Recently released inspection reports of Lucas County food-service operations:

Critical violations

Subway, 1355 South McCord, Holland, inspected June 8. Walk-in cooler, front prep top cooler, and beverage cooler holding TCS foods (time/temperature control for safety) between 42-44 degrees. All TCS foods, held cold, must maintain an internal temperature of 41 degrees or below. Contact repair service and adjust as necessary. Inspector: Paul Bauman.

Tim Horton s, 5800 W. Alexis, Sylvania, inspected June 8. Observed gloves and other items in hand sinks. Hand washing facility shall be maintained so that it is accessible at all times for employee use. Hand sinks may not be used for any other purposes other than hand washing. Repair hot water knob and drain line or rear hand sink by three-compartment sink. Repair leak in drain line under dish machine dump sink. Observed build-up on vents above prep area and storage shelves. Clean on a regular basis. Observed jacket on storage shelving in prep area. Store personal belongings in designated area away from food items. Inspector: Brandon Tester.

Frisch s Big Boy, 1405 South Reynolds, Maumee, inspected June 9. Buffet cold holding at 48 degrees. Sliced tomatoes on ice at cook line holding at 73 degrees. Reach-in cooler under grill cold holding between 43-45 degrees. All TCS foods, held cold, must maintain proper internal temperature. Contact repair service for coolers. All products held on ice for temperature control must be completely surrounded by ice to the level of the food intended to be kept cold. Inspector: Bauman.

Magic Wok, 6829 Airport, Holland, inspected June 9. Food in top of prep top cooler reading 46 degrees. Bottom half of cooler is reading 35 degrees. Only potentially hazardous foods in top are chicken, beef, shrimp, and eggs. These items were moved to bottom of cooler. Inspector: Nick Kusina.

OB s Bar & Grill, 2243 Tremainsville, inspected June 9. Observed all doors open at time of inspection. All doors must be closed and tight sealing to prevent the entry of pests. A tight-fitting screen door may be used. No sanitizer bucket or sinks set up with wiping cloths sitting out. A sanitizer bucket must be set up with the proper solution at 50-100 ppm bleach or 200 ppm quat. All wipe cloths are to remain in sanitizing solution when not in use. Grill line is extremely dirty. Clean and sanitize grill line and pizza oven to prevent bacteria growth and a potential pest problem. Inspector: Stacy Seger.

Toledo Country Inn & Suites, 541 West Dussel, Maumee, inspected June 9. Cooler in kitchen holding food at 47 degrees. All TCS foods must be held cold at proper temperature.

Local 14, 5411 Jackman, inspected June 10. Observed chicken being prepped in three-compartment sink. Chicken can only be prepped in a prep sink with a two-inch air gap. If a prep sink is not available, the sanitizer side of the three-bay can be used if an air gap is installed. Inspector: Seger.

McDonald s, 567 East Manhattan, inspected June 10. Observed whisk for iced tea stored in a container of sanitizer under the hand sink. In-use utensils must be stored in a clean and dry area or in a clean and dry container. Do not keep utensils in sanitizer. Sanitizer is only effective on clean, rinsed items and only safe when allowed to air dry. Corrected. Observed cut tomatoes at room temperature without any documentation of time. Facility is approved to hold cut tomatoes with time in lieu of temperature procedures only if time is documented and product is discarded after time allotted. Observed build-up on fountain pop machine at drive-up window. Clean and sanitize these areas on a routine basis to prevent build up. Observed build-up on inside of ice dispenser at customer walk-up counter. Clean and sanitize ice machines according to manufacturer s specifications. Inspector: Kerry Stanley.

McDonald s, 3138 Secor, inspected June 10. Observed wet towels stored out of the sanitizer wipe cloth bucket. Store all wet towels not in use in the sanitizer bucket to prevent the growth of bacteria. Inspector: Gillian Wilke.

Tarboosh Deli, 1385 Conant, Maumee, inspected June 10. Falafel cooling out at room temperature at 70 degrees. Hot food cooling must reach 70 degrees within two hours and 41 degrees within an additional four hours (six hours total). Foods passively cooling at room temperature must be aggressively cooled once they fall below 135 degrees. Falafel was voluntarily discarded. Foods packaged on site and offered for customer self service retail not labeled. All of these packages must be labeled, in English, with the common name of the food product, the ingredients, weight, name, and address of facility, and contain an allergen statement. Inspector: Bauman.

Applebee s Neighborhood Grill & Bar, 266 East Alexis, inspected June 11. Observed raw beef steaks above cooked bacon crumbles in the beer walk-in cooler. To prevent cross contamination that can cause food borne illness, store all raw meats and shell eggs below all other food items. Then continue to store raw meats and shell eggs according to required cooking temperatures. Corrected. Observed in-use utensils stored in standing water-dipper wells shut off at inspection. In-use utensils must be stored in a clean, dry area or container. If using water-dipper, wells must be maintained in the on position to properly allow food particles to drain off utensils without accumulation. Observed TCS (potentially hazardous) foods in broil prep top cooler at 43-45 degrees. All refrigeration units are required to hold product at 41 degrees or less. Repair or adjust unit to hold product at proper temperature. Observed TCS foods in combo freezer/cooler unit at 50-51 degrees. Do not use this unit to hold TCS foods until repaired to hold product at proper temperature. Observed bar four-compartment sink set up with wash, rinse, sanitize, and rinse with quat at 400 ppm. Product directions indicate proper level to be at 200 ppm. Use test kit to verify. Ware washing must be wash, rinse, and sanitize with the item allowed to air dry. Facility must eliminate to double rinse process. Instructions on the bottle do not indicate that a double rinse should be used. Observed chemical dish machine dispensing 10 ppm chlorine. To properly sanitize with chlorine, concentration must be 50 to 100 ppm. Corrected. Inspector: Stanley.

Cafe Marie, 3344 Secor, inspected June 11. Observed a blocked hand sink. All hand sinks must be free and easily accessible. Do not store dishware on the hand sink. Corrected. Raw chicken stored above raw sausage in the walk-in cooler and raw shell eggs stored above raw ground beef in the under counter cooler. Follow proper food order. The quat sanitizer in the wipe cloth bucket was above 400 ppm. Quat must be at 200 ppm. Contact your chemical supplier to adjust the concentration of the quat sanitizer that comes out of the dispenser. Liquid eggs sitting out at room temperature at 59 degrees. Must be kept in cold at proper temperature to prevent bacteria growth. To control the temperature during food preparation, reduce the amount of food in prep and chill the food using ice bath. Cooler is holding TCS food above 41 degrees. Contact a licensed repair service to fix cooler. Make sure to not double pan food items. Prep top cooler is holding food on the top part from 31-63 degrees. The cooler is in need of repair to hold all food at proper temperature. Inspector: Wilke.

New Chet s Hot Dog, 1024 East Manhattan, inspected June 11. Observed raw ground beef stored at room temperature on shelf above a box of chips. Follow food storage order and cold holding temperature. Corrected. Various TCS foods stored at room temperature ranging from 55-68 degrees (cracked eggs, cooked potatoes, ground beef). To limit bacteria growth, all TSC foods must be held cold at proper temperature. Product discarded at inspection. Do not leave TCS foods at room temperature. Keep in the cooler or in proper ice bath. Observed high temperature dish machine rinsing at 158 degrees. High temperature dish machine must rinse at 180 degrees to have the surface temperature of items reach a minimum of 160 degrees. Service called at inspection. Facility must manually ware wash in the three-compartment sink until repaired and verified by this department. Three-compartment sink verified at 100 ppm chlorine. Dish machine repaired at inspection rinsing at approximately 180 degrees, surface temperature reaching a minimum of 160 degrees. Approved for use. Inspector: Stanley.

Quality Inn, 1401 East Mall, Holland, inspected June 11. Various food items on buffet line being held between 52-61 degrees. All TCS foods must hold cold at 41 degrees or less. Store buffet products directly in ice to maintain proper temperature. Corrected. Inspector: Bauman.

Toledo Fairfield Inn, 521 West Dussel, Maumee, inspected June 11. Hard boiled eggs being held at 63 degrees. Must be maintained cold at proper temperature. Eggs voluntarily discarded. Discontinue use of dome enclosure for cold holding of hard boiled eggs. This container, by design, is not capable of maintaining food at 41 degrees. Food held cold by ice method must be completely submerged in ice. Inspector: Bauman.

Parkway Cafe The Toledo Hospital, 2142 North Cove, inspected June 12. Milk in reach-in cooler at 48-50 degrees. TCS food must be held cold at 41 degrees or less. All product was removed from service. Cooler was too full; do not block fan with product. Cooler will be serviced. Inspector: Debbie Dacquisto.

The Original Gino s, 449 West Dussel, Maumee, inspected June 12. Cold cuts stored above fill line in prep top cooler at 53 degrees. Diced tomatoes in frozen storage container at 47 degrees. All TCS foods, held cold, must maintain proper internal temperature. Do not store food items above the level of the container in prep top cooler. Obtain a second frozen insulated food container and rotate throughout the day. Thoroughly wash and sanitize pop nozzles on a nightly basis. Mold growth observed at time of inspection. Sanitizer not registering in sanitizing buckets. Rotate sanitizer buckets more frequently to maintain the 200 ppm required to effectively sanitize food surfaces. Repair broken cold water handle on kitchen hand sink. To facilitate good hand washing practices, cold water must be available for mixing with hot water. Water is currently too hot to wash hands properly. Inspector: Bauman.

The Skillet Cafe, 5042 Lewis, inspected June 12. Observed raw bacon (pork and turkey) stored on shelf above tomatoes, lemons, and ice tea in the one-door air cooler. Follow proper food storage order. Observed cooling of potatoes and chicken completely covered in the two-door cooler. Cooling must be conducted rapidly under supervision. Follow proper cooling process. Inspector: Stanley.

Tim Horton s, 161 West Dussel, Maumee, inspected June 12. Walk-in cooler holding food at 43 degrees. Contact repair service and adjust cooler accordingly to hold food cold at proper temperature. Inspector: Bauman.

Maumee Little League Baseball, 1313 Eastfield, Maumee, inspected June 13. Popcorn scoop and ice scoop stored with handles in product. Store scoops in separate designated container or with handles up out of product to prevent potential contamination. Inspector: Bauman.

Speedway, 502 Conant, Maumee, inspected June 8. Walk-in cooler holding at 47 degrees. Contact repair service and adjust cooler as necessary. Inspector: Bauman.

Mr. Freeze, 2031 North McCord, inspected June 11. Wiping cloths stored on counter. Keep all wiping cloths in sanitizer when not in use. Inspector: Kusina.

Tom s Carry Out, 2861 Lagrange, inspected June 11. Observed raw bacon and shell eggs stored above bottled beverages in the one-door beverage cooler. To prevent cross contamination, store in proper order. Inspector: Stanley.

Snappy Mart, 208 Clayton, inspected June 12. Observed chemicals stored above soda. Store all chemicals below and away from food to prevent cross contamination. Inspector: Wilke.

Jing Chuan Restaurant, 4424 Secor, inspected June 11. Observed chicken being stored over pork in walk-in cooler and in freezer. Chicken must be stored under pork products. Follow proper food order. Several containers in dry storage are not properly labeled. All products must be labeled with current product. Food stored on floor and floor of freezer. All food and food contact items must be stored a minimum of six inches above the ground. Inspector: Seger.
