Cold holding must be maintained at 41 degrees or less to prevent growth of bacteria. Do not use this cooler to hold PHF until it is repaired. Grease buildup found on hood, equipment, and floor. Lucas County Corrections Center, 1622 Spielbusch Ave., inspected Oct. 1. Critical violations: Water is pooling where floor tiles are missing and sand bags are being used to surround area. Repair all floor tiles immediately. Several cockroaches were observed in the dish machine area. Provide invoices from pest control company and be sure all food and water is cleaned up every night. PHF was found at improper temperatures. Hot holding must be maintained at 135 degrees or higher and cold holding at 41 degrees or less. Pink build-up found in ice machine. Clean and sanitize all inside surfaces every six months. Raw shell eggs being stored above celery in walk-in cooler. Shell eggs must be stored on bottom shelf. Three noncritical violations. Chang's China Bistro, 2300 Village Drive, inspected Oct. 1. Critical violation: Disposable cup being used as scoop in dry good. Use a handled scoop to prevent hand contamination. Nine noncritical violations. Inspector: Becker.

Bay Center Recreation, 3316 Navarre, Oregon, inspected Oct. 2. Critical violation: observed chemicals above bag-in-a-box pop boxes, corrected. Inspector: Nye.

Kentucky Fried Chicken, 1010 Conant, Maumee, inspected Oct. 3. Critical violations: observed PHF holding below 135 degrees in hot boxes. The hot box does not have a temperature gauge or thermometer and is not maintaining food above 135 degrees; replace unit with a commercial unit. The fryer is leaking grease; repair immediately. Al Smith's Place, 3550 Executive Pkwy., inspected Oct. 6. Critical violation: no sanitizer in wipe cloth bucket at the cookline at time of inspection. Five noncritical violations. Inspector: Sattler.

Sammy's Too, 8165 Airport Hwy., Holland, inspected Oct. 6. Critical violation: cross contamination present in the walk-in cooler. Raw shell eggs above ready- to-eat food products; corrected. Four noncritical violations. Inspector: Becker.

Six Pack Bar and Grill, 3223 West Sylvania, inspected Oct. 6. Critical violations: quat sanitizer in the bar 3-bay sink was low at 100 ppm instead of at 200 ppm. No wipe cloth sanitizer bucket set up and a few wet towels stored on counter top. Set up bucket of sanitizer and keep the wet towels in bucket when not in use. Four noncritical violations. Inspector: Gillian Wilke.

St. Vincent Medical Center/Heart, 2400 Cherry, inspected Oct. 6. Critical violations: clean all mobile pushcarts used in kitchen and maintain clean. Clean inside box area of pizza cooler, and maintain clean. Clean gaskets and outside of salad prep top cooler. Clean and sanitize inside of ice machine. Maintain clean. Gnats observed in mop-cart wash are by drain. This drain must be free draining to eliminate buildup which allows gnats to multiply. Seven noncritical violations. Inspector: Dacquisto.

The Dog House, 4935 Lewis, inspected Oct. 6. Critical violations: chili was 56 degrees at inspection, improperly cooled. Was discarded. Kitchen handsink blocked by a trash can at inspection; lots of gnats flew out of wet towel in basin of sink. All hand sinks must be kept easily accessible and clear to promote proper hand washing. Contact licensed pest control service to address gnat problem. The 3-bay sink not properly set up for ware washing process at time of inspection. Facility is washing produce but does not have an air gapped prep sink. Contact licensed plumber to install prep sink with 2-inch air gap. One noncritical violation. Inspector: Wilke.

Wayne Trail Elementary, 1147 Seventh St., inspected Oct. 6. Critical violation: all produce must be washed prior to serving. Apples are not to be washed in a vented prep sink but must be washed at the high school in the prep sink. Inspector: Becker.

Kentucky Fried Chicken, 3506 Dorr, inspected Oct. 7. Water from the handsink is splashing onto the box of mashed potatoes stored next to sink. Install a splash guard onto handsink. Quat sanitizer in the wipe cloth bucket is too strong at 400 ppm instead of 200 ppm. Eleven noncritical violations. Inspector: Wilke.

Original Gino's Pizza, 5307 Monroe, inspected Oct. 7. Critical violation: pizza prep top cooler holding PHFs at unsafe temps. Hold at 42 degrees or less. Observed past due dates on individual containers of potato/pasta salad inside display case cooler. Discard any past due date containers. One noncritical violation. Inspector: Tester.

Robinson Junior High School, 1075 Horace, inspected Oct. 7. Critical violation: Observed product in electric hot box at 112 to 124 degrees instead of 135 degrees or greater. Adjustment made on unit. Provide information to staff on proper operation of unit and how to properly set temperature. One noncritical violation. Inspector: Debbie Dacquisto.

Eleventh Frame Fryer, 5133 Telegraph, inspected Oct. 8. Critical violations: observed food handlers without hair restraints at inspection; corrected. Observed no sanitizing wiping bucket set up at inspection; corrected. Observed mold growth on inside of ice machine; clean and sanitize. Two noncritical violations. Inspector: Kerry Stanley.

Biggby Coffee, 3160 Markway, inspected Oct. 8. Critical violation: observed lots of wet wiping towels on countertops. Store all wet towels in sanitizer bucket when not in use. Quat sanitizer in sanitizer bucket was too low; maintain at 200 ppm. Milk in working 2-door cooler was 50 degrees instead of 41 degrees or less. Do not allow food to sit out. The 2-door cooler on right is holding food at 44-45 degrees instead of 41 degrees or less. Repair immediately. Facility may not wash lettuce and tomatoes in the 3-vat sink but in a food prep sink with a 2-inch air gap. Plumbing must be installed by licensed plumber. Seven noncritical violations. Manos, 1701 Adams, inspected Oct. 8. Critical violations: date mark all refrigerated RTE foods holding longer than 24 hours which are potentially hazardous foods. Do not save gyro meat cones overnight. Gyro meat must be completely cooked and shaved in one day of service. Cool down cut meat and reheat it for service. Gyro cones cannot be properly cooled and reheated. All products in the 2-door cooler in kitchen must be held at 41 degrees or less; repair cooler. Meat walk-in cooler must hold at 41 degrees or less; thermostat adjusted at time of inspection. Six noncritical violations. Inspector: Dacquisto.

Jim & Ed's Bar and Grill, 719 Bush, inspected Oct. 8. Critical violations: observed PHFs in 3-door cooler at 46-52 degrees instead of 41 degrees or less. Do not use cooler until repaired.
