Recently released inspection reports of Lucas County food-service operations:

George A. Phillips Academy, 3648 Victory, inspected Nov. 13. Critical violations: Hamburger being held at 54 degrees at lunch center. Yogurt being held in the single door Frigidaire was 43 degrees. Cold holding potentially hazardous foods must be maintained at 41 degrees or less. Three other non-critical violations. Inspector: Bruce Keiser.

Step Inn Coffee Howe, LLC, 3101 Lagrange, inspected Nov. 6. Critical violations: Observed openned container of macaroni salad without a date mark. All PHFs kept longer than 24 hours must be date marked and held at 41 degrees or less for no more than 7 days. Observed bulk cooling and reheating of hot dog sauce. Facility is not currently equipped for these activities. Four non-critical violations. Inspector: Kerry Cutcher.

McDonald s, 5210 North Summit, inspected Nov. 13. Critical violation: Observed no sanitizing wiping bucket set up at time of inspection. Wet wiping cloths were on surfaces. During all hours of operation, facility must have sanitizing buckets set up with a chemical concentration of chlorine 11 ppm to properly store and sanitize wet wiping cloths. Ham on the counter was 61 degrees. All PHFs must be held at 41 degrees or below, or at 135 degrees or above. Three non-critical violations. Inspector: Cutcher.

Vito s Pizza, 7060 West Sylvania, inspected Nov. 13. Critical violations: Observed employees grabbing lettuce from the prep cooler with their bare hands. There should be no bare hand contact with ready-to-eat foods. PHFs in small prep cooler were being held at 65 degrees and above, all PHFs must be held at 41 degrees or below. One non-critical violation. Inspector: Cynthia Garcia.

Chuck E Cheese, 5077 Monroe, inspected Nov. 13. Critical violations: Ham in prep top cooler being held at 43-53 degrees. All PHFs must be held at 41 degrees or less. All refrigerated ready to eat PHFs must be date marked.The Quat sanitizer is too high at 400 ppm in the 3-bay sink and wipe cloth bucket. Quat must be 200 ppm. Seven non-critical violations. Inspector: Kelly Sattler.

McDonald s, 36 North Reynolds, inspected Nov. 14. Critical violations: The vegetable soup in the soup warmer was 114 degrees. All PHFs must be held hot at 135 degrees or higher to prevent growth of bacteria. Four non-critical violations. Inspector: Gillian Wilke.

Quality Time Child Care, 2315 Dorr, inspected Nov. 14. Critical violation: Milk in reach-in was 44 degrees. All PHFs must be held at 41 degrees or less. Five non-critical violations. Inspector: Wilke.

H-Town LTD, 711 Marmion, inspected Nov. 14. Critical violations: Chicken wings in the kitchen reach-in were 43-44 degrees and the milk in the bar cooler was 42 degrees. All PHFs must be held at 41 degrees or less to prevent the growth of bacteria. No hot water available in the men s or women s restrooms. Restore hot water to both bathroom handsinks to promote proper hand washing. Seven non-critical violations. Inspector: Wilke.

Long John Silver s, 1027 Conant, inspected Nov. 15. Critical violations: Raw fish should be stored above sauces in walk-in coolers to prevent potential cross-contamination. Four non-critical violations. Inspector: Paul Bauman.

Subway, 584 West Dussel, inspected Nov. 15. Several PHF items open without any date marking. All PH ready-eat-foods held longer than 24 hours must be marked with a seven day discard date. Inspector: Bauman.

Bob Evans Restaurant, 1220 East Alexis, inspected Nov. 6. Critical violations: Observed dish washer and grill cook without hair restraints. Employees handling food or clean items must wear hair restraints to protect from contamination. Scoop for coleslaw and tongs for lettuce were submerged in food product at server prep top. These should be stored handle side up and out of food product. Observed wet wiping cloths on surfaces throughout facility. Store wet cloths in sanitizing solution with a chemical concentration of (Quat 200 ppm) to limit bacterial growth. Observed hot food cooling in walk-in completely covered. To cool foods properly, pans must be uncovered or lids must be vented to allow heat to escape. This is a repeat violation. Observed PHF items at temperatures ranging from 45-72 degrees. These must be held at 41 degrees or less. Observed Quat in wiping buckets at 100 ppm. Proper concentration for Quat must be 200 ppm to properly sanitize. Salad prep top unit unable to hold PHFs at 41 degrees or below. Have salad prep unit repaired or adjusted. Grill cooler drawers not turned on at inspection and holding PHFs at 58-68 degrees. Be sure all equipment is properly funtioning and able to hold food at 41 degrees or below. One non-critical violation. Inspector: Cutcher.

HJs Prime Cut, 505 East Alexis, inspected Nov. 7. Critical violation: Ice scoop stored with handle in ice. Store scoop handle side up and out of ice or in a clean dry container to protect from contamination. Various food items in walk-in cooler were uncovered. Food must be protected from contamination. Open bags of beans and flour were on dry storage shelf. Transfer food once opened to a food grade container or bag that is sealable. Various cooked PHFs were without date marks. Chicken noodle soup in hot holding at 116 degrees. All PHFs in hot holding must maintain 135 degrees or above. Egg mix was at 57 degrees in dissolved ice bath and open jar of garlic was at 72 degrees on counter. For an ice bath to be successful it must be an ice water solution, up to the level of food product on all 4 sides. Three non-critical violations. Inspector: Cutcher.

Fallen Timbers G.C., 7711 Timbers, inspected Nov. 8. Critical violations: Dishmachine at this time is not putting out sanitizer. Have machine repaired. Sanitizer should be at 50-100 ppm at all times. Date mark all PHFs with a 7 day expiration date once product is opened, prepared or thawed. One non-critical violation. Inspector: Sara Becker.

Waffle House, 7050 Orchard Center, inspected Nov. 8. Critical violation: Full hair restraints must be worn at all times by persons working in the kitchen. Inspector: Becker.

Casa Fiesta, 1705 Wheeling, inspected Nov. 8. Critical violations: Dish washer handling soiled and clean dishes without washing hands in between, or before handling clean dishes. Observed drinks without lids in kitchen and dish area. Employee drinks must have lid or cap to protect food and food contect items from contamination. Ice scoop was stored with handle submerged in ice. To protect from contamination scoop should be stored handle side up out of ice or in clean container. Raw animal products must be stored below ready-to-eat foods in walk-in cooler to prevent cross contamination. No sanitizing wiping buckets set up in kitchen and wet wiping cloths on surfaces. Facility must have sanitizing wiping buckets set up during all hours of operation with a chemical concentration of (Quat 200 ppm/chlorine 100 ppm) to properly sanitize surfaces and store damp cloths. Food on steam table and in prep top exposed to splash from handsink. Install splash guards to protect food from contamination. PHFs were not date marked. All PHFs kept longer than 24 hours must be date marked, held at 41 degrees or less. This is a repeat violation. Beans cooling overnight found at 50 degrees. Proper cooling must be done rapidly to limit bacteria growth, 135-70 degrees within 2 hrs, additional 4 hours to reach 41 degrees. This is a repeat violation. Observed open container of drink mixes labeled refrigerate after opening, follow label instructions. Thirteen non-critical violations. Inspector: Cutcher.

Kentucky Fried Chicken, 3506 Dorr, inspected Nov. 6. Critical violation: Bag of raw chicken being quick defrosted in bag found at 68 degrees. While quick defrosting under cold water product must be maintained at 41 degrees. Five non-critical violations. Inspector: Keiser.
