Tax preparer from Indiana indicted for tax fraud

An Indiana woman has been indicted by a federal grand jury for multiple counts of preparing false tax returns at several Toledo tax firms, the U.S. attorney's office has announced.

Janice L. Sullivan of Evansville, Ind., faces 30 counts of aiding and assisting in preparing false tax returns while employed as a tax preparer for the firms of H&R Block and Jackson Hewitt in Toledo from 2000 through 2003.

The indictment alleges that Ms. Sullivan prepared 30 tax returns containing false business loss deductions, rental property loss deductions, earned income credits, and child care credits, causing the Internal Revenue Service to issue $124,662 in fraudulent refunds.
