Eastborn Mini Mart, 1110 East Broadway, inspected Sept. 9. Critical violations: Provide paper towels for the restroom hand sink. All hand sinks must be stocked with soap, and paper towels and hot water at all times for proper hand washing. Observed shell eggs and raw bacon stocked over milk and ready-to-eat food. Toledo Preparatory Academy, 540 Independence, inspected Sept. 5. Critical violation: The 4-door cooler is holding PHF above 41 degrees. All PHF must be held at 41 degrees or less to prevent the growth of bacteria. Food was voluntarily discarded. Golden Jade, 3171 Navarre, inspected Sept. 2. Critical violations: Observed food at unsafe temperature of 52 to 54 degrees. Child Nutrition Services, 1702 Woodville, inspected Sept. 8. Critical violation: Observed no hot water in hand sink. All hand sinks must have water at a minimum of 100 degrees for proper hand washing. Increase hot water temperature capacity or fill pans with cool or warm water. Do not use all your hot water. Ducat s Imperial Lanes, 5505 West Central, inspected Sept. 8. Critical violation: Refrigerated chicken wings must be date marked when the package is opened. Friendly s Restaurant, 1502 South Reynolds, Maumee, inspected Sept. 8. Critical Violation: Eliminate standing water from bottom of prep top cooler. Do not store food or food containers in the standing water. One noncritical violation. Inspector: Mike Brady.

Granite City Food & Brewery, 2300 Village West, inspected Sept. 8. Critical violations: Provide a handeled scoop for the salt bin. Pan being used as a scoop is a potential source of hand contamination. All utensils must be wrapped if presetting the table for service to prevent contamination of the utensils. Handle of the waffle fry scoop found in the fries. If handle cannot be kept out of food product then remove the scoop when it is not in use to prevent hand contamination. All food employees must not wear jewelry or watches on the hands/arms in order to prevent contamination of food. Hand wash sink at the dish machine has blocked accessibility; knives and a used glove found in the sink. Unit must be kept free of unnecessary items. Hand soap not present at the dish machine area hand sink. Hand sinks must always be stocked with proper supplies for adequate hand washing. Hand sink at the bar with blocked accessibility, used liquor bottles being held in the sink. Sink shall be kept free of unnecessary objects to promote hand washing. Five noncritical violations. Holiday Inn Express, 3154 Navarre, Oregon, inspected Sept. 8. Critical violations: Observed food at unsafe temperatures, eggs 111 degrees and gravy 132 degrees. All potentially hazardous food must remain at 135 degrees or higher to prevent bacteria growth that can cause a food-borne disease. Turn temperature up so it can maintain food at 135 degrees or higher or repair unit to hold food at safe temperatures. Observed food at unsafe temperatures, cream cheese at 51 to 57 degrees. All potentially hazardous food must remain at 41 degrees or lower to prevent bacteria growth that can cause a food-borne disease. All sanitizer buckets must be 200 ppm quat to properly sanitize counters. No other violations. Inspector: Nye.

Arbor Hills Junior High School, 5334 Whiteford, Sylvania, inspected Sept. 9. Critical violation: Temperatures of PHF on hot hold line and hot box below 135 degrees. All PHF must be held hot at 135 degrees or higher to limit bacteria growth. No other violation. Inspector: Brandon Tester.

Briskets, 2474 Navarre, Oregon, inspected Sept. 9. Critical violations: Observed heating of food product in the steam table sauce at 38 degrees in steam table. To properly reheat food product to limit bacteria growth that can cause a food-borne disease it must be reheated over direct heat. Observed a pest control spray stored on the same shelf as food product and over food products. All chemicals must be stored below/away from any food products prevent chemical contamination. All pest control must be done by a professional pest control company. Do not service facility without pest control service. All sanitizer buckets must be 50-100 ppm chlorine to properly sanitize counter tops. Seven noncritical violations. Inspector: Nye.

LOF Cafeteria, 811 Madison, inspected Sept. 9. Critical violation: Observed chlorine sanitizer buckets at 50 ppm and below. To allow for proper sanitation maintain sanitizer buckets at 100 ppm chlorine. Use test kit to check levels. Do not add soap to sanitizer buckets. No other violations. Inspector: Kelly Cipit.

St. Charles Mercy Hospital, 2600 Navarre, Oregon, inspected Sept. 9. Critical violation: Observed food at unsafe temperatures, melon at 50 degrees. All PHF must be held at 41 degrees or lower to prevent bacteria growth that can cause a food-borne disease. Two noncritical violations. Inspector: Nye.

IHOP, 6920 Central, inspected Sept. 11. Critical violations: Observed wiping cloths resting out of sanitizer. Keep cloths in sanitizer when not in use to prevent growth of bacteria. Observed employee handling ready-to-eat food with bare hands. Any ready-to-eat food must be handled with gloves or utensils. Observed moldy lemons stored with fresh lemons in walk-in cooler. Keep all produce fresh, free from mold to prevent contamination. Three noncritical violations. Inspector: Brady.

Waffle House #1544, 6715 West Central, inspected Sept. 11. Critical violation: Observed eggs stored above raw hamburger; eggs and all raw poultry products must be stored under all other raw products in coolers. One noncritical violation. Inspector: Brady.

Compass, 2465 Collingwood, inspected Sept. 12. Critical violations: Walk-in cooler milk at 42 degrees; PHF must be held at 41 degrees or less to inhibit the growth of bacteria that can cause food-borne illness. Adjust/service cooler to run at 41 degrees or less. Two-door cooler by cook line, bean soup at 48 degrees; PHF must be held at 41 degrees or less, All hazardous foods discarded at time of inspection. One noncritical violation. Inspector: Dacquisto.

DeVeaux Middle School, 2620 West Sylvania, inspected Sept. 12. Critical violations: The fish sandwich in the hot holding unit was only 130 degrees. All PHF must be held hot at 135 degrees or higher to prevent the growth of bacteria. The cream cheese setting out at room temperature in the ala carte server was 58 degrees. All potentially hazardous food must be held at 41 degrees or less to prevent the growth of bacteria. The ala carte server does not have a cooler to store the cream cheese and therefore must provide a small NSF approved, commercial grade cooler to store the cream cheese to control its temperature. Provide spec sheets to this office for approval before installation. The sanitizer in the wipe cloth bucket was too strong at 400 ppm. It should test at 200 ppm to properly sanitize food contact surfaces. No test kit was available today; provide a kit to verify proper concentration. No other violation. Inspector: Wilke.

Elmhurst Elementary School, 4530 Elmhurst, inspected Sept. 12. Critical violation: Observed a wet towel in dish room on counter and no wipe bucket set up. Provide a bucket with sanitizer to store wet towels used to sanitize surfaces. No other violation. Inspector: Wilke.

Golden Phoenix Restaurant, 5201 Monroe, inspected Sept. 12. Critical violations: Observed containers of food on the floor. Store all food at least six inches off the ground to protect from contamination. Observed wet cloth on the prep table. In-use wet cloths must be kept in sanitizer to prevent cross contamination. Chlorine bleach must be at 50-100 ppm. Observed raw meat stored above rice and vegetables in the cooler. Raw eggs are stored over produce. Store raw meat and eggs below cooked items and produce to protect from cross contamination. All chemicals must be stored in one place away from food and utensils to protect from contamination. Employees using a pan of water on the cook line to rinse their hands off after frying food. All employees must wash their hands at the hand sink. The only place employees are allowed to wash their hands is at a stocked hand sink to protect from contamination. Go over hand washing rules with all employees. Ten noncritical violations. Inspector: Gottschalk.

Jan s Chili Dogs, 5668 West Central, inspected Sept. 12. Critical violation: Hotdog sauce and chili are 45 degrees in the walk-in cooler at the time of inspection. The ambient air temperature in the walk-in is 44 degrees. All equipment must hold foods cold at 41 degrees or lower at all times. Adjust or repair the walk-in cooler immediately. Five noncritical violation. Inspector: Sattler.

KFC, 1830 Laskey, inspected Sept. 12. Critical violation: Observed condensation dripping from an air vent above the chicken prep station. Immediately contact a licensed repairman to service the vent to eliminate the drip to prevent contamination. Six noncritical violations. Inspector: Wilke.

Magic Wok, 2040 Laskey, inspected Sept. 2. Critical violations: The sanitizer in the wipe bucket was low at 100 ppm. Quat sanitizer must be 200 ppm to properly sanitize. Adjust the concentration accordingly. The fried chicken holding under the heat lamps was 127 degrees. All PHF must be held hot at 135 degrees and greater to prevent the growth of bacteria. Chicken was voluntarily discarded. The noodles in the walk-in were 49 degrees and made yesterday. Noodles were out of the cooler to be weighed earlier this afternoon before inspection, and it appears they were out too long. All PHF must be held at 41 degrees or less to prevent the growth of bacteria. While portioning bags of noodles make sure to use small amounts and create an ice bath to control the noodles temperature while working with them. One noncritical violation. Inspector: Wilke.

7-Eleven Food Store #17534C, 2601 Bancroft, inspected Sept. 8. Critical violations: PHF must be held at 135 degrees or greated. You must make sure taquitos reach 135 degrees on back of roller grill before they are moved to front for customer self service. Roller grill turned to warmer setting. Clean, sanitize and air dry white board at cooler. Do this daily especially underneath. Adjust sanitizer quat dispense system to be at 200 ppm, currently at 100 ppm. Two noncritical violations. Inspector: Dacquisto.

Little Caeser s Pizza, 2535 Starr, inspected Sept. 9. Critical violation: Observed cooler not holding at proper temperatures 44 to 45 degrees. All equipment must be in good working order and hold food at 41 degrees or less. No PHF was inside cooler at time of inspection. Two noncritical violations. Inspector: Nye.

Sautter s Food Center, 5519 Main, Sylvania, inspected Sept. 10. Critical violations: Observed improper storage order of foods in several coolers. Raw chicken above bacon and ribs and ground sausage above ribs and other whole meats in self-serve meat display cooler. Practice proper storage order: chicken/poultry on bottom, ground meats, whole meats/fish then ready-to-eat foods on top. Observed temperatures of PHF in deli self-serve cooler above 41 degrees. Service clean cooler to maintain temperatures of PHF at 41 degrees or less. Will reinspect with in 48 hours. Four noncritical violations. Inspector: Tester.

Liberty of Toledo Inc. dba: Club Liberty, 813 Matzinger, inspected Sept. 9. Critical violations: Observed bottle of juice submerged in edible ice in ice bin. Food items must be protected from contamination. Do not store any items in edible ice. Observed three-compartment sink at less than 200 ppm quat. To properly sanitize, quat must be at 200 ppm. Corrected to 200 ppm at inspection. Observed unlabeled chemical bottle behind bar. Label all spray bottles with common name of ingredients. Seven noncritical violations. Inspector: Kerry Stanley.

Tasty Thai, 1515 South Byrne, inspected Sept. 10. Critical violation: Observed several rags used for wiping food contact surfaces without sanitizer. Keep wipe cloths in sanitizer solution throughout workday for wiping all food contact surfaces. No other violations. Inspector: Brady.

Steak n Shake #406, 5395 Monroe, in spected Sept. 11. Critical violations: Label all chemical spray bottles for proper identification. Observed improper holding temperatures of PHF in soup hot holding table and burger prep top cooler. PHF must be held at 41 degrees or less or 135 degrees or more to limit bacteria growth. Adjust soup steam table and keep top of burger prep top cooler closed to maintain proper temperatures. Two noncritical violations. Inspector: Tester.

Sherman Elementary School, 817 Sherman, inspected Sept. 12. Critical violations: Observed students making bare hand contact with self-service bread, and sneeze guards were up at inspection. Keep sneeze guards down to protect food from contamination. No bare hand contact is permitted with ready-to-eat foods. More assistance is needed for younger children with tongs. Consider individually wrapped bread slices. Observed hand sink blocked by cart at inspection. Do not block hand sinks. Keep accessible at all times to ensure proper hand washing. Eagle s Landing Golf Club, 5530 Bayshore, Oregon, Sept. 16. Critical violation. Observed food at improper temperatures, hamburger 45 degrees and hot dogs 45 degrees. Ruby Tuesday #4646, 2915 Glendale. Critical violations: Observed ground beef packages stored above beef steaks in walk-in cooler. Store ground beef below whole steaks. Observed scoops for mashed potatoes stored in standing water. Do not store any food utensils in standing water at room temperture to prevent bacterial growth/contamination. No other violation. Rescue Mental Health, 3350 Collingwood, inspected Sept. 2. Critical violation: PHF found at 46 degrees. All PHF must be held cold at 41 degrees or less. Two noncritical violations. Inspector: Dacquisto.

Subway, 942 Phillips, inspected Sept. 2. Critical violations: Large amount of frozen water from leaking condensor in walk-in cooler. This must be repaired. This is a repeat violation. Quat sanitizer is too strong in third sink. Unit must dispense at 200 ppm. Too much buildup in ice machine; thoroughly clean and sanitize this unit. Product being stored on floor in walk-in cooler.
