Restaurant inspections

Recently released inspection reports of Lucas County food-service operations:

Clay Avenue Community School, 1030 Clay, inspected Oct. 1. Critical violation: No sanitizing wiping bucket setup at time of inspection. During all hours of kitchen operation, there must be a sanitizing bucket set up with chemical concentration of the proper strength. Inspector: Kerry Cutcher.

Tavern 531, 531 Oak, inspected Oct. 1. Critical violations: Wet towel at bar. All wet towels, when not in use, must be in sanitizer. No prep sink available. Observed whole lettuce and tomatoes in cooler. Facility does not have proper sink to wash produce. All produce must arrive prewashed and precut. Current invoices must be available on location at all times. Provide invoice for produce or install sink with a 2-inch air gap to the sewer line. All work must be done by a licensed plumber with proper permits pulled. Pickles and pickled cauliflower are not licensed to be sold. They are not labeled correctly and not labeled with a Department of Agriculture establishment ID number or street address or phone number. These items are embargoed and may not be sold. Facility may only sell food from certified and inspected wholesaler. Inspector: Jennifer Gottschalk.

Arby s, 1455 Secor, inspected Oct. 1. Critical violations: The top position of the prep top cooler is holding 50-58 degrees and turkey and swiss cheese at 48 degrees. All PHF must be held at 45 degrees or less to prevent the growth of bacteria. The ambient air temperature of the top was 46 degrees and not capable of holding at 45 degrees or less. Do not use top. Food voluntarily discarded. The red sauce on ice was at 61 degrees. The bottle stated it is to be refrigerated after opening. All PHF must be held at 45 degrees or less to prevent the growth of bacteria. Observed an employee beverage, without a lid, stored in the reach-in freezer next to food. All employee beverages must have a lid and be stored below and away from food to prevent contamination. Observed a can of Raid behind service counter. The use of this product in this facility is not permitted. Contact a licensed pest control service to treat the facility. Five noncritical violations. Inspector: Gilllian Wilke.

Jax Bar and Grill, 3560 Dorr, inspected Oct. 15. Two critical violations: Provide stainless cold plates to separate ice to be consumed from the beverage hose system. Observed drink mix out at room temperature. Three noncritical violations. Inspector: Kelly Cipiti.

Trot N Charlie s, 4837 North Detroit, inspected Oct. 18. One critical violation: Observed wet wiping cloths on surfaces and no sanitizing wiping bucket set up. Three noncritical violations. Inspector: Cutcher.

Duds N Suds Laundromat, 3251 Navarre, Oregon, inspected Oct. 10. Two critical violations: Observed food at unsafe temperatures. Observed some ready-to-eat potentially hazardous foods without a date mark or with an out-of-date date mark. One noncritical violation. Inspector: Julie Nye.

Sigma Restaurants, Subway, 3721 Navarre, Oregon, inspected Oct. 11. Three critical violations: observed food at unsafe temperatures. Observed no date marking on bagged meat, chicken. Observed employee beverage on shelf in walk-in cooler; employee items must be stored below and away from all facility food in a designated area. One noncritical violation. Inspector: Nye.

Picrose, 3253 Franklin, inspected Oct. 15. One critical violation: observed quat in 3-vat sink at 100 ppm instead of 200 ppm. Four noncritical violations. Inspector: Cutcher.

11th Frame Fryer at New Glass Bowl Lanes, 5133 Telegraph, inspected Oct. 15. Four critical violations: observed food handler change gloves after handling money without washing hands. Must wash hands between glove changes and tasks. Observed ice cream scoop stored at room temperature in standing water; store scoop on clean, dry surfaces, and wash, rinse, and sanitize at least every four hours. Observed no date marking of opened packages of potentially hazardous foods, such as hot dogs and deli meats, repeat violation. Observed mold on inside of ice machine. Seven noncritical violations. Inspector: Cutcher.

Wernert Elementary School, 5050 Douglas, inspected Oct. 16. One critical violation: pizza in the McCall 3-door cooler was running at 50 degrees; food must be held at 45 degrees or less. Contact licensed repairman; do not store potentially hazardous food in the cooler until approval given by the health department. One noncritical violation. Inspector: Wilke.

Montrie Auction Gallery, 4017 Lagrange, inspected Oct. 16. One critical violation: observed ranch dressing in glass display at 54 degrees; must be held at 41 degrees or less. One noncritical violation. Inspector: Cutcher.

Chipotle Mexican Grill, 1385 Conant, Maumee, inspected Oct. 16. One critical violation: steak in hot holding area at 124 degrees. Must maintain internal temperature of 135 degrees. Steak reheated and hot holding equipment turned up. Inspector: Paul Bauman.
