Recently released inspection reports of Lucas County food-service operations:

Critical violations

Libby Inc., 940 Ash, inspected July 27. Critical violations: Salad dressing served as buffet style self-serve, no sneeze guard was present to protect food from contamination. Raw animal products were stored above hashbrowns, raw food products need to be stored under all other food items. Various potentially hazardous foods were not date marked. Various potentially hazardous foods were held at temperatures from 50 degrees to 53 degrees F, these foods must be kept at 45 degrees F or below. Inspector: Kerry Cutcher.

Buck Shots Sports Bar, 1128 North McCord, inspected July 28. Critical violations: Facility must use date marks on all perishable foods, labeled with the date of thaw, preparation or opening, and should be held for no more than 7 days . Inspector: Jaclyn Kistler.

Harnoor LLC, DBA East Side Mini Mart, 615 Oak, inspected July 25. Critical violations: Gloves need to be provided for handling hot dog buns. Hot dogs were being stored on top of ice bags in ice freezer, these need to be in a commercial grade freezer. Inspector: Jennifer Gottschalk.

Bambino s Pizza, 4466 Monroe, inspected July 28. Critical violations: Observed bare hand contact with ready to eat food. Improper washing of cook ware, there was no sanitizer being used. Spray head sets below flood level of sink. Replace spring spray head to prevent a submerged inlet. Inspector: Bruce Keiser.

Toddler School IV, 806 Starr, inspected Aug. 1. Critical violations: Observed open cheese in cooler without a date mark. Milk in the infant black refrigerator was at 50 degrees F. Milk and formula must be held at 41 degrees or lower. Out of date baby food was found in the storage room. Inspector: Gottschalk.

Loyal Order of the Moose, 226 South Byrne, inspected Aug. 1. Critical violations: Potentially hazardous food items found at 52 degrees, must be kept at 41 degrees or lower. Potentially hazardous foods found with no date marks. Inspector: Cynthia Garcia.

Frisch s Big Boy, 3537 Secor, inspected Aug. 8. Critical violations: Under counter cooler is holding at 47 degrees F. All potentially hazardous foods must be held at 41 degrees or lower. Many potentially hazardous foods were found at room temperature, including cheeses for sandwiches, hashbrowns, eggs, butter packets and half and half. These foods must be held at 41 degrees or lower. Macaroni and cheese was overfilled in steam well. Dish machine was not dispensing any chlorine. Improper food storage, ground beef must be stored below lunchmeats in the undercounter cooler. Sunset Village, Barlow Kitchen, 9640 Sylvania-Metamora, inspected Aug. 9. Critical violations: Dish machine was not sanitizing at time of inspection. Inspector: Jaclyn Kistler.

Bambino s Pizza, 1928 Broadway, inspected Aug. 12. Critical violations: Potentially hazardous foods in walk-in cooler and under prep top without date markings. Provide a thermometer for prep top and prep cooler to constantly monitor temperatures. Inspector: Ryan Sekinger.

Summit Diner, 1039 North Summit, inspected July 28. Hair restraints were not being worn by food handler. Hair restraints must be worn at all times by food handlers. Single use container was being reused, and needed to be discarded. Use only commercial grade, NSF, or equivalent equipment or utensils. French fries were found in bucket on the floor, all food items must be kept at least 6 inches off the floor. Coffee was stored on the floor in dry storage area. Unlabeled chemical bottle was in storage, all chemicals must have proper label to prevent chemical contamination. Inspector: Kerry Cutcher.

City Q Barbeque, 7402 West Central, inspected July 29. Critical violations: Provide a barrier between customers and BBQ pit prep area to prevent contamination of food or food contact surfaces. Bag of ice mixed with loose ice in ice machine. Inspector: Kistler.

Sunset House, main kitchen, 4020 Indian, inspected Aug. 1. Critical violations: Repair dishroom handsink. Very low hot water pressure and no cold water at time of inspection. Inspector: Sattler.

Jersey s Sports Cafe, 1001 Starr, inspected Aug. 1. Critical violations: All hand sinks in the kitchen and bar areas must remain clean, clear, and stocked with soap and paper towels for hand washing. Provide a proper stopper for drain at the bar sink. Inspector: Gottschalk.

Rod s Bar and Grill, 2755 Lagrange, inspected Aug. 1. Hair restraints were not being used at time of inspection. No gloves were available at time of inspection. Potentially hazardous foods were not marked with date of expiration. Inspector: Cutcher.

Pete s Hot Dog, 2529 Starr, inspected Aug. 1. Critical violations: Pie cooler was holding at unsafe temperatures. All potentially hazardous foods must be kept at 41 degrees Farenheit or lower. Inspector: Julie Nye.

Toledo Hilton, 3100 Glendale, inspected Aug. 1. Critical violations: Splash guards needed on both sides of 2 handsinks at new bar. Consumer advisory needed on new menu. Eliminate standing water from bottom of Delfield, reach-in portion of prep top cooler. Inspector: Mike Brady.

Mad Dog Rib and Grub, 1014 Starr, inspected Aug. 2. Critical violations: Date markings were not observed on all opened food items. Inspector: Gottschalk.

Brandywine Pool Shack Bar, 6904 Salisbury, inspected Aug. 2. Critical violations: All chemicals should be stored below food or food contact items. Inspector: Kistler.

Subway, 1100 McCord, inspected Aug. 2. Critical violations: Keep lids on pre-weighed meats, maintain at 41 degrees F or lower. Inspector: Kistler.

Sam s Diner, 578 Dussel, inspected Aug. 2. Critical violations: Repair dishwasher, no sanitizer or detergent was being dispensed. Inspector: Steven Perrine.

Westgate Coney Island, 3301 West Central, inspected Aug. 3. Critical violations: Cheese sitting on counter was found at 64 degrees Farenheit during inspection. New prep cooler must be maintained at 41 degrees Farenheit or lower. Reheat foods to at least 165 degrees Farenheit on a direct heat source before placing in steam table. Walk-in cooler condenser is leaking. Inspector: Sattler.

Jewish Community Pool, 6465 Sylvania, inspected Aug. 3. Critical violations: employess were touching cones with bare hands. Gloves need to be used when handling ready to eat foods. Inspector: Kistler.

Positive Beginnings, 601 Fassett, inspected Aug. 3. Critical violations: Raw eggs were being stored on top shelf of refrigerator, all raw eggs need to be stored below other food items. A child was observed making a sandwich with regular utensils that need to be cleaned and sanitized. An ant trap was found on the counter near the microwave oven. Traps should not be placed on food contact surfaces. Inspector: Gottschalk.
