Install food prep sink with air gap connected to floor drain. Provide plan for where this will be installed. Inspector: Debbie Dacquisto.

Carlita's Mexican and Italian Caf , 4709 Douglas, inspected May 22. Critical violations: keep all food items covered in reach-in freezer. Prep sink must have 2-inch air gap installed. Inspector: Kevin Halligan.

Hops Restaurant Bar and Brewery, 461 Dussel, Maumee, inspected May 23. Critical violation: repair dish tank ASAP; chlorine not dispensing. Sanitizer must be checked every shift; start a daily log. Inspector: Halligan.

Lickity Split, 2021 Glendale, inspected May 19. Critical violation: do not keep basement window open with garden hose attached. Keep window clear to prevent pest entrance. Inspector: Mike Brady.

Marco's Pizza, 2116 South Reynolds, inspected May 20. Critical violation: garlic sauce must be kept refrigerated at all times; do not leave out at room temperature. Inspector: Brown.

University Carry Out, 1852 West Bancroft, inspected May 19. Critical violations: large walk-in cooler not keeping proper temperatures, foods discarded. Provide hot water for hand sink; water heater not on. Inspector: Dacquisto.

Dollar Dollar Plus, 1341 Dorr, inspected May 21. Critical violation: all foods items must be kept 6 inches off floor, repeat violation. Inspector: Steve Perrine.

Speedway, 5904 West Central, inspected May 21. Critical violation: one package of frozen hot dogs not date marked, removed from freezer. Temperature of cold food display not being written down; temperatures for all coolers must be logged daily. Inspector: Herzberg.

Tim Horton's, 2621 Navarre, Oregon, inspected June 5. Critical violations: Observed food prep personnel rinsing tuna in three-compartment sink, this practice could contaminate food items. Make sure food prep sink is being utilized for food prep. Make sure food prep handlers used gloves when preparing ready-to-eat foods to prevent contamination. Inspector: Heidi Sanders.

Golden Jade, 3171 Navarre, Oregon, inspected June 5. Critical violations: Observed mold buildup in soft drink nozzles. Nozzles must be soaked nightly to prevent mold buildup. Sanitizing cloths must be kept in sanitizing solution when not in use. Dish machine is not sanitizing at proper sanitizing strength. Provide proper test strips for sanitizer buckets. Cooley Canal Yacht Club, 12235 Bono, Curtice, inspected June 6. Critical violations: Provide sanitizer buckets for sanitizing equipment and surfaces. Make sure all utensils are stored handles up to prevent contamination of service end of utensil. Inspector: Sanders.

Darlington Nursing and Rehab Center, 2735 Darlington, inspected June 9. Critical violation: Cheese sauce found cooling in a covered deep pan in the walk-in cooler. Transfer sauce to a shallow, two-inch pan to cool. Inspector: Dacquisto.

Rosie's, 5815 Secor, inspected June 10. Critical violations: Keep wipe cloths in sanitizer buckets when not in use. Keep all shallow pans of food kept covered in walk-in cooler to avoid possible cross contamination. Label all bulk food items and bulk spices to avoid potential cross contamination. Kmart, 2830 Navarre, Oregon, inspected June 9. Critical violation: Prep top found out of temperature (50 degrees.) All potentially hazardous foods must be discarded and cooler must be repaired. Inspector: Sanders.

Summit Carryout, 3740 North Summit, inspected June 9. Critical violations Observed employees food stored in the last door of the 15-door cooler — no food for sale was stored in the last three doors. Do not store employees food in the same reach-in cooler that stores for sale. Remove employee food. Inspector: Jennifer Gottschalk.
