Restaurant inspections

Recently released inspection reports of Lucas County food-service operations:

VFW Post 5536, 1032 West Sylvania, inspected Aug. 28. Critical violations: Remove or pack up and store all unused equipment and utensils in order to facilitate cleaning. Upstairs rental hall three-compartment sink must be kept clear at all times and available for use. Inspector: Steve Perrine.

Chuck's Hungry I, 4477 Monroe, inspected Aug. 29. Critical violations: Observed food residue on slicer. Clean and sanitize slicer after using. Four noncritical violations Inspector: Bruce Heiser.

Six Pack Bar and Grill, 3223 West Sylvania, inspected Aug. 30. Critical violations: Thousand island dressing and cheese held at improper temperatures. To prevent the growth of pathogens which may cause a food borne out break keep potentially hazardous foods (PHF) under proper refrigeration temperatures. Product voluntarily discarded. Five noncritical violations. Inspector: Heiser.

Inverness Club, pool snack bar, 4601 Dorr, inspected Aug. 30. Critical violation: nacho cheese in dispenser at 125 degrees. All hot PHF must be held at 135 degrees or higher to prevent growth of bacteria. Keep the dispenser on once cheese is warmed. Cheese may not be cooled down at the end of the night. One noncritical violation. Inspector: Gillian Wilke.

The Oar House, 5044 Suder, inspected Aug. 30. Critical violations: No sanitizing bucket observed at time of inspection. During hours of operation, facility must have sanitizng bucket setup with a chemical concentration of (quat 200 ppm, chlorine 50-100 ppm) to properly sanitize surface and store wet wiping cloths. Observed bag of pop corn kernels on floor behind bar. To protect food from contamination, store at least six inches off the floor. Observed boxes of soft drink syrup on floor. Keep all food and food contact items at least 6 inches off the floor. Observed PHF in prep top and walk-in cooler at improper temperatures. All PHF in cold holding must maintain 41 degrees or less to limit bacteria growth. All PHF discarded. Provide a proper temperature measuring device to verify temperatures of foods. Observed equipment not function or maintained properly. Prep-top hold food at 54 degrees. Walk-in cooler at 48-49 degrees. Walk-in freezer has excessive ice accumulation. This kitchen is temporarily closed by order of the Toledo Lucas County Health Department. The facility cannot re-open until repairs are made and verified by this department. No handsink observed in back prep and ware washing area. Install a commercial grade NSF approved handsink in back area. Observed broken and chipping asphalt in outside storage and bar area. Surfaces must be smooth and non-absorbent and easy to clean. Observed employee smoking back warewashing area. There is to be no smoking in food or food contact items areas to protect and food contact items from contamination. Observed several items (grills, walk-ins) that have not been approved by the Toledo Lucas County Health Department. All equipment or menu item added is subject to the approval by this office. Nine noncritical violations. Inspector: Kerry Cutcher.

El Rodeo Mexican Restaurant, 5125 Monroe, inspected Aug. 28. Critical violations: Found tamales in the prep top cooler at 51 degrees. All PHF must be held cold at 41 degrees or less at all times. After foods are cooked it must be cooled rapidly to prevent the growth of harmful bacteria. Tamales were cooked yesterday and cooled and were to be reheated. Tamales were discarded at time of inspection due to temperature abuse. A pan of cooked chicken was hot holding on top of the oven in the kitchen. This is an incorrect way to hold PHF. Chicken was moved inside the oven for proper holding at 135 degrees or higher. Small pass-through walk-in cooler is holding at an ambient temperature of 46 degrees. All cooler must maintain foods cold at 41 degrees or less at all times. Repair cooler. Beans, pork , and chicken in the walk-in cooler were cooled improperly. These foods were prepared and cooled the day before and are still holding at 43-45 degrees when the ambient cooler temperature is 38 degrees. All food must be cooled rapidly for 135 degrees to 70 degrees within two hours and then from 70 degrees to 41 degrees or less within the next four hours. Cool foods in shallow pans and use ice wands to speed up the cooling process. Cooler drawer under the grill on the cook line holding foods at unsafe temperatures. All PHF must be held cold at 41 degrees or less at all times. Do not use cooler until it can maintain proper temperatures. Meats, poultry, and seafood in cooler voluntarily discarded. No consumer advisory on menu. This is required. Found four open bottles of drink mixes at the bar at room temperature. Label states that product must be refrigerated once opened. Mixes discarded. Handsink at bar blocked by trash can. Hansinks must be kept clear and accessible at all times. This is a repeat violation. All doors to the facility must remain closed unless screen doors are installed to prevent pests from entering. Front door and side patio door were both open at time of inspection. Observed many flies and fruit flies in all areas of the facility. Call pest control operator. Observed raw chorizo stored directly above milk, cheese, and whipped cream in the walk-in cooler. Raw meats must be stored below ready-to-eat (RTE) to prevent cross contamination. Outside dry storage area is left open during the day. This door must remain closed at all time to prevent contamination by pests. Observed many bags of dry goods open in the dry storage area. Once bags are opened food must be stored in a sealed food grade, reusable container. Found wet wiping cloths under the vegetable slice and on table tops throughout the kitchen. Wet wiping cloths must be stored in sanitizer to prevent bacterial growth. RTE, PHF, must be date marked. Foods may be held up to 7 days at 41 degrees. Kitchen handsink did not have soap at time of inspection. All handskinks must be stocked with soap and paper towels at all times. Fifteen noncritical violations. Inspector: Kelly Sattler.

Ponderosa Steakhouse, 2080 Woodville, Oregon, inspected Aug. 29. Critical violations: Dressing in container at room temperature. All dressings must be kept under refrigerated after opening. Observed dish person touching clean dishes after handling dirty dishes. Dish person must wash hands between handling dirty and clean dishes to prevent contamination. Observed an open drink on equipment. All employees drinks must have a lid and straws to prevent contamination. This is a repeat violation. No date-marking observed on PHF, RTE food items. Observed potatoes being prepped in the three-compartment sink. All prepping must be done in the prep sink. Two handsinks were not stocked with paper towels. All handsinks must be stocked with soap, warm water, and paper towels. Sanitizer buckets had no sanitizer in them. Sanitizer buckets must have proper levels of sanitizer in them. Observed wiping cloths around facility. All cloths must be stored in sanitizer solution when not in use. Butter and sour cream at 50-60 degrees. All PHF must be kept at 41 degrees or lower to prevent contamination. Twelve noncritical violations. Inspector: Julie Nye.

Amigo's, 5327 Dorr, inspected Aug. 29. Critical violations: Observed food handler preparing a taco without gloves. All food handlers must wear gloves when handling RTE foods. Raw shrimp stored above salsa and raw hamburger stored above vegetables. Always store RTE foods above raw meats to prevent cross contamination. Observed chemical stored above freezer in the liquor room. All chemicals shall be stored in a designated area below and away from food to prevent chemical contamination. Cooled ribs in freezer and in prep top cooler were not date marked and do require proper date labeling. Cooler that hold food at 41 degrees or less may store food for up to seven days. Coolers holding at 42-45 degrees may store food for four days. Thirteen noncritical violations. Inspector: Wilke.

Ranya's, 608 Adams, inspected Aug. 29. Critical violations: Observed employees wrapping silverware without gloves. Employees should wash hands, place gloves on and then wrap silverware on a clean and sanitized surface. Observed containers of food that were prepared the day before with wrong date or not labeled at all. Once food is prepared or the manufacturer's seal is broke food kept for more than 24 hours must be marked with a date and after seven days including day of prep and or opening must be discarded or completely used. Five noncritical violations. Inspector: Kelly Ivans.

St. Luke's Hospital, 5901 Monclova, Monclova Township, inspected Aug. 30. Critical violation: Two pans of meatballs found in hold holding at improper temperature. Reheat all items to 165 degrees and maintain them at 135 degrees or more. Two noncritical violations. Inspector: Sara Becker.
