● There was buildup inside the ice machine.

● Food employee(s) did not wash hands in situations that specifically require them to do so.

● Food employee(s) were not washing hands in an approved hand washing sink or automatic hand washing facility.

● Gnats were in the facility.

Bee Creative Child Care, 5252 Heatherdowns, inspected July 23.  

● Food that had been date marked was not discarded when required.

● Food items were not being held at the proper temperature – instead sitting out at room temperature that were holding in the “temperature danger zone.”

● Food items in a reach-in cooler were not being held at the proper temperature.

● Hand-washing sink was being used for purposes other than hand washing.

Our Time Carry Out, 2100 Dorr, inspected July 23.  

● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

● The store was selling food service cups of ice when it is not licensed to do this.

● Raw bacon was stored above cooked meats/​sausage.

● A ceiling condenser in a walk-in cooler has shields, but some moisture droplets will hit the beverages stored immediately below.

● There was not a compartment sink available to clean, wash, rinse, and sanitize tongs used for Amish beef sticks.

Agenda Sports Bar & Grill, 159 Matzinger, inspected July 23.  

● Spaghetti from the previous day was not being held at the proper temperature.

● Boiled eggs were not date marked to indicate when the food should be discarded.

● There was buildup of food debris on the blades of the potato slicer.

● All food items in the under-counter cooler were not being held at the proper temperature.

● Food employee(s) were coughing and sneezing in their hands and serving customers at the bar.

● There was buildup of food debris inside the pizza oven.

● There was buildup of black debris inside an ice machine.

● There were no signed employee(s) health forms at the establishment.

● Pepperoni, cheese, sauce, cut lettuce, and cooked ground beef were not being held at the proper temperature.

Heartland Holly Glen, 4293 Monroe, inspected July 24.  

● There was buildup of dirt and debris inside the ice machine.

● Cooked greens were not being held at the proper temperature.

● Sliced ham and opened boiled eggs in a bag did not have date markings to indicate when they should be discarded.

● Expired proccoli was not discarded.

● Sanitizer solution in a spray bottle used for cleaning the food counter did not meet minimum requirements.

● The hand-washing sink was not easily accessible.

Al Smith’s Place, 3550 Executive, inspected July 24.  

● Cooked green beans were not cooled using the proper time and temperature parameters.

● Whipped butter and southwest dressing was kept past its discard date.

● There was buildup on the shield of the ice machine.

● There was buildup on soda nozzles on the self-serve beverage machine.

Sori Sushi, 5236 Monroe, inspected July 25. 

● Cream cheese and rolls inside a cooler did not have date markings to indicate when they should be discarded.

● The hand-washing sink was not easily accessible.

Six-Pack Bar & Grill, 3223 W. Sylvania, inspected July 25.  

● Food employee(s) did not wash hands before putting on gloves after returning from a walk-in cooler and also touching raw beef.

● Raw shell eggs werew stored above cooked beef and boiled eggs inside reach-in coolers.

● There was no sanitizer solution in a bucket or spray bottle.

Signal Nite Club, 3922 Secor, inspected July 25.  

● There was buildup of dirt and debris inside soda nozzles.

Shell Ottawa Hills #510, 4310 W. Central, inspected July 25.  

● Raw shell eggs were stored above ready-to-eat food items in the display cooler.

● There was buildup on soda nozzles.

Rosie’s Fine Foods, 606 N. McCord, inspected July 25.  

● An employee plating food, cutting bread, making drinks, making salads, and touching clean utensils with a soiled bandaged finger.

● Raw beef was stored over ready-to-eat food in a walk-in cooler.

● An employee touched cut lemons for drink by smashing lemons down into a container with their bare palm.

● An employee touched bread with their bare hands while cutting.

● Deli meats were not properly date marked to indicate when the food should be discarded.

● The inside of the ice machine, a can opener, and food containers were dirty.

● There was buildup of food debris inside the meat slicer machine.

● There was no sanitizer solution in a bucket or spray bottle.

● Cut melon in the produce display was not being held at the proper temperature.

● The deli counter case wasn’t holding food items at the proper temperature.
