Burger King, 1856 Laskey, inspected June 5. Critical violation: Creamer inside small single-door reach-in cooler by drive thru was at 48 degrees and milk was 44 degrees. All potentially hazardous food (PHF) must be held at 41 degrees or less to prevent bacteria growth. Repair service call. Creamer discarded. Two noncritical violations. Inspector: Gillian Wilke.

Lux, 707 Matzinger, inspected June 5. Critical violation: Observed buildup on inside of soft drink gun. Clean and sanitize on routine basis. Allow to air dry before using. One noncritical violation. Inspector: Kerry Cutcher.

Gateway Middle School ball field, 900 Gibbs, Maumee, inspected June 7. Critical violations: Person in charge initially denied inspector access. Hot dogs found holding at 47 degrees in a non-commercial cooler/freezer. All PHF must be held cold at 41 degrees or less. Non-commercial unit is not holding at proper temperatures, this unit must be replaced with a commercial NSF approved unit. Hot dogs discarded. All PHF opened must have a seven-date expiration date. Hot dogs have no date marking. Hair restraint must be present on employees working with open food. Inspector: Sara Becker.

Biggby Coffee, 4204 Sylvania, inspected June 9. Critical violation: Observed wet wiping cloths on prep surfaces. All wiping cloths must be stored in sanitizer, when not in use, to prevent bacteria growth. Four noncritical violations. Inspector: Kelly Sattler.

Packo s at the Park, 7 South Superior, inspected June 9. Critical violation: Observed food holding at 43-46 degrees in the cold well front counter. To prevent bacteria growth, hold food at 41 degrees or lower. Call service. Maintain proper temperatures. Two noncritical violations. Inspector: Kelly Cipiti.

Critical violations: Observed various PHF in main walk-in cooler, two-door salad cooler, and two-door under counter unit holding for at improper temperatures. Adjust or repair units so proper temperatures are maintained. Observed egg wash and deli meat held at room temperature during busy times at 47-62 degrees. All PHF must maintain 41 degrees or less to limit bacteria growth. Keep egg wash on proper ice bath and deli meats in two-door under counter cooler. Observed items without proper date marking. No hot water available at hand sinks in both men s and women s rest rooms. Hot water at minimum of 100 degrees is required at hand sinks. One noncritical violation. Inspector: Kerry Cutcher.

Jimmy John s, 405 Adams, inspected June 10. Critical violations: Bleach in sanitizer wiping bucket too strong at 200 ppm dilute to 100 ppm. Chemical spray bottles not properly labeled. Label containers with common name. Five noncritical violations. Inspector: Wilke.

Pita Sub, 5155 Main, Sylvania, inspected June 10. Critical violation: No date marking observed. All PHF kept longer than 24 hours must be properly date marked. Two noncritical violations. Inspector: Brandon Tester.

Cafe Marie, 3344 Secor, inspected June 11. Critical violations: Chicken salad is stored in a pan that is too shallow for the prep top cooler and is holding at 44 degrees. All PHF must be held cold at 41 degrees or less at all times. Use deep pans for items in this cooler. Omelette mixture is holding at 48 degrees and the ambient air temperature for this cooler is at 45 degrees. All coolers must hold PHF at 41 degrees or lower. This is a repeat violation. If cooler cannot be repaired it must be replaced. High temperature dish machine is not properly sanitizing dishes at time of inspection. The final rinse is not reaching 180 degrees. Repair immediately. Observed wet wiping cloths out on counter. Store wet wiping cloths in sanitizer solution of proper concentration when not in use. Four noncritical violations. Inspector: Sattler.

Heartland Holly Glen, 4293 Monroe St. inspected June 11. Critical violations: The milk holding in the glass door reach-in was at 45 degrees and cooler at 44 degrees. All PHF must be held at 41 degrees or less to prevent bacteria growth. Adjust the cooler temperature accordingly. Three noncritical violations. Inspector: Wilke.

King s Taste, 5141 North Summit, inspected June 11. Critical violations: No date labels on cut ham. All PHF must be properly date marked. Eight noncritical violations. Inspector: Jennifer Gottschalk.

McDonald s, 5057 Monroe, inspected June 11. Critical violation: Sanitizer in wipe cloth bucket is weak. Maintain chlorine sanitizer at 50-100 ppm. Two noncritical violations. Inspector: Sattler.

Playmakers Toledo Sports Eatery, 2600 Sylvania, inspected June 11. Critical violations: Date mark items such as ranch dressing that is being made. Observed sliced pizza cheese at 44-48 degrees on make table. Rearrange prep top to provide a place for the cheese so it maintains proper temperature. Cheese removed from service. Observed moisture and buildup underneath white boards on pizza make table. Wash, rinse, sanitize, and air dry these daily. Fifteen noncritical violations. Inspector: Dacquisto.

Mancy s Bluewater Gille, 461 West Dussel, Maumee, inspected June 12. Critical violations: Prep-top cooler holding food at 44 degrees. Two-door, under-count cooler holding food between 50-52 degrees. All PHF held cold must maintain an internal temperature of 41 degrees or less. Raw shell eggs stored above cut potatoes in walk-in cooler. To prevent potential cross contamination, store all raw eggs on the bottom shelf of coolers. Inspector: Paul Bauman.

Southbriar, 5147 Main, Sylvania, inspected June 12. Critical violations: Observed cooked chicken stored under raw beef and pork. Proper storage order: chicken and eggs on bottom, then ground meats, then whole meat and fish and the cooked and ready-to-eat foods on top to prevent cross contamination. Observed several PHF above 41 degrees. All PHF must be held at 41 degrees or less to limit bacteria growth. Potato pancake mix discarded. Closely monitor all coolers for proper temperature. Five noncritical violations. Inspector: Tester.
