Recently released inspection reports of Lucas County food-service operations:

Steak N Shake, 5395 Monroe, inspected March 10. Critical violation: Label all spray bottles with common name of contents for identification. Five noncritical violations. Inspector: Brandon Tester.

Penn Station East Coast Subs, 4798 Monroe, inspected March 10. Critical violation: Wet wiping cloths must be stored in sanitizer to prevent bacteria growth. Observed wet wiping cloth on prep counter at time of inspection. Eight noncritical violations. Inspector: Kelly Sattler.

Rose Garden Again, 1800 Miami, inspected March 11. Critical violations: Observed butter, garlic butter, and chocolate syrup at room temperature. All potentially hazardous food (PHF) must be held cold at 41 degrees or less to prevent bacteria growth. Read all labels for proper food storage. Discarded food items at time of inspection. Found PHF at 53-58 degrees on the left side of the prep cooler. All PHF must be held cold at 41 degrees or lower. Removed and discarded all PHF from this section. Make any needed repairs to this cooler to ensure that it maintains proper temperature. No date labeling observed on some food items. Observed PHFs that were expired. Practice good date marking and product rotation. Observed raw shell eggs being stored on top of bread. To avoid cross contamination always store raw shell eggs below ready-to-eat (RTE) food. Correct. Observed two waffle griddles in the kitchen area. This equipment was never approved and this food not approved to be added to menu. Cease use of this equipment immediately. Complied at time of inspection. Sanitizer bucket tested at 0 ppm. Maintain 200 ppm quat sanitizer. Check with test kit to ensure proper concentration sanitizer is maintained. Church s Chicken, 2124 Franklin, inspected March 12. Critical violation: Keep handsink stocked with soap and paper towels. Seven noncritical violations. Inspector: Debbie Dacquisto.

Wendy s, 1945 Woodville, inspected March 5. Observed food held at unsafe temperatures. All PHF must be held cold at 41 degrees or lower to prevent bacteria growth that can cause food-borne disease. Five noncritical violations. Inspector: Julie Nye.

Navy Bistro, 30 Main, inspected March 12. Critical violations: Observed PHF held past the seven-day expiration date. Practice good food rotation. Observed shredded cheese and sour cream out at room temperature. These items are PHF and require temperature control. All cold PHF must be held at 41 degrees or lower. Observed mashed potatoes at 129-130 degrees on the hot holding line. All PHF holding hot must be maintained at 135 degrees or above. Inspector: Gottschalk.

Tango s, 30 Main, inspected March 12. Critical violations: Observed PHF in single-door cooler. Hold food at 48-51 degrees. All PHF held cold must be maintained at 41 degrees or lower. Observed soy sauce and chocolate syrup on counter. Both labels state keep refrigerated after opening. Discard items. Follow label instruction. Refrigerate necessary items to prevent bacteria growth. Observed raw steak stored over yogurt and bagels and raw bacon over cooked pasta. To avoid cross contamination follow proper storage orders. Store all raw meats below ready-to-eat foods. Observed significant build-up on the inside of ice machine. To prevent the growth of bacteria, completely sanitize all inside surfaces of the ice machine. Properly stock handsinks with soap and towels. Inspector: Gottschalk.

Subway, 3721 Navarre, Oregon, inspected March 10. Critical violations: Observed cheese at unsafe temperature of 127 degrees. All PHF must be held at 135 degrees or more to prevent growth of bacteria to prevent food-borne illness. Six noncritical violations. Inspector: Nye.

Tropics Lounge, 1483 Sylvania, inspected March 11. Critical violations: Provide a minimum of six more cooling wands are needed for the volume of food that is cooled. Clean and sanitize inside of ice machine, maintain clean. Corned beef was improperly cooled. Nine noncritical violations. Inspector: Dacquisto.

Maumee Elks, 137 West Wayne, Maumee, inspected March 11. Critical violation: Date mark all PHF with a seven-day expiration date. Seven noncritical violations. Inspector: Sara Becker.

Tony Packo s Express, 4701 Talmadge, inspected March 13. Critical violation: Observed several PHF items throughout facility without proper date marking. All PHF kept longer than 24 hours must be properly date marked. Two noncritical violations. Inspector: Tester.

Glass City Bar & Grill, 1705 Tollgate, Maumee, inspected March 14. Critical violations: Dishmachine in bar area is not dispensing sanitizier. Contact repair service. Manually wash all glassware until repairs are complete. Creme puffs found at room temperature. All PHF must be held hot at 135 degrees or more or cold at 41 degrees or less. Items discarded. Numerouse opened PHF being held without any date marking. All opened/prepared, RTE, PHF held longer than 24 hours must be marked with a seven-day discard date. Cheese stored improperly in prep-top cooler did not appear wholesome. Food should not be stored on top of wells in prep-top. Cheese was discarded. Raw eggs stored above RTE foods in walk-in cooler. To prevent potential cross-contamination, store all raw foods below RTE foods. Discard container of tartar sauce held beyond seven-day discard date. Eight noncritical violations. Inspector: Paul Bauman.

International House C Store, 2801 West Bancroft, inspected March 14. Critical violation: Observed batteries and cleaning supplies stored above/on food. Store all chemicals below and away from food to prevent from chemical contamination. Two noncritical violations. Inspector: Gillian Wilke.

7-Eleven, 537 Woodville, inspected March 10. Critical violations: Observed salsa, pickles, relish at 44 degrees in the prep top cooler. PHF must be maintained cold at 41 degrees or lower. Observed space between the container allowing cold air to escape. Provide spacers or use larger containers so air does not escape. Observed eggs stocked above milk and juice in the reach-in door of the walk-in cooler. Store eggs below all RTE food and drinks to prevent cross contamination. Four noncritical violations. Inspector: Gottschalk.

Papa John s Pizza, 3231 Dorr, inspected March 10. Critical violations: The re-packaged bags of chicken wings in the walk-in were not properly dated. All opened/prepared, PHF held over 24 hours must be labeled with a date to prevent the growth of bacteria. The three-compartment sink was incorrectly set-up at the time of inspection and the quat sanitizer in the last bay was only 100 ppm. The wash basin is not being used because when it is drained the grease trap overflows. Immediately empty and clean the grease trap. Properly setup the three-compartment sink and ensure sanitizer is of proper strength. Five noncritical violations. Inspector: Wilke.
