held a total of seven days once cooked, prepared, or opened. Corrected. Repeat violation. Milk in cooler in server area at 51 degrees and the gauge on the outside of the cooler is reading 50 degrees. Repair cooler to cold hold at 41 degrees or colder. Product discarded that was held in the cooler. Inspector: Stanley.

Cock ’n’ Bull of Toledo, 9 N. Huron, inspected Dec. 10. Sanitizer not set up as required during hours of operation. Corrected. Prep-top cooler holding food at 45 degrees rather than at minimum of 41 degrees. Adjust cooler temperature. Provide an internal thermometer to monitor temperatures closely. Improper date marking of food. Overall, date marking must be addressed with entire staff. Seven days is the maximum holding period. Food handler preparing a salad with bare hands. To prevent contamination, ready-to-eat foods must be handled with a clean gloved hand. Hand and wrist jewelry are not permitted on food handlers. Inspector: Kelly Cipiti.

Hollingworth School, 590 Oswald, inspected Dec. 10. Paper-towel dispenser not working. Repair dispenser to ensure hands are properly washed. Inspector: Cipiti.

Nick & Jimmy’s Bar & Grill, 4956 Monroe, inspected Dec. 11. Beef tips in the walk-in cooler dated 11/27, which is beyond the seven-day maximum limit for holding of refrigerated, ready-to-eat, food. Make sure employees are changing dates and thoroughly cleaning containers between batches, including removal of stickers. Store wet wiping towels in sanitizer buckets when not in use to prevent bacteria growth. Inspector: Sattler.

Harnoor LLC d/b/a: East Side Mini Mart, 615 Oak, inspected Dec. 11. A can of insect killer spray stored by the ware wash area. Chemicals used for pest control must be applied by a licensed pest-control operator. Remove insect spray from facility. Power steering fluid being stored directly on top of the ice machine and bags of cat litter being stored directly on top of beverage complex. To prevent contamination, store these items below or away from food and food-contact areas. Inspector: Cipiti.

Frisch’s Big Boy, 2924 Navarre, Oregon, inspected Dec. 13. Several spray bottles without labels. Working bottles must be labelled with common names of contents for proper identification and to prevent contamination. Inspector: Brandon Tester.

The Original Royal Pizza, 4843 N. Detroit, inspected Dec. 10. Wipe cloth stored on a work surface and no wipe cloth bucket set up. During hours of operation, the facility must have a wipe cloth bucket set up to properly store damp/soiled cloths and to properly sanitize surfaces. Chlorine at 100 ppm. Cooked chicken below 135 degrees cooling at room temperature. To limit bacteria growth, once the food item reaches 135 degrees, an active cooling process must begin. Corrected. Inspector: Stanley.

Stop N Shop, 5409 Dorr, inspected Dec. 12. Bleach and drain chemical stored above food utensils and plates. Store chemicals separate from food utensils. Inspector: Kurt Susdorf.

In & Out Mart, 5101 W. Alexis, inspected Dec. 7. Oven cleaner stocked above food-contact items. Store chemical below food-contact items. No mop sink available and no hand sink available at the three-bay sink. A mop sink must be available at all times for filling and dumping mop water. A hand sink (not including restroom sink) must be available for proper hand washing. Contact a licensed plumber to install sinks and pull permits if required. Stock hand sinks with soap and paper towels. Inspector: Gottschalk.

Jonny Almond Nut Company, 5001 Monroe, inspected Dec. 10. Some pre-packaged, self-service items are missing ingredient statements. No ingredients listed for gummy bears and other foods that are packaged in mixtures. Pre-packaged, self-service food must be properly labeled. Inspector: Sattler.

Milo’s Food Market, 4508 Lewis, inspected Dec. 12. Potato soup for self service not labeled properly. Provide proper labels for all prepared food items. Facility is using ROP for lunch meat. Lunch meat does not meet the criteria for ROP and shortens the quality of the meat. Do not use this process for deli meat. Inspector: Mike Brady.

Star Bar, 5215 Monroe, inspected Dec. 11. No soap and paper towels available at the bar hand-washing sink. The bar must have a hand sink designated and stocked properly at all times with soap and paper towels. Corrected. Inspector: Sattler.

Sunset House - Main Kitchen, 4020 Indian, inspected Dec. 12. Lunch meat (turkey and ham) beyond the seven-day discard date in walk-in cooler. Monitor dates in cooler more closely. Lunch meat discarded. Several fruit flies by two-bay prep sink. Contact licensed pest-control company to treat fruit flies and have drain at prep sink cleaned out. Inspector: Susdorf.

Children’s Discovery Center, 228 N. St. Clair, inspected Dec. 7. Milk and ranch dressing sitting out at room temperature, holding above 41 degrees. Put these items inside cooler when not actively using them to prevent bacteria growth. The dish machine is not working and not being used. This is the only means of proper ware washing. Repair dish machine by Dec. 10. Inspector: Cipiti.

Jeannie’s Coffee Shop, 1 Goverment Center, inspected Dec. 11. Improper date marking observed. Deli meat was removed from the freezer with date on package of 11/15. Once removed the package was given seven-day date; this is not correct. Inspector: Cipiti.

McDonald’s, 5057 Monroe, inspected Dec. 12. Wet towels out on work surfaces in several areas. Wet wiping towels must be stored in sanitizer solution when not in use to prevent bacteria growth. Inspector: Sattler.

Marco’s Pizza, 3510 Dorr, inspected Dec. 11. The hand sink is draining very slowly. Unclog the plumbing and call a licensed plumber if necessary to service the sink. The sink must drain properly to allow for frequent and proper hand washing. Corrected. Inspector: Gillian Wilke.

Golden Koi Restaurant, 3550 Executive, inspected Dec. 12. Tofu sitting out at room temperature after being cooked at 77 degrees. This is a food and must be properly cooled under refrigeration and then held cold at 41 degrees or lower at all times. Rice noodles are sitting out at room temperature at 57 degrees. Once they are cooked or water is added, these must be held cold at 41 degrees or lower under refrigeration. Noodles were refrigerated. Keep wet wiping cloth in sanitizer bucket when not in use to prevent bacteria growth. Inspector: Sattler.

Subway, 1465 Secor, inspected Dec. 11. The under-counter cooler is holding at 45 degrees. Food shall be held at 41 degrees or lower to prevent bacteria growth. Adjust the cooler accordingly and call for service if needed. Inspector: Wilke.

Toledo Country Inn & Suites, 541 W. Dussel, Maumee, inspected Dec. 11. Single-service items being held in frozen cold-hold containers at 61 to 62 degrees. Food held cold must maintain an internal temperature of 41 degrees or below. Either obtain backup frozen containers to rotate more frequently or utilize mechanical cooler available on counter. Facility is operating without a sanitizer solution setup in place. To prevent potential cross contamination, a working supply of sanitizer should be set up during operation. Inspector: Paul Bauman.

Glass City Bar & Grill, 1705 Tollgate, Maumee, inspected Dec. 11. Fish in reduced-oxygen packaging being held in a thawed state under refrigeration. To prevent the growth of potentially harmful anaerobic bacteria, fish in reduced-oxygen packaging must remain frozen. To store fish in a thawed state, it must be removed from this packaging. Repeat violation. Inspector: Bauman.

Dale’s Bar & Grill, 322 Conant, Maumee, inspected Dec. 11. Ready-to-eat food being held without date marking. Open/prepared ready-to-eat food held longer than 24 hours must be marked with a seven-day discard date. Some date marking, but not all required products were dated. Inspector: Bauman.

La Fiesta Mexican Restaurant, 1406 Reynolds, Maumee, inspected Dec. 11. High-temperature dish machine not reaching 180 degrees on rinse cycle. To effectively sanitize, this machine must reach a rinse temperature of 180 degrees. Contact service to repair machine. Inspector: Bauman.

Ann & Tony’s Learning Center II, 3613 Monroe, inspected Dec. 11. Bleach concentration in three-bay sink at 200 ppm. Bleach should be 50 to 100 ppm to prevent chemical contamination. Corrected. Wipe cloth resting outside sanitizer. When not in use, wipe cloths should be placed in sanitizer solution to prevent bacterial growth. Corrected. Inspector: Nathan Fries.
