Recently released inspection reports of Lucas County food-service operations:

Subway, 5840 West Central, inspected May 10. Critical violations: There is a small amount of mildew in ice maker bin. Provide a covered waste container in unisex restroom. Inspector: James Herzberg.

Rally's, 5855 West Central, inspected May 10. Critical violations: Two food workers wearing a visor without a hairnet and another without any hair restraint. All food workers must wear hats or hair nets, a visor can be worn only if a hair net is worn underneath. Replace broken metal ice scoop. Inspector: Herzberg.

Andre's Lounge, 1337 North Summit, inspected May 10. Critical violations: Chicken soup in three-door cooler at 48 degrees. All potentially hazardous foods must be kept at 45 degrees or less. Soup discarded. All refrigerated, potentially hazardous foods must be date marked. No date markings at time of inspection. Chicken soup was improperly cooled. Foods must be cooled to 70 degrees in two hours and down to 45 degrees or less in four hours. Inspector: Kelly Sattler.
