Recently released inspection reports of Lucas County food-service operations:

BPO Elks Toledo Lodge, 3520 Holland-Sylvania, inspected June 29. Critical violations: Observed handle of ice scoop touching ice at the bar. Keep ice scoop handle from direct contact with ice to prevent contamination of ice. Chicken salad at 50 degrees, bologna at 47 degrees, and cheese slices at 50 degrees. All potentially hazardous food (PHF) must be held cold at 41 degrees or below or hot at 135 degrees or above. Four noncritical violations. Inspector: Cynthia Garcia.

Glendale Gardens, 2521 Glendale, inspected June 25. Critical violations: Observed that dish machine not dispensing sanitizer properly. Repair called. Observed employee using same gloves for handling raw chicken and raw cucumbers. Reviewed cross-contamination rules. One noncritical violation. Inspector: Mike Brady.

Applebee's Neighborhood Grill and Bar, 2825 Glendale, inspected June 28. Critical violations: Observed standing water in prep top cooler along cook line. Eliminate immediately. Observed soft drink holsters without drain lines above clean glasses in bar area. Install drain line and re-wash glasses. Thermometer needed in refrigerated drawers on cook line. Inspector: Brady.

China One Buffet, 4751 Monroe, inspected June 30. Fish fillets at 51 degrees. Hold PHF cold at 41 degrees or less. All other cold foods on buffet holding properly between 38-39 degrees. Two noncritical violations. Inspector: Dacquisto.

Fu Yi Chinese Restaurant, 7130 Airport, Springfield Township, inspected July 5. Critical violations: Observed employee washing and rinsing a rice cooler inset at three-compartment prep sink. Post sign above sink stating "For Food Only." All dishes must be washed, rinsed, and sanitized. All dishes must be run through dish machine. Inset placed in dish machine. Observed open drink cups stored on cooking and prep line. All employee drinks must have lids and straws and be stored below food and food contact surfaces. Food must be date marked. Nine noncritical violations. Inspector: Jaclyn Kistler.

City Q Barbeque, 7402 West Central, inspected July 7. Critical violations: Observed deli case holding at improper temperature. Do not use until unit holds at 41 degrees or below. Foods at improper temperatures voluntarily discarded. Dishmachine not properly sanitizing. Do not use dish machine until repaired. Five noncritical violations. Inspector: Kistler.

Elephant Bar Restaurant, 5001 Monroe, inspected July 7. Critical violation: Observed raw meat stored above ready-to-eat (RTE) foods. Raw eggs stored above RTE foods. To prevent cross contamination, store raw meats and eggs below RTE foods in proper storage order. Two noncritical violations. Inspector: Kelly Sattler.

Downtown Variety, 601 Adams, inspected July 3. Critical violation: Place eggs on bottom shelf to prevent contamination if they should crack or break and leak onto product below. Inspector: Ivans.

D&C Carryout, 525 Dorr, inspected June 29. Critical violations: Entire deli display case was not functioning properly. Cooler was holding food at 50-56 degrees. All PHF must be held at 41 degrees or below. All voluntarily pulled from sale to get returned for store credits. Papers will be supplied in two weeks stating returned product. Do not use cooler until rechecked. Cheese in prep-top was at 46 and 47 degrees. It was placed in tray and stacked on top of deli meat. Cheese should be in pan that is placed directly into unit to maintain 41 degrees or below. Food in bottom of prep top cooler was not datemarked. Repeat violation. At next inspection, any food not date marked will be discarded. Water at handsink is shut off because of leak. Repair leak to encourage frequent handwashing. Using three-comparment sink for handwashing is not allowed. Facility is using gyro improperly. At the end of the day the bulk gyro meat must be shaven down completely and placed in cooler. Meat should reach 41 degrees within six hours. Use microwave to reheat to 165 or purchase pre-cooked pre-sliced gyro meat. Nine noncritical violations. Inspector: Ivans.

Bambino's Pizza, 4466 Monroe, inspected July 3. Critical violations: Date mark all RTE, PHF not used within 24 hours must be datemarked. Spring spray head on three-compartment sink rests at improper level in sink. Repair or replace spray head to provide for proper air gap. Seven noncritical violations. Inspector: Heiser.

Speedway, 1500 South Reynolds, Maumee, inspected July 5. Critical violation: Observed chemicals stored next to and above food. Three spray bottles were hung on racks where open bags of cappuccino mix are stored. Bottles moved. Also, observed box of starting fluid stored on a rack directly above snack mix and other food. All chemicals must be stored away and below food in a designated area to protect food from chemical contamination. One noncritical violation. Inspector: Wilke.

Seaway Market Place, 1707 Cherry, inspected July 5. Critical violations: Observed improper storage of raw animal products in meat cases. All raw animal products must be stored below all other foods items and according to cooking temperatures in both coolers and freezers to prevent cross contamination. Repeat violation. Observed stacking of ground beef tubes without separation. Food items must be protected from contamination. When stacking tubes use separation. Example: plastic wrap or any food grade covering. Observed produce sprayer with excessive residue buildup. Nozzles must be removed on regular basis and be properly washed, rinsed, and sanitized. Repeat violation. Nine noncritical violations. Inspector: Kerry Cutcher.

Kroger, 4633 Suder, inspected July 5. Critical violation: Observed improper storage of raw animal products in meat cases. Example: raw pork over saurkraut, sausage above bacon, and bacon above precooked hot dogs. All raw animal products must be stored below all other food items and according to cooking temperatures in both coolers and freezers to prevent cross contamination. Two noncritical violations. Inspector: Cutcher.

Major Magic's All Star Pizza Review, 5838 Monroe, Sylvania, inspected July 10. Critical violations: Observed employee putting on gloves without washing hands first. Food employees must wash their hands before putting on single use gloves. Employee was preparing pizza without hair and beard restraints. A second employee was preparing cheesy bread also without hair restraints. Employees must wear hair restraints such as hats, hair covering or nets, beard restraints designed to be worn to effectively keep hair from contacting exposed food, clean equipment, utensils, lines, or unwrapped single service or single use articles. Observed two containers of food dated June 29 in walk-in cooler. RTE foods held longer that 24 hours must be clearly marked with a proper "use by" date. Pizza sauce at 80 degrees in warmer. The correct holding temperature is 135 degrees or more. This facility is not licensed to cool and reheat leftovers. Observed bag of onions on floor in back storeroom. All food items are to be stored at least six inches above the floor. Eleven noncritical violations. Inspector: Garcia.

McDonald's, 567 East Manhattan, inspected July 11. Critical violations: Uncovered fries were in fryer basket in walk-in freezer. Food must be protected from contamination. Store only in containers that cover food completely and cover with lid or food grade covering. Fryer basket has open sides not suitable for storage. Seven noncritical violations. Inspector: Cutcher.

Mr. Philly, 525 East Manhattan, inspected July 11. Critical violations: Observed garlic butter on counter top at 81 degrees with label stating "keep refrigerated." PHF must be maintained cold at 45 degrees or less or hot at 135 degrees or more to limit bacteria growth. Repeat violation. Product discarded. Observed PHF items in prep top at 55-56 degrees. All refrigeration must maintain food at 45 degrees ore below. Product discarded. Do not use upper half of prep top cooler to store PHF until repaired. Four noncritical violations. Gladieux Meadows, 4480 Heatherdowns, inspected July 12. Critical violation: Roof leaking in food closet during inspection. Food in area removed from service due to water damage. Repair immediately. Inspector: Brady.

Fallen Timbers Golf Course, 7711 Timbers, Monclova Township, inspected July 10. Critical violations: Raw beef products and chicken being stored improperly. Rearrange food to prevent cross contamination. Hot dogs found in prep top holding at 50 degrees. Prep top has frozen up at time of inspection. Repair or replace unit. Hot dogs discarded.
