The Lucas County Coroner has ruled in the following deaths:

Theodore Comprix, 54, of Toledo, Sept. 19, at Flower Hospital. Accidental, complications of blunt force trauma to head; deceased had an unwitnessed fall.

Nicholas Detillion, 27, of Toledo, Aug. 5, at home. Accidental, drug toxicity.

Frank Martin, 92, of Sylvania, Ohio, Sept. 23. Accidental, multiple blunt force trauma; decease was in auto accident.

Melting Pot Restaurant, 5839 Monroe, Sylvania, inspected Oct. 1. Unlabeled chemical spray bottle observed in the bar and server area. All chemical spray bottles must be labeled with the chemical's common name to allow all employees to know contents. Inspector: Jennifer Gottschalk.

Biggby Coffee, 3160 Markway, inspected Oct. 4. The quat sanitizer in the buckets setup is low. Maintain quat sanitizer concentration at 200 ppm to properly sanitize food contact surfaces. The back hand sink was blocked by a spatula at time of inspection. All hand sinks must be free and clear to promote frequent and proper hand washing. Inspector: Gillian Wilke.

Dairy Queen, 6803 Spring Valley, Holland, inspected Oct. 4. Observed food stored in walk-in cooler with no date mark. All food kept longer that 24 hours must have a discard date that is no longer that seven days, to prevent bacteria growth. Inspector: Nicholas Kusina.

DeVeaux Middle School, 2620 West Sylvania, inspected Oct. 4. The pasta salad holding in the cold well was 44 degrees. All food must be held at 41 degrees or less to prevent bacteria growth. The pasta salad was in a tray that was not set inside the cold well. Ensure food is in the well to hold proper temperature. No sanitizer bucket set up at time on inspection. A bucket of sanitizer shall be set up at all hours of operation to thoroughly sanitize food-contact surfaces. Inspector: Wilke.

Cottage Inn Pizza, 240 West Alexis, inspected Oct. 5. Observed bleach in wipe buckets over 200 ppm. A concentration on 50-100 ppm bleach must be used to prevent chemical residue that can lead to chemical poisoning. Use test strips to verify concentration. Observed spray bottles that are not labeled with what appears to be de-greaser. All spray bottles must be labeled with common name of product. Observed employee food and beverages stored in Pepsi merchandiser with product that is for sale. A separate area must be made available to store employee personal items, either away from or below items that are offered for sale. Inspector: Stacy Seger.

Kindercare Learning Center, 6367 River Crossing, inspected Oct. 4. The hot water at the white hand sink had a temperature of 73 degrees. Hot water at 100 degrees must be available at all hand washing sinks for proper hand washing to occur. Repair this sink to have 100 degrees hot water available and contact office after repair. The three-compartment sink hot water reached 103 degrees today. Found time/temperature control for safety foods at 42 degrees and 44 degrees in the two-door cooler. All food must be held at 41 degrees or lower to prevent bacteria growth that could cause food-borne illness. Inspector: Gottschalk.

Steak & Shake, 2141 Reynolds, inspected Oct. 4. Observed standing water in bottom of front line meat cooler. (No items stored in water). Eliminate ASAP. Observed sanitizer for wipe cloth buckets dispensing only 100 ppm quat sanitizer. Must be 200 ppm hand mix until repaired. Front line meat cooler observed at 45 degrees. Must be 41 degrees or less to prevent bacteria growth; will be rechecked. Inspector: Mike Brady.

The Hungry I, 6060 Renaissance, inspected Oct. 4. Observed wet wiping cloth rags at the bar and next to the pop complex in the waitress station. Provide a sanitizer wiping cloth bucket to store all in-use wiping cloths. Wet wiping cloths on the counter can support possible bacteria growth. Found sliced tomatoes at 46 to 48 degrees double stacked in the top section of the prep-top cooler. All items in the top section of the prep-top cooler must be placed in a spot to maintain at proper temperature. Do not double stack food in the prep-top cooler. All time/temperature control for safety foods must be held at 41 degrees of lower at all times to prevent possible bacteria growth. Found chicken at 71 degrees out at room temperature during lunch. All food items must be held at proper temperature hot at 135 degrees or higher or cold at 41 degrees or lower to prevent bacteria growth. Maintain chicken at proper temperature. Do not keep food items out at room temperature. Observed bottle of glass cleaner hanging on the rack with to-go containers. Store all chemicals in one area away from food and food contact items to prevent cross contamination. No consumer advisory printed on lunch menu. A consumer advisory is required on menu if consumer can order meats and seafood under-cooked. An asterisk must be placed next to each menu item that can be ordered under-cooked. The asterisk will refer the consumer to the advisory. The consumer advisory must be printed large enough for the public to read it. Note: The consumer advisory on the dinner menu is very small and the font should be increased. See our Website for handouts on writing consumer advisory. Inspector: Gottschalk.

John's Korner Bar & Grill, 2202 Tedrow, inspected Oct. 5. Bar cooler holding at 46 degrees. All foods, held cold, must maintain an internal temperature of 41 degrees or below. Adjust cooler settings. Inspector: Paul Bauman.

Andrea's Sports Pub, 3338 West Alexis, inspected Oct. 6. Observed food in prep-top cooler holding at unsafe temperatures. All foods must be held at 41 degrees or below to prevent bacteria growth that can cause food-borne illness. Hamburger and tomatoes discarded at time of inspection. Remaining product moved to upright cooler. Until prep-top is repaired, upright must be utilized. Inspector: Seger.

Vito's, 524 Main, inspected Sept. 28. The two sanitizer buckets and the sanitize bay of the three compartment sink were below 200 ppm quat. A quat test kit was available to check concentration of sanitizer. This kit must be used on a daily basis to verify the correct concentration of sanitizer. Contact your chemical company to re-calibrate sanitizer to correct concentration of 200 ppm. The shredded cheese and pepperoni on the far end of the prep-top cooler line were holding at 44 degrees. These items must hold cold at 41 degrees or below to prevent bacteria growth that can lead to food-borne illnesses. Keep lid closed as much at possible. Inspector: Kelly Cipiti.

Magic Wok, 5905 West Central, inspected Oct. 5. The breaded chicken is hot holding at 106 to 110 degrees under the heat lamp at time of inspection. All foods must be held hot at 135 degrees or higher at all times to prevent the growth of harmful bacteria. This pan is not completely under the heat lamp at this time. Provide additional hot holding or heat lamps for all foods that are held hot. There is no sanitizer detected in the sanitizer bucket at time of inspection. Change sanitizer as often as needed to maintain sanitizer at the proper concentration. Chlorine sanitizer must be used at 50-100 ppm and a quat sanitizer at 200 ppm. Inspector: Kelly Sattler.

Ursuline Center, 4035 Indian, inspected Oct. 5. Observed raw bacon stored above mashed potatoes in the walk in cooler at time of inspection. Store raw meats separate from or below all ready-to-eat foods to prevent cross contamination. This was corrected at time of inspection. Inspector: Sattler.

Cafe Marie, 3344 Secor, inspected Oct. 6. Observed wet towels stored in the hand sink at this time. Do not use hand washing sink for storage or any other purpose than hand washing. This sink must be maintained clean and accessible for use at all times. There is no sanitizer bucket set up at the cook line at this time. The sanitizer bucket set up at the server area has no sanitizer left in it at time of inspection. Keep a sanitizer bucket set up in all food prep areas during operation. Change out sanitizer as often as needed to maintain the proper concentration of chemical. Quat sanitizer must be maintained at 200 ppm and chlorine sanitizer at 50 to 100 ppm. Inspector: Sattler.

Playermakers Burgers & Brew, 35 East Alexis, inspected Oct. 7. Observed an uncovered employee beverage storage on a table with equipment near the ice machine. Employee beverages must have a lid or a sealable cap and be stored to prevent contamination to food and clean items, surfaces, equipment, etc. Observed the edible ice in the ice bin behind the bar used to chill beverage garnishes, such as lemons. Food must be protected from contamination. Do not use the edible ice to chill any items. Edible ice must be separate from ice used as a coolant. Observed no sanitizing wiping bucket setup. During all hours of operation, the facility must have sanitizing wiping buckets setup in kitchen and bar to properly sanitize surfaces and to properly store damp/soiled cloths. Observed improper thawing of taco meat and burger patties at room temperature. To limit bacteria growth that can cause food-borne illness, only the following thawing methods are approved: 1. Under refrigeration. 2. Completely submerged under constant running cold water in the prep sink. 3. Cook from a frozen state. 4. Using the microwave only if cooking immediately after thawing. Observed cheese sauce sitting on a table at room temperature at 54 degrees. To limit bacteria growth that can cause food-borne illness, all potentially hazardous foods must be held cold at 41 degrees or lower, or held hot at 135 degrees or higher. Do not leave food items at room temperature. Observed excessive buildup present inside the ice machine. Clean and sanitize according to the manufacturer's specifications. Observed a beer pitcher stored in the hand sink behind the bar. Do not block any hand sink. The hand sink must be accessible at all times for proper hand washing. Do not use the hand sink for storage of any items. Observed no soap and paper towels at several hand sinks in the kitchen. Keep all hand sinks stocked with soap and paper towels at all times ensure proper hand washing. Inspector: Kerry Stanley.

McDonald's, 5210 North Summit, inspected Oct. 8. Observed buildup inside the ice machine. Clean and sanitize according to the manufacturer's specifications. Inspector: Stanley.

Subway, 4870 North Summit, inspected Oct. 8. Observed uncovered employee beverages stored on a back preparation table. Employee beverages must have lids with a straw to protect food and clean items from contamination. Observed a box of cookies stored on the floor of the walk in freezer. All food items must be protected from contamination, store in a clean and dry area at least six inches off the ground. Observed an unlabeled spray bottle on a preparation table. Label all spray bottles with the common name of ingredients to prevent contamination. Inspector: Stanley.

3 Happiness by SU, 845 South McCord, inspected Oct. 13. Observed food stored on floor in walk in freezer, keep all food at least 6 inches off of floor. Observed handles on equipment with build of food, clean regularly. Inspector: Kusina.

Toledo Retirement Residence, 4560 West Alexis, inspected Oct. 13. Found container of ham dated 10/3 in the two-door cooler. All open and prepared time/temperature control for safety foods can only be held for a total of seven days. The day the food is opened or prepared is day one. The food was discarded at the time of inspection. Make sure all open and prepared food is properly dated and rotated. Inspector: Gottschalk.

Maplewood Elementary School, 6769 Maplewood, Sylvania, inspected Oct. 13. Currently, a new Maplewood Elementary School is being built in front of this school. At this time, this department has not received plans for the new school. All plans must be submitted to this department for approval prior to beginning construction. Provide this department with a full set of plans, plan review fee, and plan review packet and menu immediately. Note: Food service licenses do not transfer. A new license will be required for the new Maplewood building. Inspector: Gottschalk.

Subway, 5141 South Main, Sylvania, inspected Oct. 13. Sanitizer bottle tested at 0 ppm for quat sanitizer. Quat sanitizer must test at 200 ppm. If a sanitizer spray bottle will be used, it should be used with disposable towels. Wet towels may not sit out on the counter. Discard the current bottle of sanitizer. The sanitizer is being placed in an old glass cleaner bottle. Never place a different chemical even if labeled in an old chemical spray bottle. A sanitizer wiping cloth bucket may be used to keep in use wiping cloths in. Maintain sanitizer at 200 ppm. Inspector: Gottschalk.

Subway, 2200 McCord, inspected Oct. 13. Observed mold growth on underside of pop dispenser, clean regularly. Observed light not working in walk in freezer, replace/repair. Inspector: Kusina.

Child's Play, 7337 West Bancroft, inspected Oct. 14. Observed foods held at 50 degrees, all food must be held at 41 degrees or less to prevent the growth of bacteria. Food removed. Inspector: Kusina.

Lam-Pam Enterprises Inc., 1016 Conant, Maumee, inspected Oct. 14. Quatemary ammonia dispensing in three compartment sink at 500 ppm. To effectively sanitize without possible chemical contamination of dishes/utensils and surfaces, quatemary ammonia should be used at a concentration of 200 ppm. Contact chemical company to recalibrate the dispensing machine. Inspector: Paul Bauman.

Carmel's Mexican Restaurant, 2947 Tremainsville, inspected Oct. 16. Observed outdated or improperly date marked food on prep line and in draw cooler. Refrigerated, ready to eat time/temperature controlled for safety food prepared in a food service operation or in a retail food establishment and held at a temperature of 41 degrees Fahrenheit (5 degrees celsius) or less for more than 24 hours, shall be clearly marked to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed on the premises, sold, or discarded. The day of preparation shall be counted as day one. All containers discarded at time of inspection. Repeat violation. Observed bulk food containers of spices that are not properly labeled with current product information. Working containers holding food or food ingredients that are removed from their original packages for use in a food service operation or retail food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be identified with the common name of the food except that containers holding food that can be readily and unmistakably recognized such as dry pasta need not be identified. Observed 2 oz. cups in spices and rice, plastic cups in ice, metal shaker in ice, and styrofoam container in rice used as scoops throughout facility. Only approved scoops with handles may be used. Handles must be stored upright to prevent product contamination from hands. Inspector: Seger.

Oakleaf Toledo Ltd., 4220 Holland Sylvania, inspected Oct. 15. Observed 2 trays of raw sausage patties stored above produce and cooked items in the walk in cooler. Store all raw meat below produce and cooked items to prevent cross contamination. Follow good storage order in all coolers. Corrected at the time of inspection. Inspector: Gottschalk.

Sylvania Country Club - Main Kitchen, 5201 Corey, Sylvania. No paper towels at the bakery hand sink. Stock this hand sink with paper towels. All hand sinks must stocked at all time with paper towels, hand soap and hot/cold water (100 degrees) for proper hand washing to occur. Inspector: Gottschalk.
