Violations:

● There was buildup in a soda gun dispenser.

Vito’s Management Co. LLC, 7060 W. Sylvania, Sylvania, inspected June 8.  

● An open bag of thawed chicken in the cooler didn’t have date markings indicating when the food should be discarded.

Subway, 5141 S. Main, Sylvania, inspected June 12.  

● There was buildup inside the ice scoop container.

● There was a dark buildup along the ice machine catch plate.

Sidelines Sylvania, 6060 Renaissance Place Unit G, Sylvania Township, inspected June 11.  

● There was no designated person in charge present in the food facility.

● Several pans of food being stored in a cooler on the line did not have any date markings on them indicating when the food should be discarded.

● Several food containers of food being stored in coolers had expired dates.

● The hand-washing sink was not easily accessible.

The Toledo Club, 235 14th, inspected June 6.  

● A wait service employee touched their nose area and then inserted their fingers into clean beverage containers.

● A dishwasher was pushing dirty dishes over to the dishwasher and then taking clean items off of clean end of machine without washing hands and putting new pair of gloves on.

● Filled pitcher(s) were sitting directly over the ice in an ice bin and condensate drops from the pitcher went into the ice bed directly below, contaminating the ice.

● The ice maker was dirty in some areas.

● There was a buildup of dirt and food debris underneath coffee machine dispenser.

Mr. Gyros & More, 3833 Airport, inspected June 6.  

● Person in charge did not ensure their employees are properly trained in food safety.

● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

● Raw chicken was stored above pickles in the walk-in cooler.

● Food items were not being held at the proper temperature.

● Mac and cheese, cooked rice were not properly datemarked to indicate when the food should be discarded.

● No sanitizer setup was present at the beginning of the inspection.

● Chemical containers were not properly labeled with the name of the material.

● There was no person in charge present in the food facility.

● There was buildup on the coffee machine.

● The hand-washing sink was being used for purposes other than hand washing.

Riverview Yacht Club, 5981 Edgewater, inspected June 7.  

● Marinara sauce and red pepper pesto were not properly date marked to show when the food should be discarded.

● Prime rib and ribs did not have date markings to indicate when the food should be discarded.

Olive Garden Italian, 1919 S. Reynolds, inspected June 7.  

● Lettuce, sliced tomatoes were not being held at the proper temperature.

● There was minor buildup inside the ice machines.

Old West End Daycare II, 1723 Broadway, inspected June 7.  

● There was no person in charge present in the food facility.

● The concentration and/​or temperature of a chlorine sanitizing solution did not meet the minimum requirements.

Meijer, 7240 W. Central, inspected June 7. 

● Water dispensed from bulk water machine had not been sampled at the required frequency or retained at the facility.

Making A Difference II, 347 S. Reynolds, inspected June 6. 

● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

Kentucky Fried Chicken, 6790 W. Central, inspected June 7.  

● Sanitizing solution in one sink did not meet the minimum requirements.

● There was buildup inside the soda nozzles.

Caribbean Breeze LLC, 1830 W. Bancroft, inspected June 7.  

● There were no signed employee heath record form at the facility as required.

● Goat stew, oxtail, curry goat, curry chicken, and white rice were not being held at the proper temperature.

● There were no date markings on a significant amount of food items inside a double door reach-in cooler.

Andrea’s Sports Pub, 3338 W. Alexis, inspected June 7.  

● Cheese was not properly date marked to indicate when it should be discarded.

● There was buildup inside a soda gun.

● The hand-washing sink was being used for purposes other than hand washing.

Wesley’s, 1201 Adams, inspected June 8.  

● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

● Ice maker was dirty.

● Ice maker scoop was resting on a dirty surface.

Tim Horton’s, 1801 W. Laskey, inspected June 8.  

● The hand-washing sink was being used for purposes other than hand washing.

Taco Bell, 1317 N. Reynolds, inspected June 8.  

● Bulk creamer was open and in use with the date marking not legible or removed so employees can’t tell when it should be discarded.

● Lips of stacked cups for customers were touching the sidewall (vents) TB Freeze machine and the lips of stacked customer cups. There was heavy dust present on these sidewalls.

● Raw shell eggs were stored above milk and water inside a beverage walk-in cooler.
