Recently released inspection reports of Lucas County food-service operations

Violations:

McDonald’s, 1401 Arrowhead, Maumee, inspected May 8. Raw shell eggs holding cold at 49 degrees in reach-in cooler in prep area. Raw eggs must be held cold under refrigeration at 45 degrees or lower. Eggs out too long for prep. Limit time foods are left out for prep so that temperature does not rise. Wet wiping towels out on counter at beverage area. Wiping towels must be stored in sanitizer when not in use to prevent bacteria growth. Inspector: Stacy Seger.

Cousino’s Steak House, 1842 Woodville, Oregon, inspected May 19. Several foods out of temperature in prep-top coolers, with lids not closed properly or broken and food double-panned. Also, ranch dressing out of temperature in improper ice bath. Cold holding must be maintained at 41 degrees or below to prevent bacteria growth. Set out smaller portion of ranch dressing when icing and ensure ice touches four sides of containers and matches food level. Dressing discarded. Inspector: Seger.

Koney Grill LLC, 30 E. Alexis, inspected May 15. Grease buildup on hood system. Hood and hood vents must be cleaned regularly to limit buildup. Bag of onions stored directly on floor. Food must be stored a minimum of six inches above floor to prevent contamination. Children in the back of the house. Persons not related to food-service operation or retail food establishment are not allowed in food preparation, food storage, or ware washing areas, except that brief visits and tours may be authorized by person-in-charge if steps are taken to ensure that exposed food; clean equipment, utensils, and linens; and unwrapped single service and single use articles are protected from contamination. Unlabeled spray bottle hanging from storage shelving unit near three-bay sink. To prevent chemical contamination, label spray bottles with contents‘ common names. Inspector: Seger.

Circle K, 1565 E. Alexis, inspected May 7. Buildup inside drain tray of pop machine. Equipment must be cleaned regularly to prevent bacteria buildup. Open-air display cooler holding at unsafe temperatures 54 to 57 degrees. Food must be held at 41 degrees or below to limit bacteria growth. Food discarded. Rear hand sink with no soap or hand towels. Hand sinks must be fully stocked with warm running water, soap, and hand towels to ensure proper hand washing. Inspector: Seger.

Rite Aid, 4869 N. Summit, inspected May 8. Two cans of outdated formula. Baby food/​formula may not be sold past expiration date. Inspector: Seger.

Courtyard by Marriott, 1435 E. Mall, Holland, inspected May 15. Hand sink blocked by food cart. Hand sinks must be accessible at all times to facilitate proper hand washing. Chemical cleaners on top of coffee maker. Chemicals should be stored below and away from food and food-contact items to prevent chemical contamination. Inspector: Nathan Fries.

Bed Bath and Beyond, 1230 S. Holland-Sylvania, Springfield Township, inspected May 15. Food stored less than six inches off floor. Food must be stored six inches off ground to prevent cross contamination. Inspector: Fries.

McDonald’s, 7159 W. Central, inspected May 15. Lack of time stamps on food held time in lieu of temperatures. Foods held time-in-lieu must have a four-hour discard time marked on them to prevent bacteria growth. Inspector: Fries.

Carmel’s Mexican Restaurant, 2947 Tremainsville, inspected May 12. Food held at unsafe temperature in six-drawer prep cooler. Food must be held at 41 degrees or below to prevent bacteria growth. Items discarded. Inspector: Kelly Cipiti.

Fifth Third Field - Birdcage Grill, 406 Washington, inspected May 12. Sanitizer in three-bay sink below 150 ppm. Empty bottle replaced and sanitizer verified at 200 ppm. Bagged romaine lettuce requires washing. Person-in-charge stated lettuce is washed in three-bay sink. This is not acceptable. No food-prep sink at this stand. Produce must be washed first in main kitchen and brought to stand in a food-grade container. Food handlers changing gloves but not stopping to wash hands between glove changes. To ensure proper hand washing, review procedures with food handlers. When switching from task to task and touching handles of coolers, etc, food handlers must stop, wash hands, and put on new gloves. Fifth Third Field - Traditional, 406 Washington, inspected May 12. Portioned containers of cheese holding at 117 degrees in pretzel warmer. Hot holding must be maintained at 135 degrees or above to prevent bacteria growth. Cheese discarded. Inspector: Cipiti.

Fifth Third Field - Traditional, 406 Washington, inspected May 12. Low-temperature dish machine not dispensing chlorine sanitizer at final rinse. Repeat violation. Do not use machine until repaired and verified at 50 ppm chlorine during final rinse cycle. Inspector: Cipiti.

Fifth Third Field - Home Run Hot Dogs, 406 Washington, inspected May 12. Prep-top cooler holding food above 41 degrees: tomatoes at 55 degrees, cheese 54 degrees, and cole slaw 56 degrees. Food must be held at 41 degrees or lower to prevent bacteria growth. Contact licensed repair service to fix cooler so it holds proper temperature. Repeat violation. Inspector: Cipiti.

Horizon Science Academy, 425 Jefferson, inspected May 9. Ranch dressing at 50 degrees and milk at 49 degrees. Cold food must be held at 41 degrees or below to prevent bacteria growth. Monitor delivery temperature and do not receive items that are above 41 degrees. Place directly into cold holding to maintain safe temperatures. Inspector: Cipiti.

Wendy’s, 108 Main, inspected May 8. Food in prep-top cooler closest to front counter holding at 50 degrees. Discard items that require cold holding: lettuce, blue cheese, sliced tomatoes, any sauces. Food must be held cold at 41 degrees or below to prevent bacteria growth. Monitor temperature closely to ensure safe holding. Corrected. Inspector: Cipiti.

Aurora Academy, 541 Utah, inspected May 8. Store raw shell eggs on bottom shelf to prevent cross contamination. Inspector: Cipiti.

Packo’s at the Park, 7 S. Superior, inspected May 7. Cole slaw on ice holding at 50 degrees. Maintain cold holding of this food at 41 degrees or below to prevent bacteria growth. Ice must surround food container on four sides and up to food level, not just on bottom of pan. Food discarded. Prep-top cooler holding food at 53 to 57 degrees. Maintain cold holding temperatures at 41 degrees or below to prevent bacteria growth. Unit not turned on before filling it with food. Dumplings holding at 117 degrees in table-top warmer. Maintain hot food at 135 degrees or above to prevent bacteria growth. Food reheated to 165 degrees in steamer. Use a thermometer to verify safe temperatures. Inspector: Cipiti.

Marshall Elementary School, 415 Colburn, inspected May 7. Cutting board used to cut melons has deep cuts, grooves, scratches. Surface is no longer smooth, cleanable, and non-porous. Repair/​replace and establish an internal preventive maintenance program to maintain smooth, cleanable, and non-porous surfaces. Melons stored under refrigeration packed in clear bins. Administration to assess risk of this practice. Current practice is to prep at dishwasher unit of three-bay sink rather than separated, stand-alone stainless steel table and nearby prep sink. Issue is cross contamination with bacteria and allergens. Cutting knife not stored in sanitary manner, resting on a cloth in rack storage. Current practice is to clean/​sanitize and store in a smooth, cleanable, and non-porous surface. Staff explained this as a infrequent occurrence. Inspector: John Markan.

The Robin Nest, 361 Teach-out, Curtice, inspected May 12. Raw bacon stored above ready-to-eat food (pickle juice). Practice proper storage order: raw bacon should be stored below ready-to-eat food to prevent cross contamination. Corrected. Inspector: Emily Anderson.
