Four northwest Ohio firms are facing fines from the U.S. Occupational Health and Safety Administration for reported safety violations.
Lingvai Excavating LLC of Bryan was fined $55,000 for reported willful safety violations, including failure to protect workers from falls or cave-in hazards. The citations were related to a May inspection at a Napoleon work site, according to OSHA.
First Choice Packaging Solutions of Fremont received a $30,800 fine for several reported serious violations, including improper lockout/tag out energy-control procedures and unsafe operation of industrial trucks. The fines resulted from an OSHA inspection in March.
Brad Germann’s R&R of Napoleon was fined $14,400 for various serious violations related to protecting employees from falls. OSHA issued the citations based on a May inspection of a work site in Maumee.
Forster Trucking & Excavating LLC in Continental was fined $12,500 for a repeat violation involving potential cave-in hazards, as well as a serious violation regarding defective rigging equipment. The citations stemmed from an Ottawa work site inspection in May.
Each company can contest the citation and the fines.
