A Michigan residential builder has been fined $19,600 for one willful safety violation and three serious safety violations found last month at a construction site in Perrysburg.

Inspectors from the Toledo office of the U.S. Occupational Safety and Health Administration said problems found at the job site run by David Houghton Builders LLC of Stanton, Mich., included employees laying sheathing on a steep roof at a height of 26 feet while not protected by any guardrail, net, or personal safety system; scaffolds that were incompetently designed and exposed employees to unsafe conditions; using a forklift to support a scaffold; and no protection to keep employees working on scaffolds from falling.

The inspection took place Jan. 7 at a home construction site at 29181 Tracy Creek Dr. in Perrysburg.
