Violations:

Wendy’s, 2866 Navarre, Oregon, inspected March 27. Chocolate and vanilla Frosty mix in both machines holding above 41 degrees. To limit bacteria growth, food in cold holding must maintain 41 degrees or below. Product discarded. Chemical spray bottle stored on shelf with food and food-contact items. Designate an area for employee belongings below or away from food and food-contact items. Corrected. Inspector: Kerry Stanley.

Ann’s Grill, 1008 Hawley, inspected March 27. A food-safety trained person in charge is not on staff during hours of operation. Seek out training to be aware of biological, chemical, and physical contamination threats and their interventions. Unprotected ceiling lights and fixtures in both kitchen areas. Repair/​replace so to prevent glass/​brittle plastic contamination. Floors need repair/​cleaning. Inspector: John Markan.

Family Food Center, 1212 N. Detroit, inspected March 27. No food-safety trained person in charge on site. Take training to raise awareness of biological, chemical, and physical contamination interventions to identify/​control risk at first opportunity. Probing thermometers and sanitizer test strips not in use or present. Must verify correct temperatures of equipment and foods. Do same for sanitizer strengths by using correct and good quality test strips. Both items needed to prevent biological issues. No sanitizing of equipment and utensils. Three-bay sink and wipe cloth sanitizer buckets not in use in meat cutting/​packaging room. Use to reduce biologicals and prevent contamination. Unprotected ceiling lights, especially in meat cutting/​packaging areas. Pests and vermin can gain entry at receiving/​delivery door and exit door. Observed quarter-inch or greater gaps at bottom of doors when closed. Repair/​replace barriers and get a pest-control contractor involved. Inspector: Markan.

Cigarettes Outlet of Toledo LLC, 2620 Airport, inspected March 24. Lack of food-safety knowledge. Training needed for biological, chemical, and physical interventions for contamination, including cleaning and sanitation for beverage machines (Bunn smoothie and cappuccino types). Inspector: Markan.

Los Dos Gallos, 5327 Dorr, inspected March 24. Grease traps for prep sink and three-bay sink are not fitted with sealed lids, making them open to kitchen. Seal correctly to prevent airborne contaminants or floor contamination. Design seal properly to simplify routine maintenance. Staff described a twice-a-month clean-out maintenance schedule. Unprotected light fixtures in storage room and in bar ice chests areas. Fit with a breakage-protective covering or use poly coated bulbs or/​tubes to keep glass out of food and off food-contact surfaces. Inspector: Markan.

Burger King, 6630 Airport, Holland, inspected March 25. Food stored on floor of walk-in freezer. All food must be stored six inches off the ground to prevent cross contamination. Corrected. Inspector: Nathan Fries.

Penn Station East Coast Subs, 1266 S. Holland Sylvania, Holland, inspected March 26. Blanched potatoes cold holding above 41 degrees. Food should be held below 41 degrees to prevent bacteria growth. Corrected. Inspector: Fries.

Holland Carry Out, 6901 Angola, Holland, inspected March 26. Raw bacon stored above ready-to-eat food/​beverages. Raw bacon should be stored below ready-to-eat food to prevent cross contamination. Corrected. Inspector: Fries.

Papa John’s Pizza, 3150 Navarre, Oregon, inspected March 27. Sauce cups labeled Keep Refrigerated and burger sauce that requires refrigeration held at room temperature above 41 degrees. Adhere to handling labels to limit bacteria growth. Product moved to refrigeration. Inspector: Stanley.

Walgreen’s, 5815 Secor, inspected April 1. Four bottles and two cans of formula that have expired. Formula and baby food items must be discarded once the expiration date has been reached. Discarded. Inspector: Stacy Seger.

Target, 817 W. Alexis, inspected April 1. Expired cans of baby formula available for purchase. Baby food and formula must be discarded once it reaches expiration date. Items must be rechecked for expiration dates. Inspector: Seger.

Dollar General, 3249 Navarre, Oregon, inspected March 26. Expired baby food on sales shelf. Baby-food items may never be sold past manufacturer expiration date. Pulled from sale. Inspector: Stanley.

Stop & Shop, 4001 N. Holland-Sylvania, inspected March 26. Found tornados at 94 degrees and 98 degrees in the warmer. Time/​temperature control of safety food items must be held hot at 135 degrees or higher to prevent bacteria growth. Tornados discarded. Inspector: Jennifer Gottschalk.

Toledo Market and Restaurant, 3410 Dorr, inspected March 24. Cooked squash cold holding at 60 degrees. Food must be kept at 41 degrees or lower to limit growth of unwanted organisms. Food discarded. Inspector: Megan Lane.

Stop & Shop, 801 S. Byrne, inspected March 31. Lack of food-safety training and comprehension. Secure training to prevent biological, chemical, and physical contamination issues. Hand-washing sink and three-bay sink not available for use, such as for cleaning Bunn smoothie/​slush machines and hot cappuccino machines. Equipment must be cleaned/​sanitized in a three-bay sink and not nearby retail customer hand sink or any hand sink. This prevents contamination/​cross contamination. Inspector: Markan.

Ya Halla, 2124 W. Alexis, inspected March 28. Wipe buckets with 0 ppm bleach. Wipe buckets must maintain a concentration of 50-100 ppm bleach for properly sanitizing surfaces. Inspector: Seger.

Cold Stone Creamery, 7633 Sylvania, Sylvania, inspected March 27. Bottle of Pine-Sol cleaner stored on top shelf above food containers. Store chemicals in one area below food and food-contact items to protect from cross contamination. Corrected. Inspector: Gottschalk.

Stop & Go, 1806 Arlington, inspected March 26. Cream dispenser holding cream above 41 degrees. Food shall be held at 41 degrees or lower to prevent bacteria growth. Cream discarded. Repair dispenser to hold product cold at 41 degrees or lower before using it again. Facility offering self-serve doughnuts in a case but does not have a booklet of ingredients for all doughnuts provided. Contact Tom’s Doughnuts to provide a comprehensive list of doughnuts that may be offered and a list of ingredients for each variety. Raw bacon stored above ready-to-eat food in cooler. Kentucky Fried Chicken, 2224 Woodville, Oregon, inspected March 27. Mashed-potato bowls being prepared on a tray on a garbage can. Food and food procedures must be protected from contamination. Preparation must occur in a clean and dry area. Products prepared on garbage can were discarded. Never use a garbage can as a prep or work surface. Inspector: Stanley.

K&J House of Meats, 3821 W. Alexis, inspected March 26. No date marking on open hot dogs in deli-service counter. Open and prepared time/​temperature control for safety food must be dated with a seven-day discard date. Day product is open or made is first of seven days. Salads must also be dated. A sticker can be placed on back of white name plate to alert employees, or use a dry-erase board by deli case to keep track of discard dates. Unlabeled spray bottle and chemical spray bottles hanging on rack and wire shelf above food containers. Chemical bottles must be labeled to allow users to know contents. Store chemicals on bottom shelf, below food containers, to prevent cross contamination. Corrected. Monitor cross contamination in meat room. Only work with one species of meat at a time on the same table to prevent cross contamination. Sanitize tables between species of meat. For example, employee was wrapping chicken with ingredients and a piece of beef was on the same table. Do not place beef on table until it is cleaned and sanitized after raw-chicken prep. Andrea’s Sports Pub, 3338 W. Alexis, inspected March 26. Shell eggs stored above lettuce in two-door door cooler. Store eggs on bottom shelf, below all cooked and ready-to-eat food, to prevent cross contamination. Inspector: Gottschalk.

Dollar General Store, 6739 W. Sylvania, Sylvania, inspected March 25. Out-of-date baby food on sales shelf. Check dates often and remove when past discard date. Baby medicine and Vicks stocked over drinks in baby-food aisle. Move drinks to top shelf, above chemicals, to protect from cross contamination. Inspector: Gottschalk.

Tim Hortons, 161 W. Dussel, Maumee, inspected March 25. Steam table holding hash browns at 91 degrees and macaroni and cheese at 125 degrees. These foods must be held hot at 135 degrees or higher at all times. Discard out-of-temperature food and adjust hot-holding unit. Spray bottles in use with no labels with contents’ names. Insufficient hot-water flow at back hand-washing sink. Adjust water flow to provide adequate 100-degree water for hand washing at all times. Dishes stored in food-prep sink. Sink must be kept clean at all times and may only be used for food preparation. Inspector: Kelly Sattler.

McDonald’s, 831 W. Alexis, inspected March 28. Except for a plain wedding band or a medical-alert bracelet, food employees may not wear jewelry on their arms or hands while preparing food. Medical bracelets shall be worn in a manner that does not pose a risk to the food. Damaged/​hanging light shield in dry storage area. Lights must be properly shielded. Replace light shield. Dish machine with no detergent and no alarm system. Ware-washing equipment shall contain a wash solution of soap, detergent, acid cleaner, alkaline cleaner, degreaser, abrasive cleaner, or other cleaning agent according to the cleaning agent manufacturer’s label instructions. Buildup on drive-thru pop system. Equipment, food-contact surfaces, and utensils shall be clean to sight and touch. Non-food-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Properly clean and sanitize beverage system. Inspector: Seger.
