● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

● A food employee touched ready-to-eat food with bare hands.

● The ice scoop was inside the ice bin making direct contact with ice.

● Raw red meat was stored above cut lettuce in walk-in cooler.

● Guacamole was not being held at the proper temperature.

● Cut lettuce products were not properly date marked.

● Ice maker, ice maker scoop rest, utensil holder and knives were dirty.

● There was an air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water.

● Hand washing sink is being used for purposes other than hand washing.
