NORWALK, Ohio — American Excelsior Co. has been fined $65,000 by the Occupational Safety and Health Administration for 12 alleged serious safety violations. OSHA said two workers suffered injuries because of a lack of machine guarding at the Norwalk production facility.

OSHA initiated its inspection in May after receiving complaints alleging unsafe working conditions.

The Norwalk Reflector newspaper reported that one worker suffered broken bones in June when his arm got caught in a machine. Details of the other injury were unavailable.

The 12 serious citations include a lack of personal protective equipment, a failure to have standard guard and stair railings, a failure to train workers in lockout/ tagout procedures, and a lack of machine guarding on various pieces of equipment.

American Excelsior, which makes custom foam products for packaging and other products, has 15 days to appeal the ruling or pay the fine.
