Bravo! Cucina Italiana, 5001 Monroe, inspected Nov. 25. Critical violation: Observed wet wiping towels stored on countertops and a sanitizer bucket at 100 ppm quat; store towels in sanitizer bucket and maintain quat sanitizer at 200 ppm. Facility not currently permitted to hold food by time in lieu of temperature, so food must be held at 41 degrees or less and 135 degrees or greater. Food must be time marked for expiration. Mad Dog Pub and Grub, 1014 Starr, inspected Nov. 24. Critical violations: Observed hot dogs dated Nov. 13; only hold open cooked foods for a total of 7 days. No date labels on cooked taco meat in walk-in cooler. Observed sliced cheese at 50 degrees in prep cooler; maintain at 41 degrees or lower. Do not double stack on the container. Food must fit in the pan inside the prep cooler to remain cold. Observed eggs and turkey stored above the beer in walk-in cooler. Raw eggs and raw meat must be stored on lowest shelf, below pop and beer. One noncritical violation. Inspector: Gottschalk.

Cafe Lourdes, 6832 Convent Blvd., Sylvania, inspected Nov. 24. Critical violation: Observed no date marking on PHFs in snack bar display cooler and kitchen 2-door upright cooler. Must date all PHFs kept longer then 24 hours. Good for 7 days at 41 degrees. One noncritical violation. Inspector: Tester.

Permanent Stand 2, Glass Bowl, 2801 West Bancroft, inspected Nov. 21. Critical violations: Facility not licensed to cater and may not hold food for catering. Observed employee open a covered pan of rice, grab rice with bare hands and eat it. No bare hand contact can be made with ready to eat foods and employees cannot eat in food prep areas. One noncritical violation. Inspector: Kelly Sattler.

Sylvania Diner, 5623 West Alexis, inspected Nov. 21. Critical violation: Label all spray bottles with common name of contents for proper identification. Observed improper thawing of ham inside prep sink at room temperature. Observed several PHFs in facility without date marking. Observed improper storage order of foods. Upright beverage cooler holding foods at unsafe temperatures. Two noncritical violations. Inspector: Tester.

Ground Round, 5806 Telegraph, inspected Nov. 21. Critical violations: Observed raw fish stored on shelf above lettuce in worktop cooler, corrected. Observed PHFs in condiment prep cooler at 44 to 45 degrees instead of 41 degrees or less; adjusted at inspection. Observed cooked rice improperly cooled and packaged in prep cooler at 51 degrees; cool rapidly to 41 degrees or less before portioning and placing in prep cooler. Observed bar 3-vat sink at 0 ppm of sanitizer; quat must be at 200 ppm or chlorine at 50-100 ppm; corrected at inspection. Five noncritical violations. Inspector: Kerry Stanley.

Elephant Bar Restaurant, 5001 Monroe, inspected Nov. 20. Critical violations: Several sanitizer buckets tested low for quat sanitizer at inspection; maintain quat at 200 ppm and keep wet wiping cloths in sanitizer when not in use. Dish machine not dispensing chlorine at inspection because of air bubble in line; check before washing dishes. Observed beans at 103 degrees after cooling over 2 hours. Beans not properly iced at time of inspection. Beans reheated to at least 165 degrees, repeat violation. Pork chops at 126 degrees in the Alto Sham; PHFs must be held hot at 135 degrees or more at all times. Three noncritical violations. Inspector: Sattler.
