 Jerome Davis, 62, of Toledo, Dec. 2, in 9800 block of Oak Place, Holland. Accidental, combined drug toxicity (principally fentanyl).

Juan Guzman, 28, of Lincoln Park, Mich., Nov. 21, at ProMedica Toledo Hospital. Accidental, blunt head trauma; deceased fell through roof while working.

Osama Hajeir, 34, of Toledo, Feb. 2, at ProMedica Toledo Hospital. Accidental, acute combined drug intoxication (fentanyl, cocaine and alprazolam).

Theodore Harrison, 55, of Toledo, Jan. 30, at home. Suicide, self inflicted gunshot wound of the head.

Leah Jones, 38, of Toledo, Dec. 1, at University of Toledo Medical Center. Accidental, fentanyl toxicity.
