Miss Lucille's Southwyck, 5255 Heatherdowns, inspected Sept. 10. Critical violation: Repair three-door prep top cooler. Cease use until it can maintain a temperature of 41 degrees or less. Inspector: Kevin Halligan.

Leo's Coney Island, 3115 Airport, inspected Sept. 3. Critical violation: Thermometers needed in two reach-in coolers. Inspector: Mike Brady.

McDonald's 1405 South Byrne, inspected Sept. 3. Critical violation: Calibrate quaternary ammonia sanitizer dispenser at three-compartment sink; it is 400 ppm and should be at 200 ppm.

Bergers Olde Tyme, 1742 West Laskey, inspected Sept. 7. Critical violations: Perishable foods must be maintained at 45 degrees or less. Cooler was turned off while being relocated and turned back on, all perishable food discarded. Do not use unit until 45 degrees can be maintained. Provide sanitizer bucket for wet cleaning. Clean and sanitize meat slicer. Inspector: Brady.

Timbo's Cones, 5437 West Alexis, inspected Sept. 7. Critical violation: Nacho cheese at 128 degrees, removed from service. Machine must be repaired before using. Inspector: Lahey.

Lutheran Home of Toledo, kitchen, 131 North Wheeling, Oregon, inspected Sept. 8. Critical violation: Observed employee washing dishes without sanitizer. Quat dispenser was empty at time of inspection. Inspector: Kistler.

Toledo Academy of Learning, 301 Collingwood, inspected Sept. 10. Critical violation: All potentially hazardous food must be held at 140 degrees or above to prevent contamination. Mayonnaise at 45 degrees, pizza at 100 degrees. Inspector: Shaw.

Walgreen's, 4580 Monroe, inspected Sept. 2. Critical violation: Adjust water flow to drinking fountain to avoid contact with orifice. Inspector: Bruce Heiser.

Sunoco Food Mart, 2515 West Laskey, inspected Sept. 2. Critical violations: Soft-drink nozzles are to be cleaned and sanitized to eliminate mold. Hand sink was blocked at time of inspection. Hand sink must be accessible at all times. Inspector: Heiser.

Sterling Food Store, 5502 Monroe, inspected Sept. 9. Critical violation: All chemicals must be stored below food and food related items. Inspector: Lahey.

Hooterville Station, 119 North Erie, inspected Sept. 8. Critical violation: Milk at 68 degrees and the cooler temperature at 67 degrees. Potentially hazardous food must be at 41 degrees or below to prevent the growth of bacteria. All potentially hazardous food out of temperature was discarded. Inspector: Shaw.

St. Charles Mercy Hospital, cafeteria, 2600 Navarre, Oregon, inspected Sept. 10. Critical violations: Custard on cold food line at 95 degrees. All foods for cold serve must be brought out at 45 degrees or below. Do not use the service line for cooling foods. Custard removed from service. Yogurt mix in machine was 49-52 degrees. Discontinue use of machine until it can maintain 45 degrees or below. Inspector: Kistler.
