6/17/2007This is only for participants in the Blade Stock Market Game, which began Jan. 1, 2007.
For entrants, complete the top part of this form and then list the new stock under Part A, with the company name and ticker symbol, and list the stock to be replaced under Part B, with the company name and ticker symbol. Any of the four stocks in your portfolio may be replaced, but the stock to be replaced and the new one must trade on the same stock exchange, and the new one must trade for at least $5 a share as of June 30, 2007. If there is confusion about what was requested on this form, the original entries of the contestant will remain in place.
