Circle K, 5342 Lewis, inspected March 11. Critical violations: Observed box of raw shell eggs stored above bottled beverages in walk-in cooler. Store all raw meat, and shell eggs below all other food items to prevent cross contamination. Observed wet/soiled cloths stored on counter at beverage station. When not in use, store all wiping cloths in sanitizing solution with proper chemical concentration to limit bacteria growth. Observed open boxes of hot dogs with open date of 3-7-08 and a discard date of 3-14-08. To limit bacteria growth, PHF may only be held for a total of seven days once opened, the day opened counts a day one, discard date should be 3-13-08. Handsink in back storage area is dirty. Providence Pub, 6719 Providence, Whitehouse, inspected March 17. Critical violations: Date mark all PHFs once opened, prepared, or thawed. Unlabeled squirt bottles found in the facility. Label all bottles with common name of the contents to prevent any issues with contamination. Five noncritical violations. Inspector: Sara Becker.

Beaner s Gourmet Coffee, 420 Madison, inspected March 18. Critical violations: Observed skim milk in thermos holding at 65 degrees. To prevent bacteria growth you must hold PHF cold at 41 degrees or below. Monitor milk in thermos and change often. Two noncritical violations. Inspector: Cipiti.

Busia s Narozny, 302 Laskey, inspected March 19. Critical violations: Observed employee coats stored on bag of onions in back storage area. Designate an area for employee belongings away from food and food contact items. Observed fish thawing in standing water in prep sink. Proper thawing to limit bacteria growth by law includes the following methods: under refrigeration, completely submerged under cold running water in prep sink, in microwave if cooking immediately, or cooked from frozen state. Observed various PHF items in single door upright cooler and walk-in cooler at improper temperatures. Refrigeration units must be able to hold PHF at 41 degrees or less. Three noncritical violations. Dairy Depot, 4160 Navarre, inspected March 19, Critical violations: Observed ice cream unit not holding at proper temperature. Vanilla, chocolate ice cream at 42-43 degrees. All PHF must be held cold at 41 degrees or less to prevent bacteria growth that can cause a food borne disease. Three noncritical violations. Inspector: Nye.

Lutheran Home of Toledo-kitchen, 131 Wheeling, inspected March 19. Critical violation: Observed improper cooling of pureed vegetables and meat. To properly cool PHF, food must cool from 135 to 70 degrees in two hours, then from 70 to 41 degrees in four hours. Food items discarded. One noncritical violation. Inspector: Nye.

UT Ottawa House, 2801 West Bancroft, inspected March 24. Critical violations: Observed cooked noodles that were improperly cooled in the walk-in cooler. Noodles were cooked six hours earlier and were at 56 degrees at time of inspection. Another container of noodles that were cooling was stacked on top of container. Do not cover cooling foods or stack items on top of each other. Noodles voluntarily discarded. Observed raw ground beef stored on top of milk. Store all RTE food above raw meat to prevent cross contamination. Pesto sauce in the hot box was at 117 degrees. All PHF must be held hot at 135 degrees or more. Apples, salad croutons, crunch noodles, and all other PHFs must be placed under the sneeze guard to prevent contamination. Thoroughly clean the hood covering the grill in the buffet area. Bleu cheese dressing at salad bar station at 66 degrees, shredded cheese at 43. All PHFs must be held cold at 41 degrees or less. Provide deeper pans to place the food closer to the cold plate. Observed wet wiping towels stored on countertops. Store all wet wiping cloths in sanitizer buckets when not in use. Eggs at salad bar at 64 degrees. Cooler was not turned on at time inspection. Food discarded. The alfredo sauce holding at the pizza buffet was at 132 degrees. All PHF must be held hot at 135 degrees or more. Pasta salad in deli display case was 52-54 degrees, turkey sandwich at 48-49 degrees. Cooler is currently holding at 50 degrees. All PHF must be held cold at 41 degrees. Food discarded. Sandwiches and salads offered for self-service were not properly labeled. All prepackaged foods must be labeled with the common name, list of ingredients, net weight, and name and address of the packager. Four noncritical violations. Inspector: Gillian Wilke.

All food must be held cold at 41 degrees at all times. Pepperoni, chicken, and shredded cheese discarded. Observed wet wiping cloths out on food prep surfaces. Arby s, 2647 Navarre, inspected March 19. Critical violations: Observed roast beef holding at unsafe temperatures. All PHF must be held hot at 135 degrees or higher to prevent bacteria growth that can cause foodborne disease. Observed food being at improper temperatures in walk-in cooler. All PHF held cold must be held at 41 degrees or lower. Two noncritical violations. Inspector: Nye.

McDonald s, 1520 Cherry, inspected March 26. Critical violations: Observed damp/soiled wiping cloths on surfaces throughout the kitchen area. To limit bacteria growth, store damp/soiled cloths in sanitzing solution with chemical concentration of 100 ppm chlorine when not in use. Three-compartment sink being used for food preparation. All food preparation must be done in a designated food prep sink with a two-inch air gap to the plumbing. Observed handsink in employee rest room blocked. Every handsink must be fully accessible and stocked with soap and paper towels at all times. At inspection, restroom was not properly stocked. Observed self-closing faucets in public/employee restrooms running for less than 15 seconds. The flow water must be for at least 15 seconds before need to reactivate. Volunteers of America, 1201 Champlain, inspected March 26. Critical violation: Observed PHF in walk-in cooler at improper temperatures. All refrigeration must hold product at 41 degrees or lower. One noncritical violation. Inspector: Cutcher.

Magic Wok, 5905 West Central, inspected March 26. Critical violations: No sanitizer in wipe cloth bucket. Maintain proper sanitizer concentration in wipe buckets, store all wipe cloths in bucket when not in use. Quat sanitizer is too strong in the three-compartment sink at 400 ppm.
