4 area companies fined for safety violations

Four northwest Ohio businesses have been fined a total of more than $62,000 for safety violations found this year by federal Occupational Safety and Health Administration inspectors.

BP Husky Refining LLC of Oregon was fined $21,000 for three serious violations found between June and August at its refinery at 4001 Cedar Point Rd. in Oregon. The violations included malfunctioning valves that vented hazardous materials.

Steve Petrick Roofing of Sandusky was fined $15,840 for two repeat serious violations found Aug. 22 at a work site at 101 Land- rush St. in Sandusky. The violations were for a lack of a guardrail or safety system for employees working seven feet off the ground and for a lack of employee training about falling hazards.

Bryan Die Cast Products Inc. of Bryan was fined $15,000 for four serious violations and two violations in the "other" category found May 20-31 at its plant at 872 Trevitt St. in Bryan. The violations included failing to protect workers from hazards from an unapproved hoist attachment and not ensuring that employees were competent to operate a powered industrial truck.

K-O-I Warehouse Inc. of 5411 Roan Rd., Sylvania, was fined $11,000 for three serious violations found June 14 at its warehouse. Violations included failure to protect workers from falls of up to eight feet and not ensuring that employees were competent to operate a powered truck for moving materials around the warehouse.
