Recently released inspection reports of Lucas County food-service operations.

Violations:

Empire Chen Restaurant, 2502 Navarre, Oregon, inspected Sept. 20. Improper food storage order in walk-in cooler and prep-top cooler. Practice proper food storage order to prevent cross contamination. Milk at improper temperatures of 42 to 44 degrees in the bar cooler. Food items must be held at 41 degrees or below to prevent bacteria growth. Lower cooler’s temperature or make repairs. Improper thawing of beef in room-temperature water. Follow proper thawing procedures. Inspector: Brandon Tester.

China King, 1308 Michigan, Waterville, inspected Sept. 18. Cros contamination present in upright freezer. Raw chicken stored over ready-to-eat foods in the unit. Rearrange freezer to prevent contamination. Inspector: Sara Becker.

Bambino’s Pizza, 1515 Eleanor, inspected Sept. 19. No sanitizer bucket set up. A sanitizer bucket must be set up during business hours. Broken handle on hand-sink faucet. Plumbing must be in good repair. Spray bottles with no labels. Properly label containers for cleaners and sanitizer. Inspector: Stacy Seger.

Sundown Cantina, 5680 Mayberry, Sylvania, inspected Sept. 19. Time/temperature control for safety foods above 41 degrees in the large prep-top cooler. Food must be held at 41 degrees or lower to prevent bacteria growth. Discard the pan of chicken that had a temperature of 46 degrees. Remove the home-type smoker from the establishment. The facility does not have space for a commercial smoker. Major menu changes need to be approved by this department. Inspector: Jennifer Gottschalk.

Stranahan Elementary School, 3840 Holland-Sylvania, inspected Sept. 18. Cheese sitting out at room temperature on serving line. Facility does not have cold-holding space on the serving line. Time/temperature control for safety foods must be held at 41 degrees or lower. Inspector: Gottschalk.

City Diner LLC, 2524 Navarre, Oregon, inspected Sept. 19. Raw sausage above steak in prep-top cooler. Practice proper food storage order to prevent possible cross contamination. Properly label chemical spray bottles. Thoroughly clean equipment regularly to prevent buildup. Inspector: Tester.

Olive Garden Italian, 5120 Monroe, inspected Sept. 11. The sun-dried tomato sauce and cheese sauce are both holding at 47 degrees on ice. Food must be held cold at 41 degrees or lower. Wet wiping towel out on a work surface. Store wet wiping towels in sanitizer buckets. Inspector: Kelly Sattler.

O.K. Wok, 3165 Navarre, Oregon, inspected Sept. 18. Rice cooling at room temperature. Follow proper cooling methods. Inspector: Tester.

Bambino’s Pizza, 2208 Airport, inspected Sept. 10. No paper towels at both hand sinks. Properly stock hand sink. Chlorine sanitizer bucket at 0 ppm. Maintain proper level of sanitizer. Inspector: Kurt Susdorf.

Sunset House - Main Kitchen, 4020 Indian, inspected Sept. 11. Many food items are past their seven-day expiration date. Food was discarded. Repeat violation. Inspector: Susdorf.

Middle East Market, 1205 Flaire, inspected Sept. 12. Meat, spinach, and cheese pies at 50 degrees in display case. Foods need to be held at 41 degrees or lower to prevent bacteria growth. No labels on the prepackaged seeds, nuts, etc. Prepackaged items for sale in the retail area must be properly labeled. Inspector: Susdorf.

Dorr Street Cafe, 5243 Dorr, inspected Sept. 12. Working spray bottles in use lack proper labeling for content. Inspector: Susdorf.

Treo, 5703 Main, Sylvania, inspected Sept. 18. Fish that is vacuum packed thawed in the prep-top cooler. Follow proper thawing method. Inspector: Gottschalk.

Gas Express, 1410 Starr, inspected Sept. 14. Spray bottle without label. Label chemical spray bottles with contents’ common names. Inspector: Tester.

University of Toledo-Phoenicia Student Union Building, 2801 W. Bancroft, inspected Sept. 14. The prep-top cooler is holding food above 41 degrees. Cold hold at 41 degrees or lower. Adjust cooler. Inspector: Gillian Wilke.

College Boys Entertainment Group, 2633 W. Bancroft, inspected Sept. 14. Cigarettes butts on the floor near the bar and other debris on the floor by the bar and in walk-in cooler. Thoroughly clean the floor of all debris and at no time may anyone smoke inside the facility. Inspector: Wilke.

Ideal Hot Dog, 1135 W. Alexis, inspected Sept. 14. Open can of sausage gravy in reach-in cooler. Once opened, products must be moved to a NSF-approved food-grade container to prevent contamination. Wet rags sitting on counter. Wet rags must be kept inside a sanitizer bucket. Inspector: Seger.

Vito’s, 4235 Douglas, inspected Sept. 7. Prep-top cooler holding food above 41 degrees rather than at 41 or lower. Adjust or repair cooler. Inspector: Wilke.

Twin Oaks Recreation, 2816 W. Sylvania, inspected Sept. 7. Home-type refrigerator is holding food above 41 degrees rather than at 41 degrees or lower. Adjust temperature. Inspector: Wilke.

Stop N Shop, 4128 Monroe, inspected Sept. 6. Facility does not have chlorine bleach or quaternary ammonia to properly sanitize dishware and food-contact surfaces. Provide sanitizers. Flies around facility and inside the doughnut case. Practice good sanitation. If necessary, contact a licensed pest-control operator. The hand sink drains very slowly and gray water leaks out of the grease trap. A foul odor is coming from the grease trap. Clean grease trap and eliminate the leak. Inspector: Wilke.

Old Westend Daycare Center, Inc., II, 539 Broadway, inspected Sept. 12. No seven-day discard date on open lunch meat. Properly label such food. Corrected. Inspector: Susdorf.

Owens Corning, 1 Owens Corning, inspected Sept. 12. No ingredient list for bagels available in cafeteria. Provide an ingredient book for all bagels provided by Barry Bagels. Inspector: Susdorf.

Sunset House-Sunset Court, 4020 Indian, inspected Sept. 11. Chemical dish machine final rinse at 0 ppm. Facility was using detergent instead of chlorine for final rinse sanitizer. Monitor where chemicals are placed in dish machine more closely. Corrected. Inspector: Susdorf.

Sunset House-Healthcare, 4020 Indian, inspected Sept. 11. The high-temperature dish machine is not reaching proper temperature of 180 degrees at the final rinse. Dish machine cannot be used until dish machine reaches final rinse of 180 degrees. Repeat violation. Inspector: Susdorf.

Rally’s, 1647 Laskey, inspected Sept. 17. Food stored directly on floor of freezer. Food must be stored a minimum of 6 inches above the floor. Inspector: Seger.

Jug’s Bowling Center, 5111 Jackman, inspected Sept. 17. Hot dogs and lunch meat lack required seven-day discard date. Inspector: Seger.

Positive Choice Activity & Learning Center, 3533 Monroe, inspected Sept. 12. Open package of smoked sausage lacks proper date mark. Corrected. Inspector: Nathan Fries.

Rite Aid, 3013 Monroe, inspected Sept. 12. Can of out-of-date baby formula. Baby foods past expiration date must be removed from sale. Inspector: Sattler.

Storyville Grille, 5201 Monroe, inspected Sept. 11. Food in glass-door cooler holding above 41 degrees. Cooler is not approved for use for working food items and needs to be replaced with a commercial grade, NSF-approved cooler. Discard out-of-temperature food and stop using cooler until it holds correct temperature. Inspector: Sattler.

Children’s Discovery Center, 2303 Cheyenne, inspected Sept. 7. Both chlorine and quaternary ammonia test strips needed. Inspector: Mike Brady.

T.G.I.Friday’s, 1334 Bernath, inspected Sept. 12. Soup well out of temperature at 100 degrees; must be at least 135 degrees. Food discarded. Move pop holster away from portable ice bin. Inspector: Brady.

Scrubs Pub, 3312 Glendale, inspected Sept. 13. Dishwasher not dispensing chlorine. Chlorine and quat test strips needed. Metal stem thermometer needed. Inspector: Brady.

Balance Pan Asian Grille, 514 The Boulevard, Maumee, inspected Sept. 17. Food being held cold on ice at 67 degrees. Follow proper icing method. Food handling and preparation activities being conducted without sanitizer buckets set up. Inspector: Paul Bauman.

Little Caesars, 731 Conant, Maumee, inspected Sept. 17. Quaternary ammonia sanitizer at 100 ppm. Use proper sanitizer concentration. Contact chemical dispensing company to adjust machine. Inspector: Bauman.

The Cookie Lady, 1647 Tollgate, Sept. 18. Employees handling exposed dough/cookies without adequate hair restraints. Acceptable hair restraints include hair nets or hats. Self-service packaged cookies require labeling: weight, ingredients, address, name. Inspector: Bauman.

TW Recreational Services Pro Shop, 1750 Park, Oregon, inspected Sept. 19. Ready-to-eat food items lack proper seven-day discard date and there’s an expired date of 9/9 on cheeseburger. Inspector: Tester.

Calvino’s, 3142 W. Central, inspected Sept. 19. Reach-in cooler holding some food above 41 degrees rather than at 41 or lower. Suggest reducing stock to allow better air flow. Inspector: Wilke.

Woodrow Child Care Center, 5900 Woodrow, Sylvania, inspected Sept. 18. No date labels on the left-over pizza in the freezer. Properly label such food. Inspector: Gottschalk.

J & G Pizza Palace, 5692 N. Main, Sylvania, inspected Sept. 18. Sliced cheese at 49 degrees in the prep-top cooler. Each food item must have its own container and must have its own spot to remain at or below 41 degrees. Four stacks of cheese discarded. Inspector: Gottschalk.

Franciscan Care Center Sylvania, 4111 N. Holland-Sylvania, inspected Sept. 19. One container of open chocolate sauce out at room temperature. The label on the bottle states it must be refrigerated after opening. Chocolate sauce discarded. Inspector: Gottschalk.

Bill Lake Memorial Hall, 1224 W. Sylvania, inspected Sept. 19. Hot dogs, sausage dogs, cheese, and chili sauce lack seven-day discard date. Inspector: Seger.

Bangkok Kitchen, 582 Dussel, Maumee, inspected Sept. 19. Dish machine not dispensing chlorine. To effectively sanitize, this machine must dispense chlorine at 50 to 100 ppm. Train staff to check machine prior to use. Bucket changed at inspection, machine violation corrected. Inspector: Bauman.
